BACKGROUND: We evaluated the pulmonary effects of two ventilator-driven alveolar recruitment maneuver (ARM) methods during laparoscopic surgery.
METHODS: Sixty-four patients undergoing robotic prostatectomy were randomized into two groups: incrementally increasing positive end-expiratory pressure in a stepwise manner (PEEP group) versus tidal volume (VT group). We performed each ARM after induction of anesthesia in the supine position (T1), after pneumoperitoneum in the Trendelenburg position (T2), and after peritoneum desufflation in the supine position (T3). The primary outcome was change i

BACKGROUND: High frequency oscillation (HFO) is a lung protective ventilation strategy for newborn infants due to the small tidal volume delivery at fast frequencies, but has been associated with an increased risk of brain injury. High-frequency oscillation with volume-targeted ventilation (HFO&VTV) is a new mode of HFO that applies a targeted tidal volume (VThf) set by the clinician and achieves less fluctuations of carbon dioxide levels and therefore reduces the risk of brain injury. The optimal starting VThf values, however, have not been explored to date. This study investigates the optimu

INTRODUCTION: After lung transplantation, the ventilated parenchyma is the donor lung rather than the recipient's native lung. Normalizing tidal volume to donor-predicted body weight (dPBW) indexes delivered ventilation to estimated graft size.
METHODS: We conducted a retrospective single-center cohort study to evaluate whether dPBW-normalized tidal volume at intensive care unit admission after transplantation (T0) was associated with time-matched peak pressure difference under clinician-selected ventilation. Peak pressure difference was defined as peak airway pressure minus positive end-expi

Background: Mechanical ventilation (MV) may be associated with extrapulmonary responses shaped by the host inflammatory-metabolic background. Methods: This exploratory study characterized renal changes during 1 h of reduced-tidal-volume mechanical ventilation in male Wistar rats with diet-induced obesity or lipopolysaccharide (LPS)-induced endotoxemia, together with concurrent respiratory, acid-base, metabolic, and hemodynamic alterations. Arterial blood gas, biochemical, hemodynamic, and urinary variables were assessed at the pre-MV and post-MV time points. Renal protein expression was evalua

BACKGROUND: Shared mechanical ventilation has been proposed during ventilator shortages, but unequal tidal volume delivery remains a major limitation when lung compliance differs. Professional societies have cautioned against this practice, yet the intervention has never been validated clinically, and ethical constraints preclude direct patient testing. Translational simulation-simulation applied beyond education to probe clinical systems, test crisis-driven innovations, and generate actionable safety data-offers a methodological pathway for evaluating interventions that cannot be studied in v

BACKGROUND: Airway pressure release ventilation (APRV) is a mode of mechanical ventilation, which has demonstrated potential benefits in trauma patients. We therefore sought to compare relevant pulmonary data and safety outcomes of this modality to the recommendations of the Adult Respiratory Distress Syndrome Network.
METHODS: Patients admitted after traumatic injury requiring mechanical ventilation were randomized under a 72-hour waiver of consent to a respiratory protocol for APRV or low tidal volume ventilation (LOVT). Data were collected regarding demographics, Injury Severity Score, oxy

This study was performed to elucidate the mechanism of improved oxygenation after surfactant replacement therapy in respiratory distress syndrome (RDS) of the newborn infants. In 26 newborns with RDS, end tidal-CO2 tension (PetCO2), arterial blood gas analysis and pulmonary function tests were measured at baseline, 30 min, 2 hr and 6 hr after surfactant administration. The changes in dead space/tidal volume ratio (VD/VT ratio=(PaCO2-PetCO2)/PaCO2), oxygenation index and arterial-alveolar partial pressure difference for oxygen ((A-a)DO2) were elucidated and correlated with pulmonary mechanics.

BACKGROUND: Acute respiratory distress syndrome (ARDS) is a highly morbid condition that has limited therapeutic options. Optimal vitamin D status has been linked to immunological effects that may benefit critically ill patients. Therefore, we investigated whether admission 25-hydroxyvitamin D levels (25OHD) are associated with clinical outcomes in ARDS patients.
METHODS: We performed a secondary analysis of data from a randomized, controlled trial comparing oxygenation strategies in 549 patients with ARDS (NCT00000579). Baseline 25OHD was measured in stored plasma samples. We investigated th

OBJECTIVE: To measure the effects of tidal volume, ventilatory frequency, and oxygen insufflation flow on the fraction of inspired oxygen in cadaveric horse heads attached to a lung model.
SAMPLE: 8 heads of equine cadavers.
PROCEDURES: Each cadaveric horse head was intubated with a nasotracheal tube that extended into the proximal portion of the trachea. Oxygen was delivered through an oxygen catheter contained within and extending to the tip of the nasotracheal tube. The trachea was connected to the lung model by use of a spiral-wound hose with a sampling adaptor. Eight treatment combinati

BACKGROUND: Optimal ventilatory settings during general anaesthesia are unknown. The goal of this trial was to test different ventilatory settings and blood pressure targets during general anaesthesia. This manuscript focuses on the ventilatory interventions.
METHODS: This was a multicentre, randomised factorial trial conducted at eight hospitals in Denmark. Four hundred and eighty-three patients ASA 3-5, scheduled for major surgery under general anaesthesia, were included. Patients were randomised to positive end-expiratory pressure (PEEP) of 5, 8, or 10 cm H2O, tidal volume of 6, 8, or 10 m

BACKGROUND: The physiological response and the potentially beneficial effects of positive end-expiratory pressure (PEEP) for lung protection and optimization of ventilation during spontaneous breathing in patients with acute respiratory distress syndrome (ARDS) are not fully understood. The aim of the study was to compare the effect of different PEEP levels on tidal volume distribution and on the ventilation of dependent lung region during neurally adjusted ventilatory assist (NAVA).
METHODS: ARDS-like lung injury was induced by using saline lavage in 10 anesthetized and spontaneously breathi

BACKGROUND: In patients in the prone position, the reliability of pulse pressure variation and stroke volume variation (PPV and SVV) and the use of functional hemodynamic tests to predict fluid responsiveness have not previously been established. Perioperatively, in this setting, optimizing fluid management can be challenging, and fluid overload is associated with both intraoperative and postoperative complications. We designed this study to assess the sensitivity and specificity of baseline PPV and SVV, the tidal volume (VT) challenge (VTC) and the end-expiratory occlusion test (EEOT) in pred

BACKGROUND: Few studies have reported the measurement of anatomical dead space (Vd,an) and alveolar tidal volume (VA) in ventilated neonates with respiratory distress.
OBJECTIVE: The aim of this study was to determine the differences in Vd,an and VA in ventilated infants between the early and recovery phases of respiratory distress using volumetric -capnography (Vcap) based on ventilator graphics and capnograms.
METHODS: This study enrolled twenty-five ventilated infants (mean birth weight, 2,220 ± 635 g; mean gestational age, 34.7 ± 3.3 weeks). We adjusted respiratory settings to maintain a

BACKGROUND: Severe kyphoscoliosis in spina bifida patients can result in thoracic insufficiency syndrome, which significantly compromises pulmonary function. While traditional clinical measures for spina bifida focus primarily on bladder function and motor strength, this case highlights pulmonary function as an essential additional indicator.
OBSERVATIONS: The authors report on the case of a 16-year-old male with spina bifida and severe thoracic kyphoscoliosis presenting with progressive respiratory difficulty limiting ambulation. Imaging revealed a thoracic curve of approximately 107° and ma

BACKGROUND: Unilateral ligation of the pulmonary artery (UPAL) induces bilateral lung injury in pigs undergoing controlled mechanical ventilation. Possible mechanisms include redistribution of ventilation toward the non-ligated lung and hypoperfusion of the ligated lung. The addition of 5% CO2 to the inspiratory gas (FiCO2) prevents the injury, but it is not clear whether lung protection is a direct effect of CO2 inhalation or it is mediated by plasmatic hypercapnia. This study aims to compare the effects and mechanisms of FiCO2 vs. hypercapnia induced by low tidal volume ventilation or instru

BACKGROUND: For adequate ventilation during bag-valve-mask ventilation, rescuers should ensure a proper mask seal using the one-handed or two-handed technique. Little is known about the magnitude of sealing forces of a bag-valve-mask needed for adequate ventilation. This study aimed to explore the effect of the 4-point sealing forces of a bag-valve-mask on tidal volume while using the one-handed technique, focusing on the moderating effect of C length (the distance from the thumb to the index finger in the C shape of the one-hand EC grip).
METHODS: A prospective, descriptive simulation study
