11C-acetate PET for early prediction of sunitinib response in metastatic renal cell carcinoma.

Sunitinib is an oral multitargeted tyrosine kinase inhibitor with antiangiogenic properties used for treatment of renal cell carcinoma and gastrointestinal stromal tumors at a dose of 50 mg/day consecutively for 4 weeks followed by 2 weeks off per cycle. At present, no data are available on the early prediction of sunitinib response in renal cell carcinoma. We report a clinical case of a patient with metastatic renal cell carcinoma diagnosed with 11C-acetate PET and conventional CT and treated with sunitinib. Partial and complete remission documented by CT was preceded by early functional tumo

GFPT2 drives sunitinib resistance of renal cell carcinoma via enzyme-dependent and -independent manners.

Intrinsic resistance to sunitinib in advanced renal cell carcinoma (RCC) remains a major barrier to improving patient survival outcomes. However, the molecular mechanisms driving this resistance remain incompletely elucidated. In this study, we first observed elevated glutamine levels in sunitinib-resistant RCC models; notably, glutamine deprivation substantially impaired the growth and proliferation of RCC cells. We further demonstrated that abnormal upregulation of GFPT2-a key enzyme in glutamine metabolism-was associated with reduced sunitinib sensitivity and enhanced drug resistance in RCC

ALDH1L2 suppresses ferroptosis-associated responses and reduces sunitinib sensitivity in renal cell carcinoma organoids.

BACKGROUND: Sunitinib is an important therapeutic option for renal cell carcinoma (RCC), but reduced response to sunitinib still limits its clinical benefit. This study explored the mechanism underlying reduced sunitinib sensitivity in RCC and aimed to identify potential strategies to improve sunitinib response. METHODS: RCC organoids were established and characterized, and sunitinib-responsive genes were screened by transcriptomic analysis. Public datasets and clinical RCC samples were used to evaluate the clinical relevance of ALDH1L2. ALDH1L2 overexpression and knockdown were performed in

Complete regression of choroidal metastases from renal cancer under sunitinib with grade 3 hyperbilirubinemia.

Sunitinib is a small molecule tyrosine kinase inhibitor that is taken by mouth. It works against several kinases, such as vascular endothelial growth factor receptor, c-Kit, and platelet-derived growth factor receptor. We report a case of total disappearance of choroid metastases from renal cell carcinoma after 6 months of sunitinib, with grade 3 hyperbilirubinemia.

Hepatitis D reactivation in a patient with metastatic renal cell carcinoma receiving sunitinib therapy.

BACKGROUND: Cytotoxic and immunosuppressive therapies for cancer treatment may allow hepatitis reactivation. Hepatitis due to viral hepatitis reactivation is detected in 14%-25% of hepatitis B surface antigen (HBsAg)-positive cancer patients undergoing anticancer treatments. Drug toxicity may be confused with hepatitis reactivation, which may cause a delay in diagnosis. CASE REPORT: A 60-year-old man with metastatic renal cell carcinoma was treated with sunitinib. Sixteen months after sunitinib inception, liver enzymes were elevated and viral hepatitis reactivation was detected as hepatitis d

Angiotensin Inhibitors as Treatment of Sunitinib/Pazopanib-induced Hypertension in Metastatic Renal Cell Carcinoma.

BACKGROUND: Research suggests that baseline use of angiotensin system inhibitors (ASIs) improves outcome in patients with metastatic renal cell carcinoma (mRCC), but it remains unknown whether the type of antihypertensive medication used to initiate management at onset of treatment-induced hypertension (HTN) is associated with outcome. We evaluated the association of ASIs and outcome among patients with mRCC treated with first-line tyrosine kinase inhibitors (TKIs). PATIENTS AND METHODS: We identified 303 consecutive patients with mRCC who were treated with sunitinib or pazopanib in a single

CircLAMA5 traps Sam68 in the cytoplasm to regulate alternative splicing and drive sunitinib resistance in ccRCC.

Tyrosine kinase inhibitor (TKI) resistance remains a major challenge in the treatment of clear cell renal cell carcinoma (ccRCC), yet the underlying molecular mechanisms are not fully understood. Here, we identify circular RNA circLAMA5 (hsa_circ_0061093) as a critical driver of sunitinib resistance in ccRCC. CircLAMA5 is significantly upregulated in sunitinib‑resistant cell lines and cell‑derived xenografts (CDX-R). Functional studies demonstrate that circLAMA5 enhances ccRCC cell survival, colony formation, and apoptosis evasion under sunitinib treatment both in vitro and in vivo. Mechanisti

Unravelling the cardioprotective effects of calcitriol in Sunitinib-induced toxicity: A comprehensive in silico and in vitro study.

Sunitinib (SUN), a drug used to treat advanced renal cell carcinoma and other cancers, causes cardiotoxicity. This study aimed to identify a potential drug candidate to counteract SUN-induced cardiotoxicity. We analysed real-world data from adverse event report databases of existing clinically approved drugs to identify potential candidates. Through in silico analyses and in vitro experiments, the mechanisms of action were determined. The study identified calcitriol (CTL), an active form of vitamin D, as a promising candidate against SUN-induced cardiotoxicity. In H9c2 cells, SUN decreased cel

Optimal Sunitinib Dosing Based on Platelet Count in an Older Patient With Gastrointestinal Stromal Tumor.

BACKGROUND/AIM: Sunitinib, a second-line treatment for gastrointestinal stromal tumors (GIST), is commonly associated with adverse events (AEs) such as myelosuppression in older patients. However, an optimal dosing schedule has not been established. This study evaluated the therapeutic efficacy and toxicity management of sunitinib administration based on platelet count. CASE REPORT: An 83-year-old man receiving imatinib for rectal GISTs was switched to sunitinib (initially 50 mg/day) due to imatinib failure. After eight days, he developed grade 2 anorexia and nausea, prompting a dose reductio

In vitro assessment of the role of endoplasmic reticulum stress in sunitinib-induced liver and kidney toxicity.

Sunitinib, a multi-targeted tyrosine kinase inhibitor, is prescribed for the treatment of metastatic gastrointestinal stromal tumors, advanced metastatic renal cell carcinoma, and pancreatic neuroendocrine tumors. Hepatotoxicity and nephrotoxicity are significant adverse effects of sunitinib administration; however, there is limited information regarding the molecular mechanisms of these adverse effects. The aim of the present study was to elucidate the role of endoplasmic reticulum stress in hepatotoxicity and nephrotoxicity induced by sunitinib. In addition to endoplasmic reticulum stress, o

A Phase II, Single-arm Trial of Sunitinib and Erlotinib in Advanced Renal Cell Carcinoma.

BACKGROUND: Overexpression of the epidermal growth factor receptor (EGFR) and its ligands occur frequently in renal cell carcinoma (RCC). Combined vascular endothelial growth factor receptor (VEGF-R) and EGFR inhibition may overcome resistance to VEGF-R inhibitor monotherapy. We performed a dose-escalation phase II study of sunitinib plus erlotinib in advanced renal cell carcinoma. PATIENTS AND METHODS: Patients with metastatic clear cell or papillary RCC were eligible. Prior therapy was allowed except sunitinib or erlotinib. Three dose levels of erlotinib (50, 100, 150 mg daily) were evaluat

Time trends for drug specific adverse events in patients on sunitinib; implications for remote monitoring.

INTRODUCTION: Sunitinib is a multi-targeted receptor tyrosine kinase inhibitor used to treat metastatic renal cell carcinoma (mRCC). Patients on sunitinib do require regular in-person appointments to monitor for adverse events (AEs). Given the Covid-19 pandemic, regular in-person visits expose patients to an increased risk of infection in addition to potentially preventable travel costs. This study investigated the feasibility of implementing a remote monitoring strategy for patients being treated with sunitinib for mRCC by examining the time trends of AEs. MATERIALS AND METHODS: In this retr

Negative Effect of Immediate Sunitinib Interruption on Survival in Patients With Metastatic Renal Cell Carcinoma.

BACKGROUND: Treatment modification due to adverse events reduces the dose intensity in cancer treatment. The prognostic impact of sunitinib treatment interruption within the initial period of therapy for metastatic renal cell carcinoma (mRCC) remains unknown. PATIENTS AND METHODS: We retrospectively evaluated 97 patients with mRCC treated with first-line sunitinib treatment. The patients were classified into two groups according to the presence of treatment interruption (TI) within the initial two cycles. The prognostic impact of TI was analyzed using the Kaplan-Meier method and log-rank test
