[Clinical and bacteriological significance of the Streptococcus milleri group in deep neck abscesses].

Streptococcus constellatus, S. intermedius, and S. anginosus, the three species of the S. milleri group, form part of the normal flora most commonly found in the mouth, throat, gastrointenstinal tract, and genital tract. The S. milleri group has become known as an important pathogen in abscess disease, but little attention has been paid to their role in deep neck abscesses. We have treated 9 patients with deep neck abscesses relating to the S. milleri group since 1991, and regarded this group as an important pathogen also in these abscesses. We studied the frequency of the S. milleri group iso

Species Designations Belie Phenotypic and Genotypic Heterogeneity in Oral Streptococci.

Health-associated oral Streptococcus species are promising probiotic candidates to protect against dental caries. Ammonia production through the arginine deiminase system (ADS), which can increase the pH of oral biofilms, and direct antagonism of caries-associated bacterial species are desirable properties for oral probiotic strains. ADS and antagonistic activities can vary dramatically among individuals, but the genetic basis for these differences is unknown. We sequenced whole genomes of a diverse set of clinical oral Streptococcus isolates and examined the genetic basis of variability in AD

Multifocal Pulmonary Granular Cell Tumor Presenting with Postobstructive Pneumonia.

Granular cell tumor (GCT) is a neoplasm of Schwann cell origin. Its presence in the aerodigestive tract is uncommon and becomes a diagnostic challenge on initial presentation. Our case is of a 59-year-old woman who presented to the emergency department with a history of productive cough and dyspnea associated with fever and chest pain. An initial chest X-ray (CXR) showed a right middle lobe consolidation with follow-up Computed Tomography (CT) scan showing a mass in the right bronchus. Bronchoscopy revealed a polypoid, sessile granular mass in the right bronchus intermedius with multiple white

Induction of experimental endocarditis by continuous low-grade bacteremia mimicking spontaneous bacteremia in humans.

Transient high-grade bacteremia following invasive procedures carries a risk of infective endocarditis (IE). This is supported by experimental endocarditis. On the other hand, case-control studies showed that IE could be caused by cumulative exposure to low-grade bacteremia occurring during daily activities. However, no experimental demonstration of this latter possibility exists. This study investigated the infectivity in animals of continuous low-grade bacteremia compared to that of brief high-grade bacteremia. Rats with aortic vegetations were inoculated with Streptococcus intermedius, Stre

Cervical Epidural Abscess Secondary to a Post-Traumatic Hematoma, Successfully Treated with Adjunctive Hyperbaric Oxygen Therapy: A Case Report.

Background: Spinal epidural abscess (SEA) is a rare but challenging disease. Hyperbaric oxygen therapy (HBOT) is used as an adjunctive therapy for SEA in a limited number of hospitals; however, its efficacy has not been well described. Case: A 70-year-old man presented at our hospital with cervical pain, fever, and impaired right shoulder movement. The patient fell after drinking 17 days prior to the presentation. He was diagnosed with an SEA secondary to a spinal epidural hematoma caused by a prior injury. The patient also had dental caries, and his blood culture was positive for Streptococcu

Cre-inducible human CD59 mediates rapid cell ablation after intermedilysin administration.

Cell ablation is a powerful tool for studying cell lineage and/or function; however, current cell-ablation models have limitations. Intermedilysin (ILY), a cytolytic pore-forming toxin that is secreted by Streptococcus intermedius, lyses human cells exclusively by binding to the human complement regulator CD59 (hCD59), but does not react with CD59 from nonprimates. Here, we took advantage of this feature of ILY and developed a model of conditional and targeted cell ablation by generating floxed STOP-CD59 knockin mice (ihCD59), in which expression of human CD59 only occurs after Cre-mediated re

Bacterial Brain Abscesses in a Patient With Transposition of the Great Arteries and Interventricular Communication.

Brain abscesses are localized infections in the brain's parenchyma, characterized by inflammation, pus formation, and the development of a surrounding capsule. These lesions typically occur due to underlying factors such as immunosuppression, ear and sinus infections, and contamination during neurosurgery. While brain abscesses are a life-threatening complication of cyanotic heart defects, they are rarely reported, with only sporadic cases previously documented. This article presents the case of an eight-year-old male patient with an uncorrected transposition of the great arteries, who was eva

Neuropathologic changes associated with systemic bacterial infection in 28 dogs.

Although systemic bacterial infection (SBI) is a common cause of sepsis and death in dogs, the neuropathology of canine SBI has been poorly characterized. Here we describe the neuropathologic features of SBI in a retrospective series of 28 dogs. The mean age of affected dogs was 5.5 y, and there was no sex or breed predisposition. Gross lesions in the brain were reported in 13 cases (46%) and consisted mainly of leptomeningeal hemorrhages in 10 of these cases (77%). Associated extraneural lesions included suppurative mitral valve endocarditis (12 cases; 43%) and pneumonia (10 cases; 36%). The

[When genetics matters: an unusual case of left-sided hemiparesis].

We report the case of a 35-year-old woman who has been admitted to our emergency room because of the sudden onset of a left-sided hemiparesis. The physical examination showed disseminated teleangiectases on the upper and lower lip, on the mucosal surface of the tongue and on the skin. The cerebral CT scan presented a right-sided fronto-parietal lesion, more likely an abscess, confirmed on surgical removal. A meticulous family history showed the presence of similar clinical features among several family members, raising the certainty about the presence of the Rendu-Osler-Weber disease. This cas

The Heart-Brain Bypass: Paradoxical Septic Embolism Through a Patent Foramen Ovale (PFO).

Brain abscesses are rare sequelae of paradoxical septic embolism from cardiac sources, particularly in the absence of valvular endocarditis. Diagnosis remains challenging, and a high index of clinical suspicion is often required. We present a 65-year-old female with a past medical history of hypertension and dyslipidemia with a chief complaint of altered mental status. She was found to have left-sided focal seizures and dysarthria. Computed tomography (CT) of the head with and without contrast, with subsequent confirmatory magnetic resonance imaging (MRI) of the head with contrast, revealed a

Intracranial Complications Following Acute Rhinosinusitis in a Pediatric Patient Requiring Surgery: A Case Report.

Intracranial complications secondary to acute sinusitis in pediatric patients are rare but potentially life-threatening. Initially, the symptoms are nonspecific, which can result in a delayed diagnosis. If a patient presents with persistent fever and severe headache, an intracranial complication should be suspected and promptly investigated using imaging techniques such as contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI). Here, we describe a case in which both a frontal epidural abscess and an interhemispheric subdural empyema developed following acute rhinosinus

Evaluating the feasibility, sensitivity, and specificity of next-generation molecular methods for pleural infection diagnosis.

Pleural infections are common and associated with substantial healthcare costs, morbidity, and mortality. Accurate diagnosis remains challenging due to low culture positivity rates, frequent polymicrobial involvement, and non-specific diagnostic biomarkers. Here, we undertook a prospective study examining the feasibility and performance of molecular methods for diagnosing suspected pleural infection. We prospectively characterized 26 consecutive clinically suspected pleural infections, and 10 consecutive patients with suspected non-infective pleural effusions, using shotgun metagenomics, bacte

Management and outcomes of craniectomy and cranioplasty in pediatric intracranial infections: a multicenter cohort study.

BACKGROUND: In recent years, there has been a noticeable increase in severe intracranial infections among pediatric patients, often requiring craniotomy or decompressive craniectomy. However, determining the optimal timing and material for cranioplasty in pediatric patients presents an ongoing challenge. METHODS: We conducted a multicenter retrospective cohort study of pediatric patients treated between 2013 and 2024. A total of 76 children with intracranial infections were included; 37% of those underwent decompressive craniectomy. Of these, 21% subsequently received cranioplasty using vario

Rare Complication of Interventional Radiology-guided Arterial Embolization of the Gastroduodenal Artery in the Setting of Acute Gastrointestinal Bleed: Migrated Coils in the Duodenum.

A 91-year-old male presented to the emergency room with hemodynamically significant upper gastrointestinal bleeding. The patient underwent an esophagogastroduodenoscopy (EGD), which showed frank blood in the duodenum interfering with the visualization. Hence, the patient underwent urgent interventional radiology (IR)-guided arteriogram and embolization. An EGD done 48 hours later showed a giant, non-bleeding, cratered duodenal ulcer with a visible vessel and vascular coils partially protruding into the duodenal bulb lumen. The patient had no evidence of bleeding post embolization. The patient
