Therapeutic approach to patients with a lower-pitched voice after thyroidectomy.

BACKGROUND: A lower-pitched voice is one of the most common voice alterations after thyroidectomy without laryngeal nerve injury. The aim of this study was to evaluate the acoustic and stroboscopic changes and the treatment outcomes in patients with a lower-pitched voice with the goal of eventually establishing a therapeutic guideline. METHODS: Patients with a lower-pitched voice were selected according to the results of acoustic analysis among thyroidectomized patients. According to their pitch-gliding ability, patients were classified into a "gliding group" and "nongliding group," and direc

Frame by frame analysis of glottic insufficiency using laryngovideostroboscopy.

OBJECTIVES/HYPOTHESIS: Glottic insufficiency (GI) can be either grossly obvious or subtle in its presentation. Subtle GI is demonstrated by various Laryngovideostroboscopic (LVS) clues, including complete but "short" phase closure of the true vocal folds (VFs) during the glottic cycle. We used the frame by frame analysis (FBFA) technique to evaluate its effectiveness in objectively contributing to the diagnosis of subtle GI in patients with atrophic and/or paretic VFs. This article intends to formally present the methods and intentions of the FBFA technique and report our findings using FBFA o

Impact of thyroidectomy without laryngeal nerve injury on vocal quality characteristics: an objective multiparameter approach.

OBJECTIVES/HYPOTHESIS: The main purpose of this study was to determine the impact of thyroidectomy on the subjective and objective vocal quality using the Dysphonia Severity Index. It was hypothesized that objective measures of vocal function and other vocal characteristics would change (temporarily or permanently) from the presurgical to the postsurgical conditions, even with the entire preservation of the laryngeal nerve, due to the surgical approach and other influencing factors. STUDY DESIGN: Prospective study in which 44 subjects were studied before (1 week) and three times post-thyroide

Bamboo node: primary vocal fold lesion as evidence of autoimmune disease.

Descriptions of vocal fold lesions related to autoimmune diseases are rare in the literature, and focus mainly on rheumatoid nodules. This is the first report in which autoimmune diseases were promptly suspected by the observation of a unique white transverse submucosal lesion in the vocal fold during clinical examination. This lesion, reported only in autoimmune disease, has been called the bamboo node and its features are different from those of rheumatoid nodules. We report here on two patients who did not have a diagnosis of systemic disease before investigation of their main complaint of

Period and glottal width irregularities in vocally normal speakers.

Vocal fold vibratory irregularities can be caused by multiple factors. The underlying mechanism for irregularities is a difference in the mass or tension of the vibratory portion of the left versus the right vocal fold. The purpose of this study is to preliminarily investigate the prevalence of vocal fold vibratory irregularities in vocally normal speakers via high-speed videoendoscopy (HSV). Two aspects of irregularity, glottal width and period irregularity, were investigated in 52 vocally normal speakers undergoing videostroboscopy and HSV. The videostroboscopy and HSV recordings were proces

Episodic paroxysmal laryngospasm: voice and pulmonary function assessment and management.

Episodic paroxysmal laryngospasm (EPL) is a sign of laryngeal dysfunction, often without a specific organic etiology, which can masquerade as asthma, vocal fold paralysis, or a functional voice disorder. The intermittent respiratory distress of EPL may precipitate an apparent upper airway obstructive emergency, resulting in unnecessary endotracheal intubation, cardiopulmonary resuscitation, or tracheostomy. During 27 months, seven women and three men, age 30-76 years, were assessed by a high diagnostic index of suspicion, an intensive history including psychosocial factors, physical examinatio

Contribution of Glottic Insufficiency to Perceived Breathiness in Classically Trained Singers.

Breathiness in the singing voice is problematic for classical singers. Voice students and singing teachers typically attribute breathiness to breath management issues and breathing technique. The present study sought to determine whether glottic insufficiency may also contribute to breathiness in a singer's voice. Studies have revealed a relationship between insufficient vocal fold closure and inefficiency in the speaking voice. However, the effect of insufficient vocal fold closure on vocal efficiency in singers has yet to be determined. Two groups of voice students identified with and withou
