Increasing Rate of Pulmonary Embolism in Trauma Patients at a Level One Trauma Center: An Opportunity for Quality Improvement?

Introduction Pulmonary embolism (PE) is the most common cause of preventable hospital death in trauma patients, with 100,000 patients dying from PE annually. A steadily increasing PE rate was observed over seven years in the trauma population at a single level one trauma center. Our study seeks to analyze this trend by examining risk factors and searching for targets for improvement. We hypothesized that a change in one or more modifiable risk factors was associated with the increased PE rate.  Methods This retrospective cohort study considered trauma patients admitted to our trauma center bet

Charges, length of stay, and complication associations with trauma center ownership in adult patients with mild to moderate trauma.

BACKGROUND: For-profit (FP) trauma centers (TCs) charge more for trauma care than not-for-profit (NFP) centers. We sought to determine charges, length of stay (LOS), and complications associations with TC ownership status (FP, NFP, and government) for three diagnoses among patients with overall low injury severity. METHODS: Adult patients treated at TCs with an International Classification of Diseases-based injury severity score (ICISS) survival probability ≥ 0.85 were identified. Only those who with a principal diagnosis of femur, tibial or rib fractures were included. RESULTS: Total charge

Epidemiological pattern of pediatric trauma in COVID-19 outbreak: Data from a tertiary trauma center in Iran.

INTRODUCTION: In Iran, like most other countries, COVID-19 has had a deep impact on children's lives. Our hypothesis was that, a significant change in the number of pediatric injuries has happened in trauma centers. In the current study, we intend to identify the possible epidemiological shift in pediatric fracture patterns, by comparing the data from 'COVID-19 era' and the mean data from the past 2 years. To the best of our knowledge there are only few reports on epidemiology of pediatric fractures during the COVID-19 outbreak. METHODS: Data are reported in two sections. In the descriptive s

The Impact of Introducing a Physical Medicine and Rehabilitation Trauma Consultation Service to an Academic Level 1 Trauma Center.

OBJECTIVE: Previous retrospective studies suggest that early physical medicine and rehabilitation (PM&R) consultation for trauma patients improves outcome and reduces acute care length of stay (LOS). There have not been controlled studies to evaluate this impact. This study assesses the impact of PM&R consultations on acute trauma patients. DESIGN: This study compared measured outcomes before and after the introduction of a PM&R consultation service to the trauma program at a large academic hospital. The primary outcome measure was acute care LOS. RESULTS: The 274 historical controls and 76

Predictors of Hypothermia upon Trauma Center Arrival in Severe Trauma Patients Transported to Hospital via EMS.

Introduction: Hypothermia in severe trauma patients can increase mortality by 25%. Active warming practices decrease mortality and are recommended in the Advanced Trauma Life Support (ATLS) guidelines. Despite this, many emergency medical services (EMS) vehicles do not carry equipment necessary to perform active warming. The intent of this study was to determine the rate of hypothermia in severe trauma patients upon major trauma center (MTC) arrival, as well as to characterize factors associated with hypothermia in trauma in order to devote potential resources to those at highest risk. Methods

Evaluating Blunt Cerebrovascular Injury Screening in High-Energy Trauma Patients: A Rural Trauma Center Study.

INTRODUCTION: Blunt cerebrovascular injury (BCVI) is an uncommon but potentially devastating complication of blunt trauma. Current screening guidelines, such as the expanded Denver Criteria (eDC), rely on clinical and radiological indicators but may miss up to 20% of cases. In August 2023, our rural Level 1 trauma center implemented a new protocol that screens all blunt trauma patients with high-energy mechanisms (HEM) for BCVI. We hypothesized that the HEM protocol would identify more BCVI cases than the eDC. METHODS: This two-phase cohort study at a rural Level I trauma center evaluated BCV

Trauma Center Trauma Sensitive Yoga Intervention (TCTSY). Improving Conditions for Incarcerated Women in Latin America.

Female prisoners in Latin America experience significantly higher rates of mental health issues compared to their male counterparts. The most prevalent include anxiety and stress, which stem from feelings of insecurity, perceived failure, long periods of detention, and the inability to care for their children. Furthermore, over 50% of female prisoners have been exposed to traumatic events, including physical and/or sexual violence, and report more adverse childhood experiences and higher rates of post-traumatic stress disorder (PTSD) than male prisoners. Understanding the root causes of these
