OBJECTIVE: To outline the procedural steps for a transfemoral, antegrade valve in a transcatheter aortic valve replacement procedure in a patient with no other access options.
KEY STEPS: Gain femoral venous and arterial access. Position a snare in the ascending aorta. Perform transseptal puncture. From the left atrium, deliver an 0.035-inch wire through the mitral valve and aortic valve into the ascending aorta. Snare the wire in the aorta and create a venous-arterial rail. Deliver a large-bore sheath through the mitral valve. Deliver the transcatheter valve through the sheath over the rail a

Signs which follow deep vein thrombosis and which develop as a feature of deep venous insufficiency caused by incomplete recanalization and of destruction of valves and endothelium are defined as a postthrombotic syndrome. The clinical symptoms range between no objective signs, severe oedema and ulceration. They depend on the amount of valvular and endothelial damage during the acute phase, localization of the deep vein thrombosis and on several other factors outside the venous macrocirculation which are poorly understood until now.

To determine what physiologic changes might contribute to the development of the postthrombotic syndrome, venous outflow, venous refilling time, and valvular competence were assessed in 32 patients (39 limbs) with documented deep venous thrombosis. The follow-up ranged from nine to 144 months (mean, 41 months) after the acute deep venous thrombosis. Pain was noted by 49% of the patients, but more objective end points occurred less frequently (edema, 21%; pigmentation, 26%; ulceration, 3%). Venous outflow was lower in the affected limbs but was not a good indicator of those patients with or wit

Venous ulcers affect approximately 1% of the world's population, increasing healthcare expenditures and decreasing quality of life. Several hypotheses may help explain their origin. Incompetent veins or valves or impaired muscle function may lead to abnormal calf muscle pump function that can elevate ambulatory venous pressure (venous hypertension). This hypertension subsequently results in local venous dilatation and pooling, concomitantly trapping leukocytes that may release proteolytic enzymes that destroy tissues. Venous pooling also induces interendothelial pore widening and deposition of

Fourteen patients with acute deep vein thrombosis of lower extremity (DVT) were treated with composite methods including thrombectomy (iliofemoral venous thrombectomy and ascending venous thrombectomy), thrombolytic and anticoagulant therapy. Follow-up for 2-27 months (average 11.5 months) showed excellent and good results in 78.6%, and the morbidity of post-thrombotic syndrome only 21.4%. The remarkable results were due to opening of occlusive vein of the lower extremities by thrombectomy, which could make thrombolytics effective and reduce the dose of thrombolytics to prevent bleeding compli

In patients with mechanical valves in the aortic and mitral positions, percutaneous access to the left ventricle (LV) for catheter ablation (CA) of ventricular tachycardia (VT) is challenging. We have recently described a novel percutaneous trans-right atrial (RA) access to the LV via a femoral venous approach for CA of VT in patients with mechanical aortic and mitral valves. With this approach, an iatrogenic Gerbode-type of ventricular septal defect is created with direct puncture of the inferior and medial aspect of the RA anatomically adjacent to the inferior-septal LV. The technique involv

Budd-Chiari syndrome with obstruction in the inferior vena cava causes increased venous pressure in the azygous-hemiazygous system and paravertebral venous plexus, which is transmitted to the epidural venous plexus, devoid of the valves. It causes epidural venous plexus engorgement and venous congestion and may present rarely with low back pain or radiating pain. However, patients developing lower limb weakness as a complication of Budd-Chiari syndrome is an infrequent and severe presentation. Herein, we describe a case of Budd-Chiari syndrome with chronic inferior vena cava thrombosis in a pa

Division of the morphologically left atrium (cor triatriatum) is a recognized clinical and surgical entity. Division of the right atrium (prominence of the eustachian and thebesian valves) is recognized pathologically, but is rare. It is unusual for this entity to be diagnosed during life. Stimulated by our experience with two patients seen at operation, one with an obstructive spinnaker-like formation and the other with a partitioned right atrium in the setting of pulmonary atresia, we reviewed the specimens in the heart museum of Children's Hospital of Pittsburgh that had prominence of the e

BACKGROUND: Haemoptysis is usually caused by pulmonary and infectious diseases. In few cases, it has a cardiac cause, such as pulmonary embolism or mitral valve stenosis. Haemoptysis may be an uncommon symptom of prosthetic valve dysfunction, being related to elevated right heart pressures.
CASE SUMMARY: A 22-year-old woman from sub-Saharan Africa known for a triple valve replacement was hospitalized for dyspnoea and haemoptysis. A careful clinical evaluation excluded the most common causes of haemoptysis. Transthoracic echocardiogram showed normal biventricular function, normally functioning

BACKGROUND: Thromboembolism, which leads to pulmonary embolism and ischemic stroke, remains one of the main causes of death. Ultrasound-assisted thrombolysis (UAT) is an effective thrombolytic method. However, further studies are required to elucidate the mechanism of ultrasound on arterial and venous thrombi.
METHODS: We employed the blood-on-a-chip technology to simulate thrombus formation in coronary stenosis and deep vein valves. Subsequently, UAT was conducted on the chip to assess the impact of ultrasound on thrombolysis under varying flow conditions. Real-time fluorescence was used to

The postthrombotic syndrome counts as a frequent long-term complication after deep vein thrombosis with approximately 20%-50% of affected patients after deep vein thrombosis. The earliest that diagnosis of postthrombotic syndrome can be made is 6 months after deep vein thrombosis. Most patients suffer from swelling and chronic pain. In all, 5%-10% of patients may even develop venous ulcers. The complex etiology consists of limited venous drainage because of chronic occlusions and secondary insufficiencies of venous valves inducing non-physiological venous reflux. Conservative management, first

OBJECTIVE: In animals and adult humans sustained supraventricular tachycardia leads to myocardial remodelling and dysfunction, persisting even after drug-induced cardioversion to sinus rhythm. This study was undertaken, to evaluate cardiac function in the human fetus by noninvasive determination of the degree of AV valve incompetence and venous blood flow, in order to enhance understanding of the pathophysiology of fetal supraventricular tachycardia. Furthermore, we wanted to determine the usefulness of these methods in the surveillance of these fetuses before and after drug-induced cardiovers

Cyanosis of the hands and feet in Buerger's disease is known as 'Buerger's colour'. This characteristic skin colour is produced by the subpapillary venous plexus. The reactions of the subpapillary venous plexus in patients with Buerger's disease were observed by analysing the fractional blood volume of tissue using a visible light reflective spectrophotometer. Capillary morphology was investigated using an intravital video-microscopic system. Twenty-seven subjects, comprising 13 normals and 14 patients with Buerger's disease, were studied. In the nailbeds of patients with Buerger's disease, an

The present study was designed to evaluate endothelium-dependent relaxation to the calcium ionophore A-23187 in isolated canine saphenous veins. Isometric force recordings and cGMP measurements using isolated veins with and without valves were performed. During contractions to U-46619 (3 x 10(-7) M), endothelium-dependent relaxations to A-23187 (10(-9)-10(-6) M) were significantly reduced in rings with valves compared with rings without valves. Endothelial removal abolished A-23187-induced relaxation. Relaxations to forskolin (FK; 10(-8)-10(-5) M) and diethylaminodiazen-1-ium-1,2-dionate; DEA-

Primary varicosis is due to congenital or acquired defects of the venous wall and valves. Subjective patient complaints are often different and even in contrast to clinical findings. The combination of insufficiency of the large superficial veins and insufficiency of the perforating veins often leads to chronic venous insufficiency with crural ulceration. Phlebography is indicated prior to surgery in order to evaluate the deep and the perforating veins and to check for variants. The recurrance rate for varicosis is low if all insufficient superficial and perforating veins are made functionless

Stasis dermatitis commonly occurs in older age. It is caused by venous hypertension resulting from retrograde flow due to incompetent venous valves, valve destruction, or obstruction of the venous system. Further tissue changes arise from an inflammatory process mediated by metalloproteinases, which are up-regulated by ferric ion from extravasated red blood cells. Stasis dermatitis presents initially as poorly demarcated erythematous plaques of the lower legs bilaterally, classically involving the medial malleolus. It is one of the spectrum of cutaneous findings that may result from chronic ve

Approximately 14%-20% of patients with critical lower limb ischemia are unsuited for distal arterial reconstruction and face major distal amputation. Distal venous arterialization is a unique procedure in which the venous bed is used as an alternative conduit for perfusion of peripheral tissues. Eighteen patients with stage IV Fontaine critical lower limb ischemia underwent venous arterialization. Preoperative angiographic findings confirmed the absence of any below-knee continuous arterial vessels. The most distal satisfactory artery was used for proximal bypass anastomosis, and venous valves

A successful clinical case of scalp replantation is presented where a suitable artery could not be identified in the amputated part. Reperfusion of the flap was achieved by an arteriovenous inflow anastomosis and a conventional venous anastomosis for drainage. The aesthetic and physiological success of this case suggests that arteriovenous scalp replantation is a potential avenue for treatment where arterio-arterial repair is not possible. The possible explanation for the success in this patient is the paucity of valves that exist in the venous network of the scalp and forehead, noted in our p

Deep vein thrombosis (DVT) is a major cause of morbidity and mortality in critically ill patients. DVT clots typically form in the sinus of the deep venous valves and are strongly associated with immobility and reduced venous flow. Physical activity alters venous blood flow, generating frequent periods of recirculatory flow and oscillatory shear stress (OSS) at venous valves. Endothelial sensing of OSS stimulates expression of a potent antithrombotic endothelial phenotype within the valve sinus, but the phenotype may be lost during prolonged immobility. To test whether restoring OSS-signaling
