[Tuberculosis of the submandibular gland: a case report].

INTRODUCTION: Tuberculosis remains an important public health problem in France. After a certain decline, its incidence has remained unchanged since 1990. 30% of tuberculosis cases are extra-pulmonary form, most of them concern nodes especially in cervical areas. CASE REPORT: This case of submandibular tuberculosis illustrates diagnostic and therapeutic difficulties. DISCUSSION: There are few clinical signs or laboratory clues suggestive of tuberculosis, however a surgery cannot be avoided and must be performed in combination with antituberculosis chemotherapy.

A case of carcinosarcoma arising in the submandibular gland.

A 54-year-old man presented with an 8-year history of a hard asymptomatic mass of the left submandibular area. Total excision of the left submandibular gland with radical neck dissection was performed under a diagnosis of a submandibular tumor, probably a malignant mixed tumor. The pathologic diagnosis was carcinosarcoma consisting of carcinomatous and sarcomatous elements. The epithelial component was composed of squamous cell carcinoma, undifferentiated carcinoma, and adenocarcinoma. The nonepithelial component was composed of chondrosarcoma, osteosarcoma, spindle cell sarcoma, rhabdomyosarc

Hydatid cyst in the duct of the submandibular gland.

The case of a hydatid cyst located in the duct of Wharton of a 56-year-old farmer is reported not only because of the unusual location of the disease, but also because the clinical manifestations and surgical findings of the hydatid cyst strongly resembled those of a salivary calculus in the duct of Wharton. The patient presented with a non-painful swelling of the right submandibular region. The mass, which appeared 5 years before seeking medical advice, augmented progressively during the first year after its onset and remained stable during the next 4 years. The subsequent surgical excision o

Clock gene expression in the submandibular glands.

Clock genes, which mediate molecular circadian rhythms, are expressed in a circadian fashion in the suprachiasmatic nucleus and in various peripheral tissues. To establish a molecular basis for circadian regulation in the salivary glands, we examined expression profiles of clock-related genes and salivary gland-characteristic genes. Clock-related genes-including Per1, Per2, Cry1, Bmal1, Dec1, Dec2, Dbp, and Reverbalpha-showed robust circadian expression rhythms in the submandibular glands in 12:12-hour light-dark conditions. In addition, a robust circadian rhythm was observed in amylase 1 mRNA

A case of Kuttner tumor of the submandibular gland.

Kuttner tumor (known as chronic sclerosing sialadenitis) is a totally benign inflammatory lesion and to date there has been no report of malignancy. However, because of its clinical features, the clinical diagnosis is often that of a salivary gland neoplasm. We report a case of Kuttner tumor and discuss its differential diagnosis, especially the pathological diagnosis. Kuttner tumor may be mainly due to T lymphocyte immune reaction and of itself is a benign lesion, but it may provide a state in which a malignant lesion arises.

Excision of the submandibular gland by an intraoral approach.

To improve the outcome in patients with benign diseases of the submandibular gland, we have developed an entirely intraoral technique for excision of the submandibular gland. This procedure is anatomically safe and can be performed with minimal morbidity. We believe the essential surgical steps are as follows: (1) infiltration with Xylocaine plus epinephrine with an adequate waiting period for hemostasis; (2) careful identification of the submandibular duct/lingual nerve relationship; (3) anterior retraction of the mylohyoid muscle to expose the superficial lobe; (4) superiorly directed, extra

[Metastasis of pancreatic adenocarcinoma to the submandibular gland].

Metastases are rarely found in salivary glands. They are exceptional in the submaxillary gland. The authors report a case of a metastatic tumour in the submaxillary gland. The primitive tumour was found in the pancreatic head.

Morphology and histochemistry of rabbit submandibular glands.

The rabbit submandibular glands are of the heterocrine type. The most distal part of the ductal system consists of secretory endpieces, which are constructed mainly of two morphologically and histochemically different and distinct structures: distally the seromucous acini and proximally the serous tubuli. The cells of the seromucous acini are strongly AB pH 2.5 positive, AB pH 1.0 negative and PAS negative, indicating production of acid glycoproteins--mainly sialomucin. The cells of the serous tubuli are AB pH 2.5 negative and strongly PAS positive, indicating the presence of neutral glycoprot

A potent vasoconstrictor in the rat submandibular gland.

We detected a novel vasoconstrictor in an arginine esterase fraction separated from fractions containing tonin and other esterases that were obtained from a rat submandibular gland extract. When tested on isolated rabbit aorta rings, the substance caused dose-related contractions that were slow in onset, long-lasting, and difficult to reverse by rinsing. The substance acts directly on vascular smooth muscle, since preincubation with plasma or intact endothelium is not required. The fact that the constrictor was destroyed by heat and incubation with pronase suggests that it is a protein. Molecu

[A case of a giant calculus of the submandibular gland].

The case of salivary calculi of the submandibular gland and of its duct is very frequent, anyway, in this field it is very unusual to find a calculus heavier than 1 g. In this report the Authors relate about a particularly interesting case of a giant calculus. This kind of calculus is very interesting because of its large size, its position and some morphological peculiarities. In the end the experts confirm the necessity, in these cases, of a surgical intervention, including the extirpation of the gland.

Computed tomography of the submandibular salivary glands.

Computed tomography offers a convenient method for the evaluation of the submandibular salivary glands and their surroundings. Normal measurements were established based on 38 normal subjects. The visualization of the ductal system in the gland and the relationship of the gland to the platysma muscle were recorded. The CT scans of 12 patients with a tongue base carcinoma and 4 patients with a verified obstruction of the submandibular duct were also reviewed to establish how the glands are affected by extrinsic lesions. The normal glands measured 28 x 18 mm (+/- 5 mm) in the axial plane. Two pa

Endoscope-assisted submandibular sialadenectomy: a new minimally invasive approach to the submandibular gland.

PURPOSE: Although submandibular sialadenectomy with a minimal incision improves overall cosmetic outcomes, visualization of the surgical field exposure is relatively limited as compared with that in the conventional procedure. To overcome this limitation, we applied the endoscope system to submandibular sialadenectomy. The aim of this study was to evaluate the technical feasibility and potential role of our endoscope-assisted submandibular sialadenectomy (EASS). MATERIALS AND METHODS: We performed EASS on 5 patients, 3 of whom had intraparenchymal sialolithiasis and 2 of whom had pleomorphic

[Reactions of the rat submandibular salivary gland to injury of the contralateral gland].

After burn or resection of one submandibular salivary gland the contralateral gland responded by an increase of proliferation. The maximal mitotic index was observed 48 hours after the resection and 72 hours after the burn. Burn of the salivary gland initiated a stable, but weak compensatory hypertrophy of the contralateral gland. Hypertrophy of the gland was accompanied by an increase in the size of the cells and the nuclei whose area enlarged by 10 and 17%, respectively. Resection of the salivary gland caused an increase in the weight of the intact contralateral gland only at the early perio

Hyperplastic sublingual gland in association with a congenitally missing ipsilateral submandibular gland.

A congenitally absent unilateral submandibular gland is a rare condition. We present such a case with an associated ipsilateral benign hyperplastic sublingual gland. The enlarged sublingual gland may well represent a compensatory response to the missing submandibular gland, but it is known that sublingual gland tumours are malignant in approximately 90% of cases and so a sublingual gland swelling is viewed with a high degree of suspicion. Clinical diagnosis of sublingual hyperplasia becomes a challenge if there is a missing unilateral submandibular gland, but a full investigation is important

Glycogen shunt is essential for submandibular gland morphogenesis.

Glycogen metabolism is an important pathway in energy metabolism, and when cells require energy, glycogen is broken down into glucose-1-phosphate through glycogenolysis, which serves as an intracellular energy source. Recently, the concept of "glycogen shunt" has been proposed to regulate the synthesis, accumulation, and breakdown of glycogen to provide glucose at the appropriate time, particularly in organogenesis. This study demonstrated the timing of the localization of glycogen and molecules related to glycogen metabolism during submandibular gland development using embryonic and postnatal
