Theophylline in Treatment of COVID-19 Induced Sinus Bradycardia.

This is a retrospective case series of two patients with laboratory-confirmed coronavirus 2 (SARS-CoV-2) infection, presented to the University of Arkansas for Medical Sciences in January 2021. Medical records of these patients were reviewed using the EPIC electronic health record system. Clinical, laboratory, and treatment data were reviewed against periods of bradycardia in each patient. Both of the patients presented with dizziness and presyncope related to sinus bradycardia in which they received treatment with 1 mg of IV atropine and theophylline 200 mg orally. We share these two cases of

[Nonconvulsive Status Epilepticus Associated with Theophylline: A Case Report].

An 87-year-old woman was admitted to our hospital because of speech disturbance and right facio-pharyngo-glosso-masticatory diplegia. She had bronchial asthma, was previously diagnosed with cerebral infarction, had experienced two events of convulsive status epilepticus, and was undergoing treatment with theophylline, levetiracetam, and clopidogrel. Head diffusion-weighted magnetic resonance imaging revealed a high-signal area in the left crus posterior capsula interna. For this, we administered cilostazol along with her regular medicines. On day 14, she had tonic-clonic convulsions, extending

Effects of theophylline on anesthetized malignant hyperthermia-susceptible pigs.

BACKGROUND: Theophylline was shown to induce contracture development in porcine malignant hyperthermia (MH) susceptible (MHS) skeletal muscles in vitro. The purpose of the current study was to investigate the in vivo effects of theophylline in MHS and MH normal (MHN) swine. METHODS: MH-trigger-free general anesthesia was performed in MHS and MHN swine. Theophylline was administered intravenously in cumulative doses up to 93.5 mg·kg⁻¹. The clinical occurrence of MH was defined by changes of central-venous pCO₂, central-venous pH, and body core temperature. RESULTS: Theophylline induced compar

Low-dose theophylline reduces symptoms of acute mountain sickness.

OBJECTIVE: Headache, nausea, and sleeplessness at altitude [acute mountain sickness (AMS)] are major health problems for several million mountain recreationists who ascend to high altitudes each year. We aimed to test the efficacy of low-dose, slow-release theophylline for the prevention of AMS in a placebo-controlled, double-blind, randomized trial. METHODS: Twenty healthy male volunteers (mean age 34.7 y) were randomized (random allocation) to receive either 300 mg theophylline daily or placebo 5 days prior, during ascent, and during a stay at 4,559 m altitude. AMS symptoms were collected u

Pharmacokinetics of intravenous theophylline in mutant Nagase analbuminemic rats.

It was obtained from our laboratories that the expression of hepatic microsomal cytochrome P450 (CYP) 1A2 increased approximately 3.5 times in mutant Nagase analbuminemic rats (NARs, an animal model for human familial analbuminemia), and theophylline was reported to be metabolized to 1,3-dimethyluric acid (1,3-DMU) and 1-methylxanthine (which was further metabolized to 1-methyluric acid, 1-MU, via xanthine oxidase) via CYP1A2 in rats. Hence, the pharmacokinetic parameters of theophylline, 1,3-DMU and 1-MU were compared after intravenous administration of aminophylline, 5 mg/kg as theophylline,

Kinesthetic stimulation versus theophylline for apnea in preterm infants.

BACKGROUND: Apnea of prematurity may lead to hypoxemia and bradycardia requiring resuscitative measures being instituted. Many treatments have been used in infants with apnea of prematurity, such as theophylline. Kinesthetic stimulation, which uses various forms of oscillating mattress, might also prevent apnea without using a standard drug such as theophylline. OBJECTIVES: Main question: in preterm infants, how does kinesthetic stimulation compare with methylxanthine therapy in the treatment of apnea of prematurity. SEARCH STRATEGY: The standard search strategy of the Neonatal Review Group

Pharmacokinetic interactions between theophylline and other medication (Part I).

Many drugs have been found to increase or decrease the clearance of theophylline, probably by interaction with one or more of the variants of the cytochrome P450 drug-metabolising system. Theophylline may be particularly susceptible to alteration of its clearance because of the particular form(s) of the P450 system involved, because its metabolism is saturable, and/or because 90% of its elimination is via metabolism. Its clearance has been found to be decreased (typically by around 25%, but often by far more) by erythromycin, troleandomycin (triacetyloleandomycin), roxithromycin, enoxacin, cip

Failure of chronic theophylline therapy to alter circulating catecholamines.

An increase in circulating adrenaline and noradrenaline has been reported following acute dosing with theophyllines. This effect on catecholamines has been proposed as a possible mechanism of action of theophyllines. In a double-blind placebo controlled trial we have studied the effects of 5 days oral theophylline therapy on circulating catecholamines and adrenaline clearance. There were no significant changes in circulating catecholamines or adrenaline clearance following theophylline. We also examined the effects of theophylline on the hypokalaemic and haemodynamic actions of adrenaline. The

Low-dose theophylline therapy in idiopathic apnea of prematurity.

The effects of theophylline at a dose of 2 mg/kg/day (with resultant serum concentrations of 2.8 to 3.9 micrograms/ml) were observed in seven premature infants with severe idiopathic apnea. The frequency of apneic episodes decreased 62 +/- 9% (SEM) and the frequency of episodes of bradycardia decreased 73 +/- 9% following 24-48 hours of this therapy. No changes in ventilatory response to CO2 resting respiratory minute volume, arterial blood gas values, resting heart rate, or serum electrolyte concentrations were observed. Theophylline at this dose appears to be a safe and effective treatment f

Theophylline-induced seizures in adults. Correlation with serum concentrations.

Eight patients developed grand mal seizures during intravenous theophylline therapy. None had a history of neurologic disorder, and all were acutely ill with severe pulmonary or cardiovascular disease, or both. Serum theophylline concentrations obtained within 1 hour of the seizure ranged from 25 mug/ml to 70mug/ml, with a mean value (53 plus or minus 4.8 mug/ml) more than twice the upper limit of the recommended therapeutic concentration. This serum theophylline concentration was greater than the concentration found in a group of patients with less severe drug-related symptoms (35 plus or min

Molecular Insights into the Binding of Theophylline with DNA: An "In Silico" Approach.

This article ventures into the interactions between DNA and theophylline (Th), a biologically active xanthine derivative with pharmacological properties, using in-silico methods. Density functional theory calculations were used to assess the interactions between theophylline and natural nucleobases: adenine (A), guanine (G), cytosine (C), and thymine (T). Different interacting orientations, namely the planar hydrogen-bonded conformations and the vertically stacked geometries between theophylline and nucleobases have been investigated by evaluating the binding energies of the dimeric structures

Pharmacokinetics of a 503B outsourcing facility-produced theophylline in dogs.

Theophylline is an important drug for treatment of canine chronic bronchitis and bradyarrhythmias, but new products require validation since pharmacokinetics in dogs can vary by formulation. A new, 503B outsourcing facility-produced theophylline product (OFT) is available for veterinary use. Outsourcing facilities have many advantages over traditional compounding sources including current good manufacturing practice compliance. The purpose of this study was to establish the pharmacokinetics of OFT in dogs. Eight healthy dogs received 11 mg/kg intravenous aminophylline and 10 mg/kg oral OFT fol

Theophylline for prevention of kidney dysfunction in neonates with severe asphyxia.

INTRODUCTION: Recent studies have suggested theophylline for prevention of kidney dysfunction in asphyxia. This study was designed to determine whether theophylline could prevent or ameliorate kidney dysfunction in term neonates with perinatal asphyxia. MATERIALS AND METHODS: We assigned 36 severely asphyxiated term infants (Apgar score < or =5) into 2 groups to receive intravenously a single dose of either theophylline (5mg/kg; n =17) or placebo (n =19) during their first 60 minutes of life. The 24-hour fluid intake and the urine volumes were recorded during the 1st, 3rd, and 5th days of lif

[Measurement of theophylline concentrations by AccuMeter--comparison with the EIA method].

Theophylline is widely used for treating patients with bronchial asthma. However, since the therapeutic concentration range is narrow, adverse reactions are frequent and often difficult to control, making monitoring of theophylline concentration mandatory for its efficient and safe use. In this study, we employed the AccMeter which allowed us to measure theophylline concentrations quickly and with ease, and compared it with EIA method. AccMeter is a kit which consists of two parts. One part consists of a chromatopaper with antitheophylline mouse monoclonal antibody fixed on it, on which a smap

A study on the route of 1-methylurate formation in theophylline metabolism.

The urinary excretion patterns of theophylline metabolites were studied in a subject on a routine, oral dose of 600 mg/day. The highest correlation was observed between the excretions of 1,3-dimethylurate and 1-methylurate (r = 0.73 +/- 0.08). The poorest correlations were observed when the excretion of 1-methylxanthine was compared to those of 1-methylurate and 1,3-dimethylurate (r < or = 0.55, P > or = 0.05). The difference in the quality of the correlations was not due to rate-limiting or class-specific carries. The results suggested that 1-methylurate did not derive solely from 1-methylxan

Effect of pretreatment with anticonvulsants on theophylline-induced seizures in the rat.

Seizures, often with fatal outcome, are a manifestation of pronounced theophylline toxicity. Prodromal symptoms are not always apparent, and the seizures are reported to be, in certain cases, refractory to treatment with anticonvulsant drugs. The purpose of this investigation was to examine, by an established animal model, which of the commonly used anticonvulsants can reduce the central nervous system sensitivity to theophylline neurotoxicity and what should be the preferred treatment in cases in which theophylline toxicity is anticipated. The anticonvulsant agents in doses that are found to
