BACKGROUND: Kidney disease requiring renal replacement therapy is common after lung transplantation and is associated with reduced survival. In 2023, the Organ Procurement and Transplantation Network implemented a safety-net policy that prioritizes kidney offers for lung transplant recipients with renal failure in the first year after transplantation. The provision is likely to increase sequential transplantation. However, the long-term survival benefit of kidney transplantation for lung transplant recipients, whether sequential or simultaneous, is not well established.

PURPOSE OF REVIEW: In patients with type 1 diabetes with extreme glycemic variability, the restoration of pancreas endocrine function is potentially and completely achieved with islets of Langerhans (tissue derived from whole organ) or pancreas (whole organ) transplantation. The aim of our review is to report on the latest studies and to highlight the benefits and risks of the two procedures, providing clearer, more selective, evidence-based clinical indications that also consider the impact on the degenerative complications of diabetes as a potential benefit.

Unilateral (left) lung allotransplantation was performed on six monkeys (Macaca fascicularis). Intravenous infusion of prostaglandin E-1 into the donor and cooling of the graft by perfusing the pulmonary artery with modified Euro-Collins solution were used for lung preservation. Because of the shortage of cadaveric donors, three techniques of partial liver grafting have been developed. These techniques are placed in perspective in relation to the organ shortage. As of December 31, 2023, a total of 11,731 liver transplants were performed in 70 institutions in Japan. There were 922 deceased donor transplants (919 from heart-beating donors and 3 from non-heart-beating donors) and 10,809 living-donor transplants. As of 31 December 2017, a total of 9242 liver transplants have been carried out in 67 institutions in Japan. There were 447 deceased donor transplants (444 from heart-beating donors and 3 from non-heart-beating donors) and 8795 living-donor transplants. We performed autologous uterus transplantation using the living-sheep donor model for team preparation before human uterine transplantation. Five sequential operations (in 3 ewes) were prospectively conducted. Surgical technique included uterus retrieval, graft preparation, and uterus transplantation. Anastomoses were performed at the level of the external iliac. At 3-week follow-up, the uterus and anastomoses were checked for strictures and thrombosis.
