Previous studies have used Badal laser optometry to demonstrate significant variance in the distribution of tonic (or "dark-focus") resting positions of accommodation (TA) for groups of observers. This study investigates whether individual differences in TA are due to individual differences in autonomic tone of the ciliary muscle by comparing separately the effects of a muscarinic receptor antagonist (Tropicamide 0.5%) and a beta receptor agonist (Isoprenaline 3%) on the distribution of TA. Ten subjects were used for each study with mean ages of 23.2 and 23.0 yr, respectively. The distribution

Dopamine (DA) has been implicated in exploration-exploitation behaviour, i.e., exploring novel, potentially better options vs. exploiting known, previously rewarding options. Impairments in this trade-off occur in psychiatric disorders involving DAergic dysfunction, including addiction and schizophrenia. Pharmacological studies revealed a contribution of DA to exploration, but inconsistent findings suggest interindividual variability in baseline DA may modulate effects. To address this, we investigated the effects of the DA precursor L-DOPA on exploration-exploitation during reinforcement lear

BACKGROUND: Sleepiness behind the wheel is the second most frequent cause of fatal highway accidents when trucks are involved. Spontaneous pupillary oscillations provide objective and quantitative measures of the tonic central nervous activation which is a precondition for higher level mental performance. First experience with this method is now available from Germany and Upper Austria, with the aim to analyse the magnitude of the problem sleepiness behind the wheel.
METHODS: The pupillographic sleepiness test (PST) is a well established method in sleep research/medicine and consists of an 11

Giant cell arteritis (GCA) is the most common vasculitis of large and medium vessels affecting adults over the age of 50. Due to its acute ischemic damage through a systemic inflammatory process, GCA is a medical emergency with the risk of permanent vision loss. Therefore, early intervention is critical. Patients often present with well-documented systemic manifestations such as jaw claudication, headache, scalp tenderness, and fatigue. These patients are usually subject to blood tests for inflammatory markers and temporal artery biopsy. However, clinical manifestations vary considerably and m

This case report involves a 2-year-old female with no past medical history who presented after ingestion of an unknown amount of 300 mg bupropion extended-release tablets. Shortly after ingestion, the mother mechanically induced vomiting, which recovered only small pieces of the tablets. The patient remained asymptomatic for a short period, then she began vomiting and had several tonic-clonic seizures, leading to emergency medical service (EMS) being called and ultimately, intubation for airway protection. On arrival at the pediatric intensive care unit (estimated 6 hours post ingestion), the

INTRODUCTION: The autonomic system is frequently affected in Sjogren's syndrome (SS), but presentation with severe autonomic neuropathy is infrequent. Herein, we present a patient with primary SS-linked autonomic neuropathy, which is significantly clinic and electrophysiological responsive to immunotherapy.
CASE REPORT: A 29-year-old female patient was admitted to our neurology department with recurrent syncope, postural light-headedness, and weight loss. Neurological examination revealed tonic pupils. The baseline composite autonomic symptom score-31 was 51 (0 to 75), and baseline functional

SARS-CoV-2 associated coagulopathy may manifest as hypocoagulability with a propensity for bleeding. Here we report a COVID-19 patient with fatal subdural hematoma (SDH) shortly after thrombendarteriectomy (TEA), and anticoagulation together with anti-thrombotic treatment. A 83-year-old male developed sudden-onset fever triggering a first ever tonic-clonic seizure. His previous history was positive for diabetes, hypertension, hyperlipidemia, aorto-coronary bypass grafting, percutaneous, transluminal coronary angioplasty with stent implantation, peripheral occlusive artery disease with percutan

Ventriculo-peritoneal (V-P) shunting is an accepted procedure before a direct approach to large tumors producing obstructive hydrocephalus. However, it has been associated with some complications. Intratumoral hemorrhage is an uncommon but fatal complication of V-P shunt insertion. The exact mechanism of this complication is unknown, but several mechanisms have been proposed including rapid lowering of intracranial pressure following V-P shunt placement or cerebrospinal fluid (CSF) drainage and sudden decrease in CSF volume and pressure after V-P shunting. We report an 8-year-old girl who pres

CONTEXT: Mycoplasma pneumonia has been related to several conditions. With this study we have tried to establish the condition more frequently associated and their physiopathological mechanism.
METHOD: One-hundred and five patients admitted to Puerta de Hierro Hospital between March 1996 and July 2001, with IgM positive serology to Mycoplasma were evaluated. Thirty four cases were selected upon the basis of two criteria: patients which positivity was confirmed by seroconversion, elevation of the antibody titer, or confirmation by complement fixation were included, and patients with some inter

BACKGROUND: Idiopathic autonomic neuropathy is a severe, subacute disorder with a presumed autoimmune basis. It is indistinguishable from the subacute autonomic neuropathy that may accompany lung cancer or other tumors. Autoantibodies specific for nicotinic acetylcholine receptors in the autonomic ganglia are potentially pathogenic and may serve as serologic markers of various forms of autoimmune autonomic neuropathy.
METHODS: We tested serum from 157 patients with a variety of types of dysautonomia. Immunoprecipitation assays with iodine-125-labeled epibatidine and solubilized human neurobla

The aim of this paper is to gain better understanding of the ocular manifestations of multiple system atrophy (MSA), a neurodegenerative disorder rarely studied in terms of its ophthalmologic features. We performed a retrospective case series (1/1/05-12/31/14) to search for patients seen at Mayo Clinic, Rochester, MN, who had mention of MSA in the medical record and an eye examination, which yielded 285 cases. Of the 285, we identified 39 cases of true MSA. Each of these 39 patients was further reviewed for ocular abnormalities potentially related to MSA. Ocular findings potentially attributab

Lymphomatoid granulomatosis, a rare condition in children, affects the lungs primarily but may have significant extrapulmonary manifestations, especially in the central nervous system. We report a case of lymphomatoid granulomatosis with onset after the completion of chemotherapy for childhood acute lymphoblastic leukemia. Two months after treatment ended, the 7-year-old girl developed splenomegaly, cervical adenopathy, and bilateral interstitial pulmonary infiltrates. She improved on cefotaxime but experienced a seizure 1 month later. A computed tomography scan of the head was normal, but her

We report a 37-year-old man with cerebral infarction due to meningovascular neurosyphilis. He developed right hemiplegia and motor aphasia preceded by left retroorbital pain lasting a month. Bilateral tonic pupils were also observed. Magnetic resonance imaging (MRI) disclosed cerebral infarction in the distribution of perforating branches of the left middle cerebral artery. Abnormal enhancement was absent in the meninges on T1-weighted MRI examination. SPECT study with I-123 iodoamphetamine showed decreased perfusion in the area of the left middle cerebral artery on early phase. A delayed SPEC

Facial lacerations from minor trauma are a common presenting complaint to the emergency department. With the increasing availability of topical anesthetics, there is a decrease in the need for injectable local anesthetic and sedation services, facilitating the ease of facial laceration repairs for young children. One such topical anesthetic is a widely used pharmacy preparation of lidocaine 4%-epinephrine 0.1%-tetracaine 0.5% gel (LET). Although the use of LET has decreased the need for injectable lidocaine and sedation for facial lacerations, both of which can be unpleasant for children with

INTRODUCTION: Diphenhydramine is an antihistamine commonly implicated in overdose. It has many pharmacologic effects, including sodium channel blockade. Overdoses in toddlers causing QRS prolongation are only rarely reported and never with effective use of sodium bicarbonate. We report a diphenhydramine overdose in a toddler with multiple markers of sodium channel blockade effectively treated with sodium bicarbonate.
METHODS: A 13-month-old infant girl was brought in by the emergency medical service for a witnessed tonic-clonic seizure. Two hours previously, the child had been found with an o

Multimodal wearable sensing is increasingly used to quantify audience engagement with short-form video, yet it remains unclear which sensing channels actually carry stimulus-driven information at the exposure durations typical of this format. We recorded electrodermal activity, photoplethysmographic pulse, triaxial acceleration, video-based eye tracking, and automated facial expression analysis from 50 young adults while they viewed 20 short-form influencer clips (5.6-19.3 s) presented in individually randomized order, yielding 1000 participant × clip trials and 12 primary indices spanning fou

PURPOSE: The purpose of this study was to investigate the dynamic characteristics of accommodative behavior in non-myopic and myopic young adults under prolonged viewing of electronic devices.
METHODS: Sixteen subjects aged 18 to 25 were recruited: 8 non-myopes (-0.25 to +2.0 diopters [D]), and 8 myopes (-0.50 to -6.0 D). Refractive state was assessed using an open-field autorefractor in dynamic mode. A comprehensive evaluation of accommodative behavior included (1) pre-task tonic accommodation (TA), computed as the difference in refraction before and after 5 minutes of dark adaptation, (2) r

Cat scratch disease (CSD) is a zoonotic illness caused by the Gram negative bacillus Bartonella henselae characterized by a small skin lesion at the site of a bite, lick or scratch by a cat, commonly followed by regional lymphadenopathy 1 or 2 weeks later. We report herein on severe neurological complications of CSD combining brainstem encephalopathy and basal ganglia impairment. This 12-year-old female acutely presented to a local hospital with profound coma and a prolonged tonic posturing of extremities. On the neurological examination she was deeply comatose with pin-point pupils and lack o

A cross-sectional study is reported in which 53 patients with Holmes-Adie syndrome have been subjected to a battery of tests of autonomic nervous function referable to the cardiovascular system, to two objective tests of sweating function, and to subjective assessment of sweating by application of quinizarin powder followed by body heating. The majority of patients were consecutive referrals; none was selected because of clinical indications of autonomic dysfunction. Eighty three per cent of these patients had at least one, 57% at least two, and 40% at least three objective test abnormalities,

PURPOSE: The onset of pantothenate kinase-associated neurodegeneration (PKAN) occurs in the first and second decade of life and a pigmentary retinal degeneration is a feature of the disorder. Since the neuro-ophthalmologic and electroretinographic (ERG) features have never been well delineated, we describe them in 16 patients with PKAN.
DESIGN: Observational case series.
METHODS: Sixteen patients with genetic and neuroimaging-confirmed PKAN were examined. Ten underwent neuro-ophthalmologic examination and all had ERGs.
RESULTS: Of the 10 who underwent neuro-ophthalmologic examination, all s
