Managing refractory glaucoma with a fixed combination of bimatoprost (0.03%) and timolol (0.5%).

Glaucoma is a chronic progressive optic neuropathy characterized by progressive loss of retinal ganglion cells, which manifests clinically with loss of optic disc neuroretinal rim tissue, defects in the retinal nerve fiber layer, and deficits on functional visual field testing. The goal of glaucoma treatment is to reduce the intraocular pressure to a level that prevents or minimizes the progressive loss of vision. The current standard of management for the newly diagnosed primary open angle glaucoma (PAOG) patient is to start topical medication. Available topical medications include: beta-adre

Enhanced ocular hypotensive response to timolol in rabbits with prior dexamethasone treatment.

The purpose of this study was to determine glucocorticoid modulation of ocular pressure to timolol applied topically to rabbit eyes that were pretreated with dexamethasone. Rabbits were pretreated with five applications of topical 0.007% of dexamethasone (0.01% dexamethasone phosphate) or saline drops, administered at ten min intervals. The eyes were then treated with timolol maleate drops at concentrations of free base of 0.00007%, 0.0007%, 0.007% and 0.07%. An additional group of rabbits received dexamethasone pre-treatment only. Intraocular pressure was measured for the next four hr. Enhanc

Effect of nasolacrimal occlusion on timolol concentrations in the aqueous humor of the human eye.

We studied the effect of manual nasolacrimal occlusion on the concentration of timolol in the aqueous humor of eyes of patients undergoing cataract extraction. Aqueous humor samples were obtained at various times after timolol maleate instillation from patients with or without 5 min of nasolacrimal occlusion; aliquots were assayed by HPLC. In patients receiving occlusion treatment, average timolol concentrations were statistically greater than those in control patients both between 15 and 90 min after instillation and also at 180 min. Pharmacokinetic analysis indicated that occlusion increased

[Many years of personal observation on the effect of timolol for arresting development of keratoconus].

On examining about 300 patients with keratoconus in the period of 20 years the authors detected in a number of cases variations of the intraocular pressure. By means of a pneumo-tonometer Digilab 30R periodic variations of the IOP were measured in 76 patients and daily variations in 42. The statistical analysis showed that the application of Timolol and reduction of the IOP variations has a decisive influence on the improvement of objective changes and the visual acuity without correction. Presently in our Department Timolol is used successfully in 160 patients. There was no deterioration in e

Improving the ocular to systemic ratio of topical timolol by varying the dosing time.

This study was performed to determine whether the ratio of ocular to systemic absorption of topically applied timolol in the pigmented rabbit can be maximized by varying the time of drop instillation. Twenty-five microliters of 0.65% timolol maleate solutions were instilled into the pigmented rabbit eye at 6 AM, 12 PM, 6 PM, or 12 AM. The time course of timolol concentration in plasma and various eye tissues (conjunctiva, sclera, corneal epithelium, corneal stroma, aqueous humor, iris-ciliary body, and lens) was monitored with the use of reversed phase high-performance liquid chromatography (H

Improved ocular: systemic absorption ratio of timolol by viscous vehicle and phenylephrine.

Increasing the ocular absorption of timolol relative to its systemic absorption is important clinically because ophthalmic timolol may cause serious respiratory, cardiac, and central nervous system side effects. The authors evaluated the effects of phenylephrine coadministration and solution viscosity on the aqueous humor:plasma and iris ciliary body:plasma ratios of peak timolol concentrations after ocular application. Timolol eye drops (5 mg/ml, 25 microliters) were administered to the eyes of pigmented rabbits. Coadministered phenylephrine (0.8-8.2 mg/ml) decreased the systemic peak concent

A clinical trial of timolol and epinephrine in the treatment of primary open-angle glaucoma.

Forty-seven patients with chronic open-angle glaucoma who required therapy but who had never been treated previously were enrolled in a prospective randomized, long-term clinical trial comparing the effectiveness of 0.5% timolol maleate and 1% epinephrine. Patients were followed for an average of 33 months. There were 20 failures, 18 of which were because of inadequate intraocular pressure (IOP) control (required 20% reduction in outflow pressure). Twelve failures were in the epinephrine group and eight were in the timolol group. During the first year of therapy, 35% compared with 0% of patien

Binding of the beta-blockers timolol and H 216/44 to ocular melanin.

The eyes from pigmented rabbits were instilled with the beta-adrenoceptor antagonists H 216/44 or timolol. After a single instillation (1.9 mumol), the iris and ciliary body contained H 216/44, which decreased with a half-life of approx. 43 days. Daily instillation caused a gradual increase in the content of H 216/44 and timolol in the iris and ciliary body, the steady-state concentration of timolol being 10 times higher than that of H 216/44. The concentrations of H 216/44 were seven times higher in the iris and ciliary body of pigmented rabbits than in albino animals. H 216/44 was reversibly

Management of retinal detachment without tear by topical therapy (atropine and timolol).

14 various kinds of retinal detachments were treated by timolol + atropine eye drops. 4 cases were primary procedures and required no further surgery. 10 cases were recurrent detachments after episcleral or endo-vitreous surgery. In 8 cases there was a notable proliferative vitro-retinopathy. 10 cases were completely flattened by this medication and we obtained the reapplication of the posterior pole and the upper retina in the 4 other cases.

A comparison between iris-ciliary body concentration and receptor affinity of timolol.

Topical unilateral application of timolol reduces the IOP (intraocular pressure) of both the treated and the untreated eye. For this reason, a cental mechanism of action, in addition to the local one, has been suggested for the reduction of IOP by this drug. In this study timolol concentrations in the iris-ciliary body of the treated and untreated rabbit eyes were compared with pharmacologically active in vitro timolol concentrations. The drug concentration in the untreated eye too was found to be high enough to exert a local action.
