[A case of amyotrophic lateral sclerosis with bilateral vocal cord paralysis necessitating tracheotomy].

Vocal cord movement disorders are increasingly recognized in patients with amyotrophic lateral sclerosis (ALS). We describe a patient with limb-onset ALS who developed vocal cord paralysis. A 74-year-old Japanese male consulted our clinic with a 6-month history of weakness in both arms. His family history was unremarkable. There were fasciculations and mild atrophy of the tongue and both arms. In the legs, muscle strength was almost normal but widespread fasciculations were present. All tendon reflexes were hypoactive and pathological reflexes were absent. Thereafter, he developed weakness of

Temporary tracheotomy in the surgical treatment of obstructive sleep apnea syndrome: personal experience.

Aim of the study is a retrospective analysis on the use of temporary tracheotomy in our snoring surgery experience. From September 1996 to April 2002, 1103 snoring surgery procedures have been carried out on various sites of the upper airways in 530 patients (mean age 50 years, 81% males) prevalently related to severe Obstructive Sleep Apnea Syndromes (33%). Of these patients, 472 (89%) were operated upon under general anaesthesia, whereas 58 (11%) received local anaesthesia. Of the 472 patients operated upon under general anaesthesia, 17 (3.6%) underwent temporary tracheotomy, which in 10 (2.

The Role of Tracheotomy and Timing of Weaning and Decannulation in Patients Affected by Severe COVID-19.

OBJECTIVES: Patients with acute respiratory failure due to coronavirus disease 2019 (COVID-19) have a high likelihood of needing prolonged intubation and may subsequently require tracheotomy. Indications and timing for performing tracheotomy in patients affected by severe COVID-19 pneumonia are still elusive. The aim of this study is to analyze the role of tracheotomy in the context of this pandemic. Moreover, we report the timing of the procedure and the time needed to complete weaning and decannulation in our center. METHODS: This retrospective, observational cohort study included adults (≥

Indications and outcomes of paediatric tracheotomy: a descriptive study using a Japanese claims database.

OBJECTIVE: To examine the incidence of and indications for paediatric tracheotomy to clarify the disease burden relevant to tracheotomy in a population-based context. DESIGN: A descriptive analysis of a retrospective cohort. SETTING: This study utilised a nationwide claims database in Japan constructed by JMDC (Tokyo, Japan). The database includes claims data for approximately 3.75 million insured persons (approximately 3.1% of the population of Japan) comprising mainly company employees and their family members. PARTICIPANTS: We identified children registered to have undergone tracheotomy

Association of Need for Tracheotomy With Decreasing Mechanical In-Exsufflation Flows in Amyotrophic Lateral Sclerosis.

Although patients with lower motor neuron and myopathic disorders can prolong their lives by depending on continuous noninvasive ventilatory support, most patients with amyotrophic lateral sclerosis (ALS) cannot and must use tracheostomy mechanical ventilation to prolong survival. This case demonstrates that this occurs because amyotrophic lateral sclerosis patients' upper motor neuron reflex laryngeal closure and stridor cause upper airway collapse that renders mechanical insufflation-exsufflation (MIE) ineffective in expulsing airway secretions as well as for permitting continuous noninvasiv

[Tracheotomy or intubation, alternatives in the treatment of respiratory insufficiency during infectious diseases in childhood].

Tracheostomy and endotracheal intubation are complementary methods in the management of acute respiratory failure (ARF). One hundred patients (n = 100) were treated at the Intensive Care Unit for Infants and Children, University Hospital of Infectious Diseases "Dr. Fran Mihaljević" Zagreb, from 1987 to 1991. They mostly suffered from severe infectious diseases in the course of which ARF developed. Endotracheal intubation was performed in 95/100 patients, while tracheostomy was done in 11/100 patients. In the majority of subjects the course of the disease required mechanical ventilation (96%).

[Outcome of patients with chronic respiratory insufficiency treated at home with tracheotomy and assisted ventilation].

60 patients were studied with severe chronic respiratory failure (IRC) and a permanent tracheostomy treated with domiciliary ventilation (VADT). The study commenced from the time of the tracheostomy and included length of survival (by the actuarial method) quality of life, subjectively and objectively (progress of blood gases in a stable clinical state and cumulative length of periods in hospital). Factors influencing the decision for tracheostomy and VADT were examined. Restrictive cases benefitted from the technique as much from the improved quality of life as from the duration of survival (

[26 cases of tracheal stenosis after prolonged intubation or tracheotomy. Personal retrospective study].

A retrospective study over a period of 10 years, involving different hospital departments, revealed a total of 26 patients with post-intubation and/or post-prolonged tracheotomy stenosis which required surgical treatment. Predisposing factors included poor general condition, infection, iatrogenic factors related to intensive therapy (respiration, inadequate nursing) and the duration. Stenosis were situated from the sub-glottis to the carina. After surgical treatment, there were ten cases of restenosis, two multiple, and one inflammatory granuloma. Intubation may be life-saving, but must be per

Transcervical Dumon Stenting via Longitudinal Tracheotomy for Recurrent Upper Tracheal Restenosis after Failed Resection.

Post-tuberculosis airway stenosis can cause fixed cicatricial narrowing and long-term disability. We report a female patient with recurrent upper tracheal restenosis despite 2 prior resections, who presented decades later with respiratory failure; intubation was impossible, necessitating emergency tracheostomy. Imaging and bronchoscopy revealed pinhole stenosis near the second tracheal ring and left main bronchial stenosis with a destroyed left lung. The patient remained tracheostomy-dependent and aphonic after infection control. Because repeat resection was high-risk and endoscopic stent deli

Unperceived bronchial bleeding complications during percutaneous dilatational tracheotomy: a case report and 3D simulation.

Percutaneous dilatational tracheostomy is an established technique for securing the airway in critically ill patients. One of the most common complications is bleeding around the incision or after injury to major vessels in anatomic proximity. We report a case in which a thrombocytopenic patient experienced life-threatening bleeding during the procedure at the bifurcation between segmental bronchus 9 and 10, apparently caused by an unrecognized guide wire-induced mucosal lesion. Immediate extensive bronchoscopy and hemostatic interventions were required to ensure oxygenation. To better illustra

High-located aberrant innominate artery: an unusual cause of serious hemorrhage of percutaneous tracheotomy.

An aberrant innominate artery located high was observed during a cadaveric neck dissection. To our knowledge, an innominate artery crossing the 4th and 5th tracheal rings has rarely been reported in available literature. Knowledge of this anomaly is important for those who are involved with percutaneous procedures in the neck to avoid major complications.

Effect of five different body positions on lung function in stroke patients with tracheotomy.

BACKGROUND: In stroke patients with tracheotomy, reduced lung function heightens pulmonary infection risk. Body position can affect lung function; however, its impact in stroke patients with tracheostomy remains unclear. OBJECTIVE: To investigate the influence of five body positions on pulmonary function in stroke patients with tracheotomy. METHODS: Pulmonary function was assessed in five body positions (supine, supine 30°, supine 60°, sitting, and prone) in 47 stroke patients who underwent tracheotomy. Diaphragmatic excursion during quiet breathing (DEQ), diaphragmatic thickening fraction d

Emergency tracheotomy and subsequent tracheal resection and anastomosis in a blue crane (Anthropoides paradiseus).

CASE DESCRIPTION: A 7-year-old female blue crane (Anthropoides paradiseus) was initially evaluated after it had suddenly developed signs of respiratory distress following aspiration of a rock. Emergency tracheotomy had been performed, and the rock had been removed from the proximal cervical portion of the trachea. Fifty-one days later, the clinical signs had returned and the crane was reevaluated. CLINICAL FINDINGS: On reevaluation, no obvious external abnormalities were appreciated at the previous surgical site and no discharge was observed from the glottis. Computed tomography and tracheosc
