Temporising external fixation reduces loss of reduction compared with plaster splinting in ankle fracture-dislocations: a systematic review and meta-analysis of cohort studies.

PURPOSE: The optimal temporising strategy for ankle fracture-dislocations is a highly debated topic. These unstable injuries are commonly managed with staged fixation to allow for soft tissue recovery, using either plaster splinting or ankle-spanning external fixation. This study aimed to compare the efficacy and safety of these two approaches. METHODS: A systematic search of MEDLINE, Embase, Scopus, Cochrane CENTRAL, and Google Scholar was conducted. Comparative studies evaluating temporary external fixation versus plaster splinting in skeletally mature patients with ankle fracture-dislocati

Patella Fractures: Approach to Treatment.

Patellar fracture morphology varies based on the mechanism of injury. Most fractures are either a result of direct impact or through an indirect eccentric extensor contraction injury. Each fracture pattern requires appropriate preoperative planning and individualization of the fixation method. Displaced fractures affect the extension apparatus, and often require surgical fixation. Surgical treatment is recommended in fractures with any of the following features: articular step-off > 2 mm, > 3 mm of fracture displacement, open fractures, and displaced fractures affecting the extensor mechanism.

Towards safer non-volatile tissue fixatives: Evaluation of choline-based ionic liquids for fixing ocular tissues.

Volatile organic chemicals (VOCs) are routinely used for processing biological tissue samples in clinical laboratories. Recognizing their serious health and environmental impacts, a few non-volatile green solvents (choline based ionic liquids, ILs) were evaluated as tissue fixatives here. Microscopic evaluation of histo-morphology, fixation and staining quality, and macromolecular integrity (DNA and proteins) were assessed in human eye tissues (sclera, choroid, retinal layers and retinal pigmented epithelium, eyelid and orbit) after IL-fixation. Formalin-fixed tissues were used as standard ref

Intramedullary pinning of concomitant fibular shaft fractures prevents postoperative tibial shaft malalignment without increasing soft tissue complications.

BACKGROUND: Tibial diaphysis fractures, accounting for 17 % of lower extremity fractures, are often associated with fibular fractures. This study compares intramedullary pinning and extramedullary plating for fibular fixation following tibial fracture fixation. METHODS: A retrospective analysis was performed on 215 patients treated between 2012 and 2022. Patients were grouped by fibular fixation: no fixation, intramedullary pin, or extramedullary plate. Logistic regression identified risk factors for postoperative tibial malalignment and soft tissue complications. RESULTS: Malalignment occur

Early Versus Delayed Plate Fixation in Pilon Fractures-Which Is More Advantageous?

Background: Pilon fractures refer to distal tibial fractures that may involve extra-articular, partial articular, or complete intra-articular components, most commonly caused by high-energy trauma. The choice between early (<72 hours) and delayed (>7 days) surgical fixation significantly impacts clinical outcomes. This study aimed to compare the effects of early vs. delayed plate fixation on fracture healing time, functional outcomes, and complication rates. Materials and Methods: This retrospective study analyzed 80 patients who underwent surgical treatment for pilon fractures between 2018 an

The role of external fixation in acute ankle trauma.

The use of external fixation methods has become increasingly popular throughout the past decade in the treatment of tibial pilon fractures to prevent the dreaded soft tissue complications after high velocity injuries. A variety of methods has been proposed, including ankle-spanning half-pin frames; circular (Ilizarov) frames with tensioned wires; or hybrid frames, either as the sole treatment or, more frequently, in conjunction with limited internal screw fixation. External fixation also has a role in staged protocols as a primary tool for reduction and preliminary fixation until soft tissue c

Two-stage operative treatment of comminuted os calcis fractures. Primary indirect reduction with medial external fixation and delayed lateral plate fixation.

The indication for otherwise established lateral plate fixation in fractures of the calcaneus becomes relative when severe comminution (Sander's Type III and IV) of the bone involves concomitant massive soft-tissue swelling. The calcaneus is shortened, Böhler's angle flattened, and lateral bulging and varus deformity persist throughout the six or more days needed for soft-tissue compromise to recede. Intraoperatively, problems with anatomic reduction are encountered primarily because of soft-tissue shrinkage and muscle contraction, underscoring the rationale for conservative treatment modaliti

Free flap reconstructions of tibial fractures complicated after internal fixation.

The cases of 15 patients are presented where microvascular soft-tissue reconstructions became necessary after internal fixation of tibial fractures. Primarily, seven of the fractures were closed. Eleven fractures had originally been treated by open reduction and internal fixation using plates and screws, and four by intramedullary nailing. All of the patients suffered from postoperative complications leading to exposure of the bone or fixation material. The internal fixation material was removed and radical revision of dead and infected tissue was carried out in all cases. Soft tissue reconstr

Management of Tibia Pilon Fractures With Ring External Fixation.

Tibia pilon fractures are complex injuries associated with ankle articular cartilage damage and soft tissue compromise. Modern circular external fixation methods are useful in their surgical management, especially for open fractures, severe soft tissue injury, fracture patterns with bone loss or compromise, severe cartilage loss, and certain patient factors. Surgical techniques involve ring block concepts, strategic distal tibia wire placements, hexapod fixators with software-guided bone alignment, and other uses of circular external fixation including ankle joint distraction, augmentation of

Decreasing formalin concentration improves quality of DNA extracted from formalin-fixed paraffin-embedded tissue specimens without compromising tissue morphology or immunohistochemical staining.

Genomic technologies are increasingly used clinically for both diagnosis and guiding cancer therapy. However, formalin fixation can compromise DNA quality. This study aimed to optimise tissue fixation using normal colon, liver and uterus (n=8 each) by varying neutral buffered formalin (NBF) concentration (1%-5% w/v) and fixation time (24-48 hours). Fixation using 4% NBF improved DNA quality (assessed by qPCR) compared with routine (4% unbuffered formal saline-fixed) specimens (p<0.01). Further improvements were achieved by reducing NBF concentration (p<0.00001), whereas fixation time had no ef

Gustilo grade IIIB tibial fractures requiring microvascular free flaps: external fixation versus intramedullary rod fixation.

BACKGROUND: Gustilo IIIB fractures involve high-energy tibial fractures for which there is inadequate soft tissue coverage. In addition to orthopedic fixation, these injuries require soft tissue reconstruction, often in the form of a microvascular free flap. Although the majority of orthopedic literature favorably compares intramedullary rod fixation to external fixation in open tibial fractures, these studies have not focused on the role of either method of fixation in relation to the soft tissue reconstruction. METHODS: Because we had noted numerous complications after providing free-flap c

Intramedullary Nailing of Forearm Fractures.

The primary goal in operative fixation of forearm fractures is to restore length, rotational stability, and maintenance of the radial bow. Plate osteosynthesis is well regarded as the gold standard of treatment though often necessitates soft tissue injury, periosteal stripping, and risk of refracture after hardware removal. While intramedullary nails have been utilized for forearm fixation since the early 1900s, technological advancements including locked intramedullary nails have lead to improved outcomes in intramedullary nail forearm fixation. In select patients, intramedullary nail fixatio

External fixation for open fractures of the upper extremity.

Recent advances in external fixation hardware, frame application, and pin-site care have resulted in the evolution of external fixation as a safe and versatile technique. It provides skeletal stability, access to the site of injury, and allows early mobilization. It avoids the disadvantages of additional soft-tissue stripping and the foreign body introduction associated with internal fixation of open fractures. The utility of external fixation principles in open hand and wrist fractures is well established. Indications for external fixation in open fractures of the forearm, elbow, and arm are

Tissue Shrinkage of Resected Specimens in Hirschsprung's Disease: Why Pediatric Surgeons Think the Bowel Specimen was Longer Than Indicated in the Pathology Report.

BACKGROUND: Hirschsprung disease (HD) is an aganglionosis of variable length starting at the rectosigmoid colon with surgery as sole therapeutic option. The length of the resected bowel segment is a crucial information for the treating surgeons and influences the prognosis of the patient. It is often artificially altered due to post operative tissue shrinkage. The objective of this study is to quantify the extent tissue shrinkage of HD specimens. MATERIAL AND METHODS: Colorectal HD specimens were measured at the time of surgery and at the time of cut-up, either fresh or after formalin fixatio

Advancements in percutaneous fixation for foot and ankle trauma.

Operative fixation of foot and ankle trauma can be challenging. Often times, the soft tissue envelope can have extensive damage as a result of the fracture. In these cases, percutaneous fixation may be used. Percutaneous fixation can benefit both soft tissue and osseous healing when used correctly. Many techniques have been described in the literature that may help to preserve blood supply, minimize soft tissue dissection, and restore a functional limb. This article reviews general guidelines for fracture and soft tissue management, osseous healing of fractures, and how certain techniques infl

Comparative analysis of fixation techniques for signal detection in avian embryos.

The choice of fixation method significantly impacts tissue morphology and visualization of gene expression and proteins after in situ hybridization chain reaction (HCR) or immunohistochemistry (IHC), respectively. In this study, we compared the effects of paraformaldehyde (PFA) and trichloroacetic acid (TCA) fixation techniques prior to HCR and IHC on chicken embryos. Our findings underscore the importance of optimizing fixation methods for accurate visualization and subsequent interpretation of HCR and IHC results, with implications for probe and antibody validation and tissue-specific protei
