Direct application of basic fibroblast growth factor improves tympanic membrane perforation healing.

Topical application of basic fibroblast growth factor (b-FGF) on tympanic membrane (TM) perforations was studied in guinea pigs. One-millimeter simple round TM perforations or 2-mm TM perforations with medially flapped borders were performed. Either b-FGF or placebo was instilled in each ear on the day of surgery and daily thereafter. Treatment was applied either directly to the perforation or to a Gelfoam pledget over the defect. When no scaffolding material was interposed, b-FGF induced a faster healing response characterized by a hyperplastic but linear subepidermal connective tissue reacti

Nature of spontaneous tympanic membrane perforation in acute otitis media in children.

The study was undertaken to define the characteristics of spontaneous eardrum perforation in acute otitis media. Eighty (29.5 per cent) out of 271 children with acute otitis media had eardrums which perforated. An increased incidence of perforation was associated with a previous history of otitis media. All perforations were small and limited exclusively to the pars tensa. In 85 per cent of the patients, the perforation was located in the anterior-inferior quadrant. It had smooth margins enabling free drainage of pus-and was associated with a favourable clinical course. In the remaining 15 per

The impact of tympanic membrane perforations on middle ear transfer function.

PURPOSE: Injury or inflammation of the middle ear often results in the persistent tympanic membrane (TM) perforations, leading to conductive hearing loss (HL). However, in some cases the magnitude of HL exceeds that attributable by the TM perforation alone. The aim of the study is to better understand the effects of location and size of TM perforations on the sound transmission properties of the middle ear. METHODS: The middle ear transfer functions (METF) of six human temporal bones (TB) were compared before and after perforating the TM at different locations (anterior or posterior lower qua

The early events in the healing of laser-produced tympanic membrane perforation.

CONCLUSION: An immense keratinocyte activity with high cell turnover produced keratin that was delivered to span the perforation. The perforation size did not define the time to closure. OBJECTIVES: Most tympanic membrane perforations heal spontaneously, whereas a fraction remains patent. A simple, nonsurgical cure for chronic perforations is required. Better understanding of the healing processes might enable the development of a simple medical cure. Therefore the structural events of the healing process were investigated, as well as its dynamics, by comparing perforation size with time to c

Embryonic stem cells enhance the healing of tympanic membrane perforations.

OBJECTIVE: Tympanic membrane perforations may cause hearing impairment and otorhea. It is a common indication for ear surgery. The aim of the study was to test whether stem cells may enhance the healing of fresh tympanic membrane perforations. METHODS: In a first assay, the status of the tympanic membrane at 5 days after myringotomy was tested in five Mongolian gerbils that were treated on one side with embryonic stem cells and on the other side with control substance. In a second assay, nine gerbils were treated in the same way, except that fluorescent-labeled embryonic stem cells were used.

[Barofunction of auditory tube in patients with central perforation of tympanic membrane].

Patients with otitis media chronica with central tympanic perforation underwent glue myringoplasty or radical operation. In 52.2 and 33.3% of them an original technique developed by the authors, has discovered defect of barofunction of the auditory tube named inflation effect of the auditory tube. High intratympanic pressure observed in this effect should be taken into consideration in tympanoplasty as it underlies rupture or dislocation of the transplant in myringoplasty or dislocation of the neotympanic membrane in reconstructive surgery on the middle ear.

Effect of healed tympanic membrane perforations on umbo velocity in the rat.

Heterodyne laser interferometry was used to measure tympanic membrane (TM) velocity at the umbo during acoustic stimulation in 2 groups of rats. The control group had a normal TM, while in the other group the TM was perforated and then allowed to heal for at least 28 days. Umbo velocity functions for constant sound pressure level stimuli were obtained for test tones between 0.3 and 40.0 kHz in each animal. The results revealed that velocity was the same in the control and healed TMs below 3.0 kHz. Above 5.0 kHz, the velocity response in the healed ear was between 3 and 12 dB smaller than in th
