Numerous controversies still exist in the diagnosis and management of valvular heart disease, for example, the benefit of treating moderate aortic stenosis (AS) or asymptomatic, severe AS and the appropriate selection criteria for transcatheter edge-to-edge mitral valve repair. While numerous imaging parameters have been linked to the risk of sudden cardiac death in mitral valve prolapse, their relative merits are unknown and this is a challenging population to risk-stratify. No large, randomized data exist to guide selection of patients for transcatheter repair of tricuspid regurgitation or v

INTRODUCTION AND IMPORTANCE: Mitral insufficiency is a common valve disease with a prevalence of 2% and increases after the age of 65. This is the second valvulopathy operated after aortic stenosis. The surgical management of mitral insufficiency has been completely changed in recent years. The mitral valvular replacement with was the gold standard; is currently preceded by conservative surgery thanks to the emergence of reproducible and durable surgical techniques. The aim of our study is to highlight the place of Neogortex in the treatment of mitral valvular prolapse.
METHODS: This is a ret

Tricuspid regurgitation due to nonpenetrating trauma occurred in a 60-year-old male patient who had received chest trauma in a motorcycle accident. He was admitted because of shortness of breath and palpitation on exertion. On admission physical examinations revealed pulsated and dilated jugular veins, hepatomegaly, and systolic murmur. The chest X-ray film showed an enlarged heart and electrocardiograms revealed complete right bundle branch block. Echocardiography demonstrated systolic prolapse of the tricuspid anterior leaflet into the right atrium. Right atrial v wave pressure was 20 mmHg.

The authors, who have experience of more than 1500 bidimensional echocardiographic recordings, underline the rapidity, safety and value of the method in cardiovascular disorders. The two-dimension echo provides better evaluation of the mitral area, right cavities and tricuspid valves. It is very sensitive in detecting mitral valve prolapse, pericardial effusion, intracavitary tumours and dissecting aneurysms and permits a detailed study of abnormal segmental contractions in ischaemic heart disease. Spatial and dynamic visualization coupled, if required, with contrast echocardiography is also o

Ventricular septal defects (VSD) are the most common congenital cardiac defect. Patients with large VSDs present early due to an increase in the volume load on the left ventricle and pressure load on the right ventricle. Few of them present late even without surgical intervention, due to partial restriction of perimembranous (PM) VSD, either by the septal leaflet of the tricuspid valve or by aortic valve cusp prolapse into the VSD. The authors observed a novel structure (ie, subaortic membrane in this case) restricting the large PM VSD in a 15-year-old child.

The authors report the clinical case of a 21-year-old man, with " straight back " syndrome, admitted due to prostration and fever. He developed acute pulmonary oedema and shock, caused by chordae tendineae rupture of a mixomatous and prolapsing mitral valve, with endocarditis. He had also a mixomatous tricuspid valve, reduced calibre distal thoracic aorta and celiac trunk stenosis. Mitral valve replacement and Vega annuloplasty were performed. This case illustrates the association between " straight back " syndrome and valvular and vascular disease and emphasizes that structural heart and vasc

We report the cases of eight children with Marfan syndrome. Seven (87.5%) were diagnosed with cardiopathy, everyone with auscultatory findings. Electrocardiographic patterns were nonspecific. Enlargement of the aortic root was present in two of the cases as seen by thorax x-rays. Echocardiography detected the presence of mitral valve prolapse in 87.5% of the patients (7 cases) and aortic enlargement in 75% of the patients (6 cases). Echocardiography also detected the presence of aortic dysplasia, tricuspid valve prolapse and right and left ventricular hypertrophy. During the follow-up period,

OBJECTIVE: Aortic valve replacement for aortic regurgitation (AR) has been established as a standard treatment but implies prosthesis-related complications. Aortic valve repair is an alternative approach, but its mid- to long-term results still need to be defined.
METHODS: Over a 12-year period, 640 patients underwent aortic valve repair for regurgitation of a unicuspid (n=21), bicuspid (n=205), tricuspid (n=411) or quadricuspid (n=3) aortic valve. The mechanism of regurgitation involved prolapse (n=469) or retraction (n=20) of the cusps, and dilatation of the root (n=323) or combined patholo

BACKGROUND: Valvular injuries in chest trauma mostly affect the aortic and mitral valves, but traumatic tricuspid regurgitation (TR) remains rare. This report describes the successful repair of traumatic TR secondary to papillary muscle rupture complicated with right ventricular (RV) free wall injury and atrial septal perforation.
CASE PRESENTATION: A 50-year-old male suffered blunt chest trauma from a tree fall, leading to multiple fractures, mediastinal hematoma, and hemoperitoneum caused by splenic bleeding. Given that heart failure worsened eventually, echocardiography was conducted on da

OBJECTIVES: Aortic dilatation occurs in many patients with bicuspid aortic valves. We have added root replacement using the remodeling technique originally designed for tricuspid aortic valves to bicuspid aortic valve repair for treatment of the dilated root. We compared the results of remodeling in bicuspid aortic valves with those in tricuspid aortic valves.
METHODS: From October 1995 through January 2004, 60 patients underwent root remodeling for bicuspid aortic valves (group A), and 130 patients underwent root remodeling for tricuspid aortic valves (group B). Correction of cusp prolapse w

Of multiple etiologies for pure tricuspid regurgitation, the causes may be divided into those associated with anatomically abnormal and anatomically normal tricuspid valves. Conditions associated with anatomically abnormal valves include rheumatic, floppy, Ebstein's anomaly, carcinoid, infective endocarditis, papillary muscle dysfunction, and other rarer causes such as radiation, hypereosinophilic syndrome, and endomyocardial fibrosis. Conditions associated with anatomically normal tricuspid valves include multiple causes of cor pulmonale (chronic obstructive pulmonary disease, primary pulmona

Although cardiac anomalies are well documented among children with Down's syndrome, data on the cardiac status of adults with Down's syndrome are sparse. Therefore, we performed cardiac auscultation and Doppler echocardiographic examinations in 35 asymptomatic adults with Down's syndrome. There were 25 men and ten women; their mean age (+/- SD) was 26 +/- 8 years. Only ten subjects (29%) had normal findings on examination. The most frequent abnormal findings were holosystolic mitral valve prolapse (MVP) in 20 subjects (57%) and mild aortic regurgitation in four subjects (11%). Of the 20 subjec

In 2002, a 37-year-old male with Marfan syndrome underwent the Bentall operation, total arch replacement, and aortobifemoral bypass for DeBakey type IIIb chronic aortic dissection, annuloaortic ectasia, and aortic regurgitation. In 2007, mild mitral regurgitation (MR) caused by mitral valve prolapse was identified. In April 2017, echocardiography revealed the worsening of MR and moderate tricuspid regurgitation (TR). Moreover, coronary angiography (CAG) revealed a coronary artery aneurysm in the left main trunk (LMT). In August 2017, the patient underwent mitral valve replacement (MVR), tricus

Rupture of a fibrous strand of a tricuspid aortic valve is extremely rare. We describe a patient with aortic regurgitation due to spontaneous rupture of the fibrous strands that suspend the aortic valve leaflets. The fibrous strands were recognized between the left and right coronary cusps, and between the left coronary and non-coronary cusps during the operation. The fibrous strand between the left coronary and non-coronary cusps had been ruptured, and thus the left coronary and non-coronary cusps were prolapsed into the left ventricle. Since a degenerative change was observed histologically

BACKGROUND: The involvement of the cardiovascular system in hereditary connective tissue disorders has been known for a long time and produces abnormalities that usually affect valves and arterial walls. Valvular diseases in typical autosomal dominant illnesses such as polycystic kidney (PK) have recently been reported.
METHODS: To test the prevalence of valvular prolapses in this disease, we studied [with clinical, chest x-ray, electrocardiographic, and echocardiographic (monobidimensional and Doppler) examination] 21 subjects with PK (Group A) and 34 unaffected relatives (Group B). 36 subje

IMAGE(S): Computed tomography and transthoracic echocardiography demonstrating right atrial (RA) clot-in-transit (CIT) causing dynamic tricuspid inflow obstruction in acute pulmonary embolism.
CASE SUMMARY: A 96-year-old woman presented with recurrent syncope, hypoxia, and shock requiring vasopressor support. Imaging demonstrated acute pulmonary embolism with right ventricular dilation/dysfunction and a mobile RA CIT intermittently prolapsing across the tricuspid valve. Systemic thrombolysis was contraindicated because of recent ischemic stroke. Selective RA aspiration thrombectomy removed a

Bicoronary-pulmonary artery fistulae are rare congenital coronary artery fistulae. We report the case of a 57-year-old man who presented with biventricular failure and angina pectoris. At angiography the coronary arteries were normal, but bicoronary-pulmonary fistulae were noted; severe mitral regurgitation secondary to mitral valve prolapse was also present. At right heart catheterization the calculated left to right shunt was 2.1. Fistula ligation and repair of both mitral and tricuspid valves was undertaken. The literature regarding coronary-pulmonary fistula is also reviewed.
