Non-invasive Mechanical Ventilation Enhances Patient Autonomy in Decision-Making Regarding Chronic Ventilation.

OBJECTIVE: Patients with respiratory failure due to progressive muscle weakness often require chronic ventilatory support, but many do not make decisions regarding ventilation prior to a crisis. We studied the use of non-invasive ventilation as a tool to enable communication and facilitate decision-making regarding chronic ventilation. METHODS: Patients with profound muscle weakness and acute respiratory failure, were supported or weaned by non-invasive positive or negative pressure ventilation. The patients were then interviewed and their informed autonomous decisions were used to plan their

Monitoring of regional lung ventilation using electrical impedance tomography after cardiac surgery in infants and children.

Electrical impedance tomography (EIT) is a noninvasive method to monitor regional lung ventilation in infants and children without using radiation. The objective of this prospective study was to determine the value of EIT as an additional monitoring tool to assess regional lung ventilation after pediatric cardiac surgery for congenital heart disease in infants and children. EIT monitoring was performed in a prospective study comprising 30 pediatric patients who were mechanically ventilated after cardiac surgery. Data were analyzed off-line with respect to regional lung ventilation in different

Cardiopulmonary Interactions: Physiologic Basis and Clinical Applications.

The hemodynamic effects of ventilation can be grouped into three concepts: 1) Spontaneous ventilation is exercise; 2) changes in lung volume alter autonomic tone and pulmonary vascular resistance and can compress the heart in the cardiac fossa; and 3) spontaneous inspiratory efforts decrease intrathoracic pressure, increasing venous return and impeding left ventricular ejection, whereas positive-pressure ventilation decreases venous return and unloads left ventricular ejection. Spontaneous inspiratory efforts may induce acute left ventricular failure and cardiogenic pulmonary edema. Reversing

Non invasive ventilation in cardio-surgical patients.

Non invasive ventilation (NIV), primarily applied in cardiogenic pulmonary edema, decompensated COPD and hypoxemic respiratory failure, has also found a wide application in the postoperative period. The expanding indications to the transcatheter treatment of diseased left heart valves have led to an increase in cardiac interventional and diagnostic procedures in severely fragile cardiac patients. As an essential part of post cardiac surgery care is ventilatory support, NIV use has expanded to cardiosurgical patients. The objective of this study was to investigate the application and the result

Negative pressure pulmonary edema with laryngeal mask airway use: Recognition, pathophysiology and treatment modalities.

Negative pressure pulmonary edema (NPPE) following the use of the laryngeal mask airway (LMA) is an uncommon and under-reported event. We present a case of a 58-year-old male, who developed NPPE following LMA use. After biting vigorously on his LMA, the patient developed stridor upon emergence, with concurrent appearance of blood-tinged, frothy sputum and pulmonary edema. He subsequently required three days of mechanical ventilation. After discontinuation of mechanical ventilation the patient continued to require additional pulmonary support using continuous positive airway pressure, with a fu

Comparing Cardiac Output Differences in Patients With Fontan Physiology Between High and Low Tidal Volume Strategies: A Randomized, Crossover Trial.

BACKGROUND: Single ventricle congenital heart disease patients palliated to Fontan physiology lack a ventricle to generate pulmonary blood flow. Positive pressure mechanical ventilation reduces pulmonary blood and therefore cardiac output as compared to regular, negative pressure ventilation. There is controversy as to whether lower or higher tidal volumes are optimal for mechanical ventilation. AIM: This crossover trial aimed to determine whether higher versus lower tidal volumes would be optimal for systemic cardiac output. METHODS: The authors recruited Fontan patients < 18 years old pres

Global and Regional Tidal Volume Distribution in Spontaneously Breathing Mechanically Ventilated Children.

BACKGROUND: Allowing the ventilated adult patient to breathe spontaneously may improve tidal volume (VT) distribution toward the dependent lung regions, reduce shunt fraction, and decrease dead space. It has not been studied if these effects under various levels of ventilatory support also occur in children. We sought to explore the effect of level of ventilatory support on VT distribution and end-expiratory lung volume (EELV) in spontaneously breathing ventilated children in the recovery phase of their acute respiratory failure. METHODS: This is a secondary analysis of data from a prospectiv

Chronic hypoventilation syndrome: treatment with non-invasive mechanical ventilation.

Chronic hypoventilation syndrome can be caused by many disease states, although it is more commonly seen in neuromuscular disorders. Assessment of hypoventilation includes measurement of carbon dioxide level, respiratory muscle strength, pulmonary function testing, and any other system involved, such as cardiac dysfunction or sleep abnormalities. Often, chronic hypoventilation is initially diagnosed during an episode of acute respiratory failure. The use of noninvasive ventilation with positive pressure, negative pressure, or gravitational devices can be an effective treatment option for some

The detection of interatrial flow patency in awake and anesthetized patients: a comparative study using transnasal transesophageal echocardiography.

UNLABELLED: The Valsalva maneuver in the awake patient and the ventilation maneuver in the tracheally intubated anesthetized patient are two provocation methods to detect a patent foramen ovale (PFO) by means of contrast transesophageal echocardiography. In 60 patients undergoing posterior fossa surgery, a contrast agent was administered via a peripheral vein during a Valsalva maneuver immediately before anesthesia induction, followed by central venous administration during a ventilation maneuver in the same patients when anesthetized and endotracheally intubated. We evaluated both maneuvers w

An unusual case of obstructive pulmonary oedema.

A four-year-old boy was admitted to intensive care via casualty having suffered an apnoeic episode due to compression of his chest by a heavy cattle bar. A petechial rash was noted on the upper chest and arms. A chest radiograph showed pulmonary oedema which resolved over a few hours following positive pressure ventilation and positive end expiratory pressure. We believe this to be an unusual presentation of obstructive pulmonary oedemia (Negative Pressure Pulmonary Oedema).

Helmet noninvasive ventilation in acute hypoxic respiratory failure.

PURPOSE OF REVIEW: Invasive mechanical ventilation is a lifesaving intervention for patients with severe acute hypoxic respiratory failure (AHRF), but it is associated with neuromuscular, cognitive, and infectious complications. Noninvasive ventilation (NIV) may provide sufficient respiratory support without these complications. The helmet interface for NIV could address concerns raised for the use of NIV as first-line therapy in AHRF. This review will summarize and appraise the current evidence for helmet NIV in AHRF. RECENT FINDINGS: There are only six randomized controlled trials comparing

Basics of mechanical ventilation for non-aneasthetists. Part 2: Clinical aspects.

Invasive and non-invasive mechanical ventilation (MV) continues to be the most significant life support method. It is, however, coupled with many risks. Historically, concepts of MV did focus on improving the arterial blood gas results rather than preventing harmful side-effects of positive pressure ventilation. Since then, multiple studies exploring this matter emerged and led to the protective MV concept. The golden mean between assuring the best oxygenation and limiting the ventilator-induced lung injury (VILI) is still a matter of debate. These considerations are especially impactful while

Operating characteristics of the negative expiratory pressure technique in predicting obstructive sleep apnoea syndrome in snoring patients.

BACKGROUND: This study examines the operating characteristics of the expiratory flow response to a negative pressure (NEP) applied to the mouth in the prediction of obstructive sleep apnoea syndrome (OSAS) in snoring patients. METHODS: Two hundred and thirty eight patients with normal spirometric values were studied. Full laboratory polysomnography was performed and an NEP of -5 cm H(2)O was applied in the sitting and supine positions. RESULTS: A significant correlation was found between the degree of flow limitation measured by NEP in both positions (expressed as the percentage of the expir

Respiratory-related activity of soft palate muscles: augmentation by negative upper airway pressure.

We studied respiratory-related activity of the soft palate muscles in 10 anesthetized tracheostomized supine dogs. Moving time average (MTA) electromyographic (EMG) activity was measured in the palatinus (PAL), levator veli palatini (LP), and tensor veli palatini (TP) with bipolar fine-wire electrodes and in the diaphragm with bipolar hook electrodes. Measurements were made during tracheostomy breathing and nasal breathing with the mouth sealed (NB). During tracheostomy breathing, all soft palate muscles displayed respiratory-related phasic inspiratory and expiratory as well as tonic EMG activ

Double-lumen endotracheal tubes and bronchial blockers exhibit similar lung collapse physiology during lung isolation.

PURPOSE: Double-lumen endotracheal tubes (DL-ETT) and bronchial blockers (BB) are frequently used to allow one-lung ventilation (OLV) during video-assisted thoracic surgery (VATS). Recently, faster lung collapse has been documented with a BB than with a DL-ETT. The physiologic mechanisms behind this faster collapse remained unknown. We aimed to measure ambient air absorption (Vresorb) and intra-bronchial pressure (Pairway) into the non-ventilated lung during OLV using DL-ETT and BB. METHODS: Patients undergoing VATS and OLV for lung resection were randomly assigned to have measurements made o

Relationship between negative-pressure ICU rooms and the risk of COVID-19-associated pulmonary aspergillosis: an ancillary analysis of the COVID-ICU cohort study.

BACKGROUND: COVID-19-associated pulmonary aspergillosis (CAPA) has emerged as a serious complication of severe SARS-CoV-2 pneumonia in critically ill patients requiring invasive mechanical ventilation (IMV). Several host- and treatment-related risk factors have been identified, but the role of environmental factors-particularly ICU room air pressure-remains underexplored. METHODS: We conducted an ancillary analysis of the COVID-ICU study, a prospective multicenter cohort of COVID-19 patients admitted to ICUs in France, Belgium and Switzerland during the first wave of the pandemic. [Non-invasive modalities of mechanical ventilation].

When first developed in the fifties and sixties during the major polio epidemics, artificial ventilation was a major advance in medical care. The negative pressure perithoracic "iron lungs" had however many disadvantages and were widely replaced by positive pressure ventilation with intubation. This invasive technique allows normalization of gas exchange but has the disadvantage of requiring artificial sedation of the respiratory muscles in many cases in addition to the risk of secondary complications. Several authors have recently shown that in many cases, intubation may not be necessary sinc

Delayed Detection of Iatrogenic Pneumothorax Under Positive-Pressure Ventilation: A Case Report.

Subclavian venous access may result in pleural injury. During positive-pressure ventilation (PPV), increased intrathoracic pressure compresses peripheral lung tissue, promoting closure of terminal airways. As a result, air leakage from injured lung regions may remain confined and not communicate with the pleural space, rendering intraprocedural and early post-procedure imaging falsely negative. We report a 67-year-old woman in whom pneumothorax became radiographically apparent only after extubation. The transition to spontaneous (negative-pressure) breathing reduced surrounding pressure, allow
