Aspirex Thrombectomy in Occluded Dialysis Access: A Retrospective Study.

PURPOSE: This study is the first to present the outcomes of the Straub Aspirex device for the salvage of occluded renal dialysis access fistulae. MATERIALS AND METHODS: This is a retrospective study, using data from the Renal Unit and Radiology Department database. It included all the patients between 2010 and 2014 who underwent percutaneous mechanical thrombectomy (PMT) treated by JD. Aspirex is an over-the-wire, 6-10 French catheter within which is a rapidly rotating helix which draws thrombus into a window near the tip which it then macerates and removes. Access survival was assessed using

Mechanical Solitaire Thrombectomy with Low-Dose Booster Tirofiban Injection.

PURPOSE: Mechanical thrombectomy using a Solitaire stent has been associated with a high recanalization rate and favorable clinical outcome in intra-arterial thrombolysis. To achieve a higher recanalization rate for mechanical Solitaire thrombectomy, we used an intra-arterial low-dose booster tirofiban injection into the occluded segment after stent deployment. We report the safety and recanalization rates for mechanical Solitaire thrombectomy with a low-dose booster tirofiban injection. MATERIALS AND METHODS: Between February and March 2013, 13 consecutive patients underwent mechanical Solit

Mechanical thrombectomy for acute ischemic stroke with cerebral microbleeds.

BACKGROUND: The influence of cerebral microbleeds (CMBs) on post-thrombolytic hemorrhagic transformation (HT) in patients with acute ischemic stroke remains controversial. OBJECTIVE: To investigate the association of CMBs with HT and clinical outcomes among patients with large-vessel occlusion strokes treated with mechanical thrombectomy. METHODS: We analyzed patients with acute stroke treated with Merci Retriever, Penumbra system or stent-retriever devices. CMBs were identified on pretreatment T2-weighted, gradient-recall echo MRI. We analyzed the association of the presence, burden, and di

Early middle cerebral artery stenosis following stent-assisted thrombectomy.

Stent-assisted thrombectomy (SAT) is an extensively used endovascular treatment method for stroke in which the thrombectomy stents come into direct contact with the vascular intimal surface and entrap the thrombus causing the arterial occlusion. Although there are a few studies that demonstrate that the vessel wall changes in the arteries where stroke intervention is performed, we observed progressive stenosis in early follow-up imaging studies in a case. We present a middle cerebral artery (MCA) stroke patient who had four repetitive stent passes during SAT and developed distal MCA stenosis 2

Percutaneous aspiration thrombectomy in treatment of massive pulmonary embolism.

BACKGROUND: Pulmonary embolism (PE) associated with haemodynamic instability has exceedingly high mortality. We describe our experience using percutaneous mechanical thrombectomy (PMT) in patients with massive PE and right ventricle dysfunction. METHODS: Sixteen patients (11 males and five females; mean age, 55.7 ± 8.3 years) with massive PE were treated with PMT. A percutaneous Aspiration Device (8 French Aspirex® aspiration thrombectomy catheter, Straub Medical, Switzerland) was used in all patients. Clinical outcomes, right ventricle and pulmonary artery pressures (PAP), thrombus clearance

Mechanical thrombectomy in proximal subclavian artery in-stent occlusion.

PURPOSE: Mechanical thrombectomy is an established tool for endovascular therapy of acute/chronic in-stent occlusions of the lower extremities, especially for the superficial femoral artery. The authors report the use of the Rotarex catheter system for an in-stent proximal subclavian occlusion. CASE PRESENTATION: A 51-year-old female patient with previous multiple attempts at left subclavian revascularization; primary endovascular stenting at the origin of the subclavian artery occluded at 3 months, and a carotid-subclavian bypass was placed distally to the stent. After 6 months from the firs

Intracranial thrombectomy using the Solitaire stent: a historical vignette.

INTRODUCTION: The endovascular treatment of acute ischemic stroke has been revolutionized in the past years by the introduction of new devices for mechanical thrombectomy. Several tools were already available in 2008. The majority allowed the recanalization of acutely occluded intracranial arteries with acceptable levels of safety and efficacy, and with occasional failures. CASE PRESENTATION: On 3 March 2008, a 67-year-old woman was treated 3.5 h after the clinical onset of a right hemispheric stroke due to an embolic middle cerebral artery (MCA) M1 occlusion. The National Institutes of Healt

Suprahepatic Budd-Chiari syndrome treated with thrombectomy and cavoplasty.

The case of a 42-year-old female with Budd-Chiari syndrome (BCS) and lupus anticoagulant is reported. This patient, who had been on chronic anticoagulants for her lupus anticoagulant, presented with abdominal pain, dyspnea on exertion, engorged abdominal wall venous collaterals, and hepatomegaly. A dynamic computerized tomography of the abdomen showed complete suprahepatic inferior vena caval occlusion at the junction with the right atrium. IVC venogram confirmed the diagnosis and also demonstrated patency of the hepatic veins. Free hepatic venous pressure was 25-26 mm Hg. Histopathologic exam

Eversion thrombectomy for portal vein thrombosis during liver transplantation.

Portal vein thrombosis (PVT) has been seen as an obstacle to orthotopic liver transplantation (OLT), but recent data suggest that favorable results may be achieved in this group of patients. The aim of this study was to analyze the incidence, management, and survival of patients with PVT undergoing primary OLT with thrombectomy. Between October 1990 and August 2000, 468 liver transplantations were performed in our center and portal vein thrombosis was present in 38 patients (8.1%). Preoperative diagnosis, extension, intraoperative management, postoperative recurrence of portal vein thrombosis,

[Ethical Issues in Mechanical Thrombectomy for Acute Ischemic Stroke].

Mechanical thrombectomy (MT) has become the standard treatment for acute ischemic stroke (AIS) caused by large vessel occlusion, and its indications have expanded substantially over the past decade. Recent randomized controlled trials (RCTs) have extended eligibility to patients presenting in extended time windows, those with large ischemic cores, and other populations previously considered unsuitable for intervention. However, the expansion of MT indications has also raised important ethical challenges. Patients with AIS often lack decision-making capacity, requiring physicians to balance res

Thrombolysis, Thrombectomy, and Antithrombotic Therapy for Acute Ischemic Stroke.

OBJECTIVE: This article summarizes the role of thrombolytic therapy, mechanical thrombectomy, and antithrombotic therapy in the early management of acute ischemic stroke. LATEST DEVELOPMENTS: Patient selection and treatment options of acute ischemic stroke are rapidly evolving. Mounting evidence supports the use of IV tenecteplase for acute ischemic stroke. Advanced neuroimaging has helped expand the time window for acute therapies. Recent studies have helped broaden the application of mechanical thrombectomy, particularly for large core infarctions. Our understanding of the best treatment ap

Procedure-related vasospasm and reperfusion vulnerability during mechanical thrombectomy.

Mechanical thrombectomy (MT) has reshaped the treatment of acute ischemic stroke caused by large-vessel occlusion (LVO), yet successful angiographic reperfusion does not reliably translate into meaningful neurological recovery. Landmark randomized trials and the Highly Effective Reperfusion Evaluated in Multiple Endovascular Stroke Trials (HERMES) patient-level meta-analysis established MT as standard of care, but they also brought into focus the persistent problem of incomplete tissue-level rescue despite technically successful recanalization. Procedure-related vasospasm is usually handled as

Time-Dependent Efficacy of Thrombolysis Before Thrombectomy: RES-Q.

BACKGROUND: Randomized evidence suggests that the association of intravenous thrombolysis (IVT) before endovascular thrombectomy (EVT) may be time-dependent. We evaluated whether treatment timing modifies the association of IVT+EVT versus EVT alone with short-term in-hospital outcomes. METHODS: We conducted a multinational observational registry cohort study using RES-Q (Registry of Stroke Care Quality) data (2022-2024) from 38 countries. Among 3132 eligible anterior-circulation large-vessel occlusion patients treated with EVT, 3009 comprised the analytic cohort (IVT+EVT or EVT alone). The pr

Blood pressure threshold and outcomes after successful endovascular thrombectomy.

BACKGROUND: Several randomized clinical trials have indicated that intensive blood pressure (BP) lowering is associated with worse outcomes, leaving the optimal BP targets following endovascular thrombectomy (EVT) uncertain. AIMS: This study aimed to investigate the relationship between specific systolic BP (SBP) thresholds, time spent outside these thresholds, and clinical outcomes. METHODS: This post hoc analysis of the Outcome in Patients Treated With Intra-Arterial Thrombectomy-Optimal Blood Pressure Control (OPTIMAL-BP) trial, included patients with successful EVT randomized to intensiv
