Botfly myiasis after travel to Bolivia.

Following a trip to Bolivia, a 32-year-old woman developed a left lower leg ulcer with a sensation of movement within the lesion. After being seen by four primary care providers, she was referred to dermatology 7 weeks after her return from Bolivia. At that time, she was found to have a 5 mm weeping ulcer, with a live larva visible at the base. We conducted a punch biopsy for botfly removal, after which the patient healed well. Herein we discuss the ways in which clinical presentation, history of travel, dermoscopy, and ultrasound can contribute to diagnosing botfly myiasis. While treatment of…

Travel-associated melioidosis: a narrative review.

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, may be considered a neglected tropical disease that remains underdiagnosed in many geographical areas. Travellers can act as the sentinels of disease activity, and data from imported cases may help complete the global map of melioidosis. METHODS: A literature search for imported melioidosis for the period 2016-22 was performed in PubMed and Google Scholar. RESULTS: In total, 137 reports of melioidosis associated with travel were identified. The majority were males (71%) and associated with exposure in Asia (77%) (mainly Thailand, …

International Travel Vaccine Recommendations for Children.

Children are at risk of contracting diseases while traveling internationally. Beyond the importance of receiving routine vaccinations, physicians should also discuss with parents the effectiveness of vaccination as a strategy to protect their child against disease before travel. This article (1) explores the universally recommended routine vaccines that are particularly important for children to be up to date before travel (ie, measles, mumps, rubella; hepatitis A and B; polio; meningococcal; coronavirus disease 2019 [COVID-19]; and influenza) and (2) explains the travel-specific vaccination r…

Congenital heart disease and air travel.

The number of individuals traveling by airplanes is increasing every year. Patients with congenital heart disease and shunts, exposure to high altitude during a flight is important since it causes pulmonary vaso- constriction leading to an increase in right-to-left shunting and a decrease in arterial oxygen saturation. Patients with cyanotic congenital heart disease and Eisenmenger syndrome should be evaluated before the flight, and necessary precautions should be taken.

Travel-associated human trichinellosis in Portugal.

Trichinellosis is an emerging or re-emerging foodborne parasitic zoonotic disease caused by nematodes of the genus Trichinella. It represents a global food safety problem and a public health hazard. This case report aims to describe the first case of human trichinellosis in Portugal since the creation of the European Union surveillance system. Infection by Trichinella spp. is a rare cause of hypereosinophilia and hospital admission, but it can cause high morbidity. Clinical detailed history is crucial to obtain a correct diagnosis.

Travel history can make the difference.

Entamoeba histolytica infections are rare in developed countries such as Belgium. A 53-year-old female patient presented with 10 days of fever and mild persisting pain in the right hypochondriac despite 6 days of antibiotic therapy. The anamnesis further revealed that the patient was born in Colombia and visits her native country on a regular basis. An abdominal CT-scan demonstrated a large hepatic abscess of 10×8 cm. The diagnosis of Entamoeba histolytica- infection was confirmed with real-time PCR (RT-PCR) from the aspirated material of the abscess. Remarkably, a half year ago, this patient

[Which vaccinations for which travel-destination?].

Several vaccinations are recommended to protect international travellers, especially to tropical countries, from diseases in other parts of the world. Firstly, the routine schedule of childhood vaccinations and booster shots according to the Swiss immunisation programme should be checked, updated or even completed. Additional vaccinations against hepatitis A and B, typhoid fever, poliomyelitis, rabies or Japanese encephalitis may be recommended. This will depend on a number of factors including the exact destination and route (developing countries, rural areas), planned activities (backpacker,…

Travel behaviour of patients with haemophilia.

BACKGROUND: We aimed to identify socio-demographic, or illness-specific variables, influencing travel behaviour of haemophilic patients. METHODS: A standardised questionnaire was sent to more than 2000 members of two German Haemophilia associations. Multivariable logistic regression with the outcomes frequent (at least two journeys per year) and long-haul travel (outside of Europe) was applied separately on adult patients and patients younger than 18 years. RESULTS: Among 345 adults, high education level, living in a partnership or travelling alone was significantly associated with frequent …

[Indications for PCR in travel medicine].

The use of PCR-based molecular diagnosis in travel medicine remains limited to specific indications such as clinical suspicion of some of the viral hemorrhagic fevers (e.g. Ebola, Marburg), differential diagnosis between Entamoeba histolytica (pathogen) and E. dispar (non pathogen) in the stools, and parasitological diagnosis of cutaneous leishmaniasis. The scope of indications is likely to expand in the coming years with the development of techniques (e.g. multiplex PCR) able to identify several pathogens from a single sample. Simplification and cost-reduction of molecular techniques, which w…

[Travel related urinary schistosomiasis: case report].

Schistosomiasis is the second most frequent parasitic infection worldwide after malaria and is a major public health problem in various countries of the world. In our country due to the increasingf travel to endemic regions, the number of cases is also rising. We report herein a case of schistosomiasis who resided in Africa five years earlier and was referred to hospital with hematuria. Direct microscopic examination of the urine specimen revealed Schistosoma haematobium eggs. Epidemiology and clinical significance of schistosomiasis are also discussed.

Legionella infections and travel associated legionellosis.

Travel associated Legionnaires' disease represents a significant cause of travel associated respiratory tract infections and impacts disproportionately on otherwise healthy individuals as a consequence of their travel abroad or within their own country. Because of the propensity of these bacteria to colonize man-made water systems, legionellosis are frequently reported in travelers who stayed in accommodations sites such as hotels or cruise ships. Since the discovery of this new pathogen and the creation of surveillance networks, the number of reported travel associated legionellosis cases hav…

Tropical sprue after travel to Tanzania.

Tropical sprue (TS) is a diagnosis to consider in travelers with prolonged diarrhea and a malabsorption syndrome after return from tropical countries, particularly India and Southeast Asia. TS is an unusual condition in tropical Africa. Textbooks of tropical medicine indicate a low endemicity in Nigeria and a limited number of cases in South Africa and Zimbabwe. A Medline search from 1979 to mid 1998 using "Tanzania and tropical sprue" as key words disclosed no hits. We report herein a case of TS in a European traveler, who lived in Tanzania for 8 months.

Emerging infectious diseases and travel medicine.

International movement of individuals, populations, and products is one of the major factors associated with the emergence and reemergence of infectious diseases as the pace of global travel and commerce increases rapidly. Travel can be associated with disease emergence because (1) the disease arises in an area of heavy tourism, (2) tourists may be at heightened risk because of their activities, or (3) because they can act as vectors to transport the agent to new areas. Examples of recently recognized diseases with relationship to travel include HIV, Legionnaire's disease, cyclosporiasis, Vibr…

[Health and travel in the tropics].

BACKGROUND: Roughly one-half of all travellers to the tropics become ill, either during or after their journey. MAJOR POINTS DISCUSSED: For counseling in the doctor's office, both the particular risks of the individual (pregnancy, childhood, chronic illnesses) and the specific situation in the country to be visited, duration of stay there, and the form of travels envisaged (hotel, trekking, safari, etc.), must all be taken into account. The major infectious diseases, their prophylaxis and their treatment are described. In addition to gastrointestinal infections, malaria today heads the list o…

Travel-related infections: prevention and treatment.

Some 15 million Americans travel abroad each year, half of them to developing countries. Not only are they venturing farther afield than ever before but they are more inclined to be adventurous. A high percentage of these hardy souls acquire some form of infection. Physicians should know the risks, their geographic distribution, and both prophylactic and therapeutic strategies.
