The effect of cocaine and amphetamines on vital signs in trauma patients.

The object of this study was to determine if the presence of sympathomimetics (cocaine, benzoylecgonine, or amphetamine), detected by routine urine toxicology screen, affect the initial presenting pulse, blood pressure, respiratory rate, or level of consciousness in trauma patients. A retrospective chart review was performed on 1,679 patients enrolled in an urban level 1 trauma registry between October 1987, and July 1992, for whom complete toxicology data were available. There were no clinically significant differences in the vital signs of patients with positive toxicology screens and those

Are There Pharmacotherapeutic Options for Underactive Bladder?

Currently, underactive bladder, the symptom-based correlate of the urodynamic diagnosis of detrusor underactivity, has no effective drug treatments. Use of sympathomimetics, targeting cholinergic receptors, is not supported by current evidence. Several potential targets are the subject of ongoing investigation.

Toxic effects of drugs used in the ICU. Toxic effects of bronchodilators.

The critical care clinician commonly encounters patients with acute bronchospasm. Therapy includes a multidrug regimen of sympathomimetics, anticholinergics, methylxanthines, and corticosteroids. The best use of these agents is predicated on knowledge and avoidance of drug toxicities. This article reviews toxic effects of these bronchodilator agents.

Clinical implications of differences in pharmacodynamic action of polar and nonpolar cardiac glycosides.

The principal effects of cardiac glycosides probably can be classified as parasympathomimetic or sympathomimetic. Data from animals and from man suggest that polar cardiac glycosides, such as ouabain and digoxin, possess greater parasympathomimetic (vagal) cardiac effect for a given amount of sympathomimetic (positive inotropic) cardiac effect than do less polar cardiac glycosides, such as digitoxin. Polar glycosides therefore offer some advantage in uncomplicated paroxysmal atrial tachycardia and in uncomplicated atrial flutter and atrial fibrillation when the principal desired effect is redu

The neural substrates for the motor-activating properties of psychostimulants: a review of recent findings.

Several different classes of pharmacological agents produce syndromes of behavioral activation in humans and infrahumans. While many of these agents, including direct and indirect sympathomimetics, methylxanthines, opiates and several neuropeptides have very distinct neurochemical profiles, it is not clear whether their behavioral stimulant action results from their action on a common neural substrate, or instead from their action on parallel but separate activation "circuits.' Using photocell measurements of motor activity in rats, it has been possible to demonstrate that some agents with ver

Day-night differences in the sensitivity of adrenoceptors in the Syrian hamster pineal gland: an in vivo iontophoretic study.

Investigations on the regulation of pineal melatonin synthesis in the Syrian hamster revealed distinct differences compared to this well-understood mechanism in rat. E.g., a circadian profile of pineal norepinephrine (NE) is absent, there is no beta-adrenoceptor sensitivity during daytime and adrenergic receptor supersensitivity is not easily achieved. To elucidate the action of NE on pineal receptor sites, the effects of iontophoretic application of adrenergic compounds on spontaneous electrical discharge rates of pinealocytes were investigated during day- and nighttime. Following application

Detrusor instability. Is the bladder the cause or the effect?

Seventy-two patients had a clinical and urodynamic diagnosis of detrusor instability. All were given four weeks of anticholinergics (oxybutinin, 5 mg three times a day) and had their evaluation repeated. Forty-one patients (57%) responded favorably to the anticholinergics, and 31 (43%) did not. The 31 discontinued the anticholinergics and were placed on sympathomimetics (phenylpropanlamine, 25 mg two times a day) for another four weeks before repeating their evaluation. Based on urethrocystometry, two groups were identified: group I (n = 44) had bladder contraction that preceded any urethral p

Ventricular tachycardia after naloxone administration in an adolescent.

Naloxone is a medication with a largely benign safety profile that is frequently administered in the emergency department to patients presenting with altered mental status. Ventricular tachycardia has been reported after naloxone administration in adult patients with prior use of opiate or sympathomimetic medications. However, no such reports exist in the pediatric population or in patients who have no known history of opiate or sympathomimetic medication use. We describe a case of ventricular tachycardia after naloxone administration in a 17-year-old male with no known prior use of opiate or

L-Type Cav1.3 and HCN Channels Mediate Heart Rate Acceleration by Catecholamines.

BACKGROUND: The ionic mechanism by which catecholamines increase the heart rate is incompletely understood. In this study, we have assessed the roles of sinoatrial node L-type Cav1.3 (α1D) Ca2+ channels, phosphorylation of L-type channel regulatory partner protein Rad (Ras-related RGK GTP-binding protein), and cAMP-dependent regulation of hyperpolarization-activated HCN (hyperpolarization-activated cyclic nucleotide-gated) channels. METHODS: We studied β-adrenergic regulation of heart rate and sinoatrial pacemaker activity in mice lacking Cav1.3 channels and in mice expressing dihydropyridine

The beta-adrenergic agonist salbutamol modulates neuromuscular junction formation in zebrafish models of human myasthenic syndromes.

Inherited defects of the neuromuscular junction (NMJ) comprise an increasingly diverse range of disorders, termed congenital myasthenic syndromes (CMS). Therapies acting on the sympathetic nervous system, including the selective β2 adrenergic agonist salbutamol and the α and β adrenergic agonist ephedrine, have become standard treatment for several types of CMS. However, the mechanism of the therapeutic effect of sympathomimetics in these disorders is not understood. Here, we examined the effect of salbutamol on NMJ development using zebrafish with deficiency of the key postsynaptic proteins D

Recommended and prescribed symptomatic treatment for acute maxillary sinusitis in Finnish primary care.

We studied the use of symptomatic medication in the treatment of acute maxillary sinusitis (AMS) in primary care and whether this use is in accordance with national guidelines. The data was collected annually in the Antimicrobial Treatment Strategies (MIKSTRA) Program in 30 primary health care centres throughout Finland during one week in November in the years from 1998 to 2002. Physicians and nurses collected the data about the diagnoses, prescription-only medicines and over the counter medicines prescribed or recommended for all patients with an infection during the study weeks. The MIKSTRA

Impaired blood pressure response to norepinephrine in a case of insulin-dependent diabetes mellitus--improvement with a beta-adrenergic antagonist.

A case of juvenile-onset insulin-dependent diabetes mellitus in a 30-year-old male patient is reported. He was admitted to the hospital because of severe diabetic neuropathy, predominantly in the lower extremities. Signs of autonomic neuropathy were not evident but the patient had severe orthostatic hypotension. Circulating catecholamine concentrations were normal; however, the blood pressure response to infused norepinephrine was reduced ten-fold compared to a group of normals. An improvement of the blood pressure response to sympathomimetic drugs was accomplished during the simultaneous admi

Ocular Pharmacology.

Medications are a common treatment for glaucoma. Pharmacologic agents include sympathomimetics, beta-blockers, miotics (direct acting and cholinesterase inhibitors), carbonic anhydrase inhibitors, and prostaglandin agonists. Other agents include hyperosmotic agents and nonsteroidal anti-inflammatory drugs. Members of the ophthalmic health-care team, with heightened attention to patient safety, prescribe, administer, monitor side effects, and educate patients on glaucoma medications.

Diabetic ketoacidosis in pregnancy.

Diabetic ketoacidosis, though uncommon, is a very serious complication in obstetrics. We reported 5 cases, which in retrospect could all have been prevented from occurring. Either a strong family of diabetes mellitus or presence of glycosuria were present in 4 cases. In 3 cases, the use of sympathomimetic and corticosteroid was the predisposing factor. Clinicians should have an index of suspicion in patients at risk and it might be prudent to screen for diabetes where the use of sympathomimetic and corticosteroid.

Fluid Resuscitation for Refractory Hypotension.

Hypotension is a common occurrence, especially in anesthetized patients and in critical patients suffering from hypovolemia due to shock and sepsis. Hypotension can also occur in normovolemic animals, anesthetized or conscious, under conditions of vasodilation or decreased cardiac function. The main consequence of hypotension is decreased organ perfusion and tissue injury/dysfunction. In the human literature there is no consensus on what is the threshold value for hypotension, and ranges from < 80 to < 100 mmHg for systolic blood pressure and from < 50 to < 70 mmHg for mean arterial blood pres

Effect of drugs on the stimulation threshold of the human heart.

Clinical measurements were made of the effects of various drugs on the stimulation threshold of human heart [right ventricle]. Of antiarrhythmic drugs, a marked and prolonged elevation of the stimulation threshold was produced by procainamide, quinidine, and aimaline. Relatively short-lasting elevation was elicited by local anaesthetics [trimecaine, lidocaine]. Beta-blockers were little effective. No detectable effect on the stimulation threshold was produced by phenytoin and digitalis glucosides from the group of antiarrhythmics. The deepest and most prolonged depression of the stimulation th

Pharmacokinetic interactions between theophylline and other medication (Part II).

Part I of this article, which appeared in the previous issue of the Journal, covered the effects or lack of effects on theophylline clearance of sympathomimetics, corticosteroids, antihistamines and other antiallergy drugs, antimicrobial agents, phenytoin, carbamazepine, barbiturates, antacids and activated charcoal. In Part II, this discussion is extended to the effects of other agents. Overall summaries, both textual and tabular, appear in Part I.

Tracheal phenol red secretion, a new method for screening mucosecretolytic compounds.

A simple method for screening drugs that influence tracheobronchial secretion has been described. After i.p. application of a phenol red solution, part of the dye is secreted into the tracheal lumen. This basal secretion is increased by both parasympathomimetics and sympathomimetics. In addition, expectorants are also capable of increasing tracheal phenol red output. Therefore this method is suitable for the study of drugs that may be capable of influencing tracheobronchial secretion.

Drugs for cough and cold symptoms in hypertensive patients.

Most cold preparations contain sympathomimetic decongestants in combination with antihistamines, anticholinergics or expectorants. All of the sympathomimetic decongestants have the potential to elevate blood pressure, yet little information has been reported on their use in patients with hypertension. Phenylephrine and phenylpropanolamine stimulate alpha-adrenergic receptors, causing elevation of blood pressure and a reflex decrease in heart rate. Ephedrine and pseudoephedrine stimulate both alpha and beta receptors, elevating blood pressure with an increase or no change in heart rate. Because
