Idiopathic Syringomyelia: Diagnostic Value of Cranial Morphometric Parameters.

Background: Identifying the etiological factors of syringomyelia, which can cause progressive neurological deficits in the spinal cord, is critically important for both diagnosis and treatment. This study aimed to assess the cranial morphometric features of patients with idiopathic syringomyelia by conducting comparative analyses with individuals diagnosed with Chiari Type I, Chiari Type I accompanied by syringomyelia, and healthy controls, in order to elucidate the potential structural contributors to the pathogenesis of idiopathic syringomyelia. Methods: In this retrospective and comparative

Scoliosis with Chiari I malformation without associated syringomyelia.

PURPOSE: Many patients with presumed idiopathic scoliosis are found to have Chiari I malformation (CM-I) on MRI. The objective of this study is to report on scoliosis progression in CM-I with no syringomyelia. METHODS: A retrospective review of patients with scoliosis and CM-I was conducted from 1997 to 2015. Patients with syringomyelia and/or non-idiopathic scoliosis were excluded. Clinical and radiographic characteristics were recorded at presentation and latest follow-up. CM-I was defined as the cerebellar tonsil extending 5 mm or more below the foramen magnum on MRI. RESULTS: Thirty-two

Terminal syringomyelia associated with lumbar limited dorsal myeloschisis.

PURPOSE: Limited dorsal myeloschisis (LDM) is characterized by a fibroneural tethering stalk linking the skin lesion to the underlying spinal cord. Terminal syringomyelia, which is located at the lower third of the cord, is often associated with a tethered cord caused by various spinal dysraphisms; however, terminal syringomyelia has not been documented in LDM. The purpose of this study was to clarify the pathophysiological mechanisms of syringomyelia in LDM. METHODS: In our 16 patients with lumbar LDM, three patients had terminal syringomyelia. We retrospectively analyzed the clinical, neuro

[Suprasellar arachnoid cyst associated with syringomyelia. Case report].

We present a case of suprasellar arachnoid cyst which was revealed by visual impairement and hypopituitarism. Neuroradiological imaging showed the peculiar association of the suprasellar cyst with cerebellar tonsillar herniation and a large asymptomatic cervical syringomyelic cavity. Surgical treatment of the suprasellar cyst allowed the reduction of both the cyst and the syrinx. A common pathophysiological mechanism of these lesions is discussed. We suggest the possibility that an initial obstruction of the basal cisterns caused the suprasellar cyst formation which led to medullar cavity form

Spontaneous drainage in syringomyelia: magnetic resonance imaging findings.

We present five cases of syringomyelia associated with Chiari I or other causes of partial obstructions at the cervicomedullary junction, with spontaneous disruption of the wall of a cervical syrinx and formation of a communication between the cavity and the subarachnoid space, shown on axial MRI. MRI can be used to investigate the hydrodynamics, showing the liquid inside the disrupted syrinx wall and the pathway of drainage. The finding of spontaneous drainage may be important for understanding the pathogenesis of syringomyelia and may be helpful for choosing a surgical approach.

Syringomyelia complicating syphilitic spinal meningitis: a case report.

An association between syringomyelia and spinal syphilis was described in the early literature but has not been the subject of reports subsequently. We give details of a contemporary case, affirmed by magnetic resonance imaging. The patient showed significant clinical recovery following penicillin treatment, while the imaging appearances became less pronounced.

Occipital fibrous dysplasia tonsillar herniation and cervical syringomyelia.

A small posterior fossa volume has been thought to lead to hind brain herniation. The resultant obstruction to the CSF pathways at the level of the foraman magnum has been implicated in the development and subsequent progression of syringomyelia. This report highlights the gradual development of a small posterior fossa due to fibrous dysplasia of the occipital bone with consequent tonsillar herniation resulting in syringomyelia.

Cerebellopontine angle meningioma causing asymptomatic syringomyelia--case report.

A 36-year-old female was admitted with a 3-month history of headache and gait disturbance. Magnetic resonance imaging demonstrated a large tumor in the right cerebellopontine angle and syringomyelia in the upper cervical cord associated with caudal displacement of the cerebellar tonsil. Complete removal of the tumor resulted in disappearance of these associated conditions. Herniation of the cerebellar tonsil and distortion of the brain stem had probably caused disturbance of cerebrospinal fluid flow, which combined with obstruction of the spinal canal, caused the syrinx.

Syringomyelia: long-term results after posterior fossa decompression.

A retrospective study of the surgical results in 31 adult patients with syringomyelia associated with Arnold-Chiari malformation was carried out. The standard surgical procedure consisted of a posterior fossa craniectomy. The vallecula was opened in 28 patients, with plugging of the central canal in 19. In three patients, severe arachnoidal scarring was present, and dissection was not attempted. The postoperative follow-up period ranged from 5 to 10 years. More than half of the patients considered themselves improved by surgery; however, only five cases showed objective improvement. Despite th

Chiari Malformation Type 1 and Syringomyelia: Why Do Patients Claim for International Guidelines? Commentary on the 2021 Chiari and Syringomyelia Consensus Document.

The diagnosis of Chiari malformation type 1 (CM1) and Syringomyelia (Syr) has become increasingly common during the past few years. Contemporarily, the body of literature on these topics is growing, although randomized controlled studies on significant case series to drive guidelines are missing in the pediatric and adult populations. As a result of the different opinions about surgical indications and techniques raised by CM1-Syr, an increasing number of well-informed but disoriented patients is emerging. To bridge this gap, an International Consensus Conference on CM1-Syr held in Milan in No

Subarachnoid-subarachnoid Bypass for Refractory Cases of Adhesive Syringomyelia.

BACKGROUND: Syringomyelia, secondary to adhesive arachnoiditis, is an intractable disease characterized by progressive extension cranially and caudally, resulting in severe neurological deficits. Subarachnoid-subarachnoid bypass has recently been reported for syringomyelia from trauma or spinal tumor surgery. METHODS: We investigated whether subarachnoid-subarachnoid bypass is a salvage treatment for refractory adhesive arachnoiditis of various etiologies, including arachnoid abnormality and hemorrhage. Patients who underwent subarachnoid-subarachnoid bypass between 2021 and 2025 for recurren

Thoracic Spinal Arachnoid Web With Syringomyelia: A Case Report.

Spinal arachnoid webs are rare intradural extramedullary lesions that most commonly occur in the thoracic spine and may cause focal dorsal spinal cord compression, cerebrospinal fluid flow obstruction, syringomyelia, and progressive myelopathy. Diagnosis can be delayed because symptoms are often nonspecific and the arachnoid web itself may be difficult to directly visualize on standard magnetic resonance imaging. The characteristic "scalpel sign," defined by focal dorsal indentation of the spinal cord, is a key imaging feature. We present a case of a 47-year-old male patient with progressive m

Posttraumatic syrinx resulting in syringomyelia and syringobulbia: illustrative case.

BACKGROUND: Posttraumatic syringomyelia (PTS) refers to an elongated, intramedullary spinal cyst, representing a spectrum of severity and a variable interval after spinal cord injury (SCI). It typically presents with worsening motor and/or sensory function after a period of relative symptom stability following injury. The worsening ascends rostrally to include spinal cord levels previously intact above the level of injury. Rarely, PTS cavities extend rostrally to brainstem levels. OBSERVATIONS: The case presented demonstrates nearly the full spectrum of features associated with PTS. The onset

Syringomyelia Mimicking as Bibrachial Variant of Motor Neuron Disease.

INTRODUCTION: Syringomyelia is a slowly progressive degenerative disorder of the spinal cord. Clinical features of syringomyelia vary from weakness in limbs to positive sensory symptoms and dissociative sensory loss. Thus, early and prompt diagnosis becomes crucial for reducing the morbidity associated with the disease. CASE DESCRIPTION: Here, we present a case of 52-year-old male presenting with progressive weakness in bilateral upper limbs without any sensory involvement which is an atypical presentation for syringomyelia. CONCLUSION: Motor neuron disease (MND) like presentation in syringo

Syringopleural shunt for refractory syringomyelia: how I do it.

BACKGROUND: Surgical treatment of syringomyelia is directed at the reconstruction of the subarachnoid space and restoration normal cerebrospinal fluid flow. Direct intervention on the syrinx is a rescue procedure and should be offered to patients with refractory syringomyelia. METHODS: We provide an overview on indications and technique of syringopleural shunt (SPS). The procedure involves the connection of syrinx with the pleural space using a lumboperitoneal shunt. The occurrence of a negative pressure inside the pleural compartment offers an appropriate gradient for drainage from the syrin
