Tuberculous coronary arteritis as a rare cause of sudden cardiac death in young adults: a case report and a literature-based review.

BACKGROUND: Sudden Cardiac Death (SCD) is generally attributed to coronary atherosclerosis. Tuberculous Coronary Arteritis (TCA), an exceptionally rare manifestation of tuberculosis (TB), is a severe, often unrecognized, infectious cause of SCD. This report highlights TCA as a fatal etiology of SCD and reviews its key clinical and pathological features to enhance diagnostic awareness. METHODS: A regular narrative literature review was performed using the MEDLINE, Scopus, Embase, and Web of Science databases, with the following search terms: ("Tuberculous Coronary Arteritis" OR "Coronary Tuber

[A case of tuberculous abdominal aortic aneurysm which ruptured and perforated the sigmoid].

Tuberculous aneurysm is a very rare disease and the prognosis is not good. The authors carried out an emergency abdominal section on a 48 year old patient who had a serious hematochezia and vasography was performed on the patient in order to diagnose the bleeding site. However, the patient had a second serious hematochezia during vasography and fell into the state of shock. Laparotomy revealed innumerable tubercular knobs were and a seemingly tuberculous abdominal aortic knob, which ruptured and perforated to the sigmoid. The aorta was clamped above and below the knob and the knob was sutured

Bloody pericardial effusion in patients with cardiac tamponade: is the cause cancerous, tuberculous, or iatrogenic in the 1990s?

STUDY OBJECTIVES: The decrease in incidence of tuberculosis, along with the increase in invasive cardiovascular procedures, may have changed the frequency of causes of bloody pericardial effusion associated with cardiac tamponade, although this is not yet recognized by medical textbooks. We analyzed the causes of bloody pericardial effusion in the clinical setting of cardiac tamponade in the 1990s; patients' survival; the effect of laboratory results on discharge diagnosis; and how often bloody pericardial effusion is a presenting manifestation of a new malignancy or tuberculosis. DESIGN: Ret

Coronary artery disease in a patient with Addison's disease: a case report and literature review.

BACKGROUND: Addison's disease which is due to dysfunction of the adrenal gland, with abnormal secretion of glucocorticoids and mineralocorticoids, is rare. By inducing inflammation and disorders of water and electrolyte metabolism, Addison's disease may accelerate progression of co-existed cardiovascular diseases. Addison's disease combined with cardiovascular disease is infrequent, only 10 cases in the literature. CASE PRESENTATION: We reported a 51-year-old male patient with unstable angina pectoris and hypotension. Changes on coronary angiography within 2 years suggested rapid progression

Non-traumatic chest pain in patients presenting to an urban emergency Department in sub Saharan Africa: a prospective cohort study in Tanzania.

BACKGROUND: Non-traumatic chest pain (NTCP) is a common reason for emergency department (ED) attendance in high-income countries, with the primary concern focused on life threatening cardiovascular diseases. There is general lack of data on aetiologies, diagnosis and management of NTPC in Sub Sahara African (SSA) countries. We aimed to describe evaluation, diagnosis and outcomes of adult patients presenting with NTCP to an urban ED in Tanzania. METHOD: This was a prospective observational cohort study of consecutive adult (≥18 years) patients presenting with non-traumatic chest pain to the Em

Bacteriologically confirmed extra pulmonary tuberculosis and treatment outcome of patients consulted and treated under program conditions in the littoral region of Cameroon.

BACKGROUND: Extra-pulmonary tuberculosis (EPTB) is defined as any bacteriologically confirmed or clinically diagnosed case of TB involving organs other than the lungs. It is frequently a diagnostic and therapeutic challenge with paucity of data available. The aim of this study was to assess the prevalence of bacteriologically confirmed EPTB; to determine the most affected organs and to evaluate the therapeutic outcome of EPTB patients treated under program conditions in the littoral region of Cameroon. METHODS: A descriptive cross-sectional laboratory-based epidemiological survey was conducte

Diagnostic Imaging of Extrapulmonary Tuberculosis Across Organ Systems.

Extrapulmonary tuberculosis (EPTB) is an infectious disease characterized by the invasion of Mycobacterium tuberculosis beyond the lungs. Diagnosis is frequently delayed due to nonspecific clinical presentations that vary by organ system, making diagnostic imaging essential for disease detection, characterization, and treatment monitoring. The objective of this review is to examine and summarize imaging-based approaches for the diagnostic evaluation of EPTB across multiple body systems, including the central nervous system, spine, cardiovascular system, lymphatic system, abdominal and hepatic

Mycobacterium tuberculosis : a new hitchhiker in the etiopathogenesis of periodontitis.

Periodontitis, a chronic inflammatory disease of the gums affects both the ligament and alveolar bone. A severe form of periodontal disease affects a strikingly high number of one billion adults globally. The disease permutes both the soft and hard tissues of the oral cavity leading to localized and systemic diseases. Periodontitis has a deleterious impact on systemic health causing diabetes, cardiovascular diseases (CVD), and other disease. The cause of the enhanced inflammatory process is due to dysbiosis and an unregulated immune response. Innate immune response and T cells trigger uninhibi

Prevalence & potential significance of vitamin D deficiency in Asian Indians.

Studies from our center and other parts of India have drawn attention towards wide prevalence of vitamin D deficiency (VDD) in our country. VDD has been reported in all age groups including toddlers, school children, pregnant women and their neonates and adult males and females residing in rural and urban India. We reviewed implications of VDD in our population based on the preliminary data available from Indian studies on skeletal health. Besides, a brief review is made on the importance of VDD in various other disorders prevalent in equivalent proportions among Indians such as type 2 diabete

Cardiac tuberculosis. A study of 19 patients.

Myocardial tuberculosis (TB) was encountered in 19 patients (0.14%) at autopsy over a 27-year period. Eight patients had miliary lesions, and 11 had nodular lesions. Myocardial TB was diagnosed ante mortem in only one patient. Three patients developed left ventricular aneurysms. Tuberculous lesions were most commonly observed in the ventricles. Nodular tuberculous lesions produced no significant cardiac dysfunction, and the severe systemic symptoms of patients with miliary TB overshadowed the effects of cardiac miliary TB. Two additional patients with disseminated TB showed a focal lymphocytic

[Myocardial localisation of tuberculosis: the diagnostic value of cardiac MRI].

INTRODUCTION: Tuberculosis can be responsible for myocardial damage, the frequency of which is probably underestimated because of the difficulty in its diagnosis. We studied the contribution of cardiac magnetic resonance imaging (MRI) in three patients. OBSERVATIONS: Three patients were treated for disseminated tuberculosis. They had moderate cardiac abnormalities (tachycardia, dyspnoea on effort). The electrocardiogram was normal in 2 patients and the echocardiography showed localized hyperkinesias. Cardiac MRI revealed intra-myocardial nodular gadolinium enhancement and hyperkinesias. The c

[Indications for admitting patients with respiratory conditions in Sub-Saharan Africa].

INTRODUCTION: Respiratory diseases are the cause of a larger number of consultations than any other organ pathology; and general practitioners therefore spend considerable time treating them. The aim of this work was to investigate the indication for admission to hospital in patients with respiratory disease in a Sub-Saharan African context. PATIENTS: This was a retrospective observational study of the indications for admission to the pneumology department of the University Hospital of Point G. in Bamako, between January 2000 and December 2009. Admission registers were used to verify data rec

[Epidemiological, clinical, therapeutic and evolutionary aspects of pulmonary aspergilloma in Dakar].

INTRODUCTION: The presentation of pulmonary aspergilloma is dominated by haemoptysis, the results of medical treatment are disappointing and there is significant morbidity and mortality following surgery; the only really curative treatment. These facts emphasise the gravity of this condition. We undertook a study to estimate the current profile of the disease and its management in Senegal. PATIENTS AND METHODS: A retrospective study took place between January 2004 and September 2008 including all the cases of pulmonary aspergilloma diagnosed in private practice and\or the thoracic and cardiov

The Potential Importance of CXCL1 in the Physiological State and in Noncancer Diseases of the Cardiovascular System, Respiratory System and Skin.

In this paper, we present a literature review of the role of CXC motif chemokine ligand 1 (CXCL1) in physiology, and in selected major non-cancer diseases of the cardiovascular system, respiratory system and skin. CXCL1, a cytokine belonging to the CXC sub-family of chemokines with CXC motif chemokine receptor 2 (CXCR2) as its main receptor, causes the migration and infiltration of neutrophils to the sites of high expression. This implicates CXCL1 in many adverse conditions associated with inflammation and the accumulation of neutrophils. The aim of this study was to describe the significance

Mortality profiles among people living with HIV/AIDS: comparison between Rio de Janeiro and other federative units between 1999 and 2015.

INTRODUCTION: The trend toward stabilization regarding the AIDS epidemic in Brazil over the past decade hides a very complex scenario, where two-thirds of the Brazilian federative units exhibit AIDS standardized mortality rates (ASMR) significantly above the national average and/or in upward tendency. ASMR in Rio de Janeiro State remains virtually unchanged over the years; the state currently occupies the second position in the national ranking of this indicator. OBJECTIVE: To assess temporal trends in causes of death searching for differential profiles that could be useful for understanding

Medical aspects of head and neck cancer.

Certain medical aspects of advanced head and neck cancer observed in 191 patients referred to a medical oncology service over a 14-year period are presented in this review. While these tumors constitute an uncommon group of advanced neoplasms, the disability and impairment associated with this disease are great. Infections, folate deficiency, and hypercalcemia are frequent complications and generally detected. The occurrence of pulmonary tuberculosis, cardiovascular and neurologic sequelae were infrequent but may be generally under-recognized. Additionally, head and neck cancer patients were f

[The danger of infection and common medical pathology among adults in the Sahel. Niger as an example].

In Niger, the infectious risk is of real concern in the field of the pathology of the adult, mainly caused by the major endemic diseases: Parasitic diseases such as malaria, bilharziasis, cutaneous leishmaniasis and recently visceral leishmaniasis, Bacterial diseases such as enterobacterial diseases, amibiasis, meningococcal diseases, tuberculosis, leprosy and treponematosis, Virus diseases such as arbovirus diseases and probably viral hepatitis. On the whole, the rate of occurrence and prevalence are not more significant than those in the neighbouring countries. On the other hand, diseases pr

Natural and unnatural history of tetralogy of Fallot repaired during adolescence and adulthood.

Surgical repair of tetralogy of Fallot (TOF) in countries with sound medical care systems is seldom delayed until adolescence. This study investigated the clinical profile and the surgical outcomes in such a population from Taiwan. Between 1970 and 2009, 179 TOF patients (56% male) received total repair at 19.2 ± 8.3 (10-49) years of age. We reviewed the medical records and interviewed the patients concerning their current status. The survival was ascertained in all by a link to our national health database. Major morbidities before cardiac repair included atrial arrhythmia (1.1%), ventricular
