Assessment of Left Ventricular, Right Ventricular, and Left Atrial Functions Using Strain Echocardiography in Patients With Pseudoexfoliation Syndrome.

Pseudoexfoliation (PEX) syndrome is a systemic disorder characterized by abnormal fibrillar material production in the extracellular matrix and its progressive accumulation in various organs, including ocular tissues and the myocardium. Strain echocardiography studies have shown that PEX may cause subclinical myocardial dysfunction; however, its effects on right ventricular (RV) function and left atrial (LA) dynamics remain unexplored. This study aimed to assess potential subclinical right and left ventricular (LV) dysfunction, along with alterations in LA function, in patients with PEX by mea

Pediatric Pacing from Proximal Right Ventricular Septum: Preserved Left Ventricular Systolic Function at Follow-Up.

Chronic right ventricular (RV) pacing can impair left ventricular (LV) function. Non-fluoroscopic 3D-electroanatomic mapping (3D-EAM) allows targeted catheter navigation to septal sites close to the conduction system, reducing fluoroscopy exposure. This study evaluated whether 3D-EAM-guided pacing of the proximal RV septum (RVS) preserves LV contractility in pediatric patients with systemic LV and complete atrioventricular block (CAVB). We conducted a retrospective, single-center study. Electrocardiograms and echocardiograms obtained before implantation and during follow-up were analyzed for Q

Effect of Preoperative Volume Overload on Left Ventricular Function Recovery After Ventricular Septal Defect Repair.

Significant left-to-right shunt across a ventricular septal defect (VSD) may lead to left ventricle (LV) volume overload and dilation. The acute loss of LV preload after repair of VSD may contribute to postoperative LV systolic dysfunction. The primary aim of the study is to assess the effect of presence of preoperative LV dilation on LV systolic function recovery after VSD repair. We evaluated the LV systolic function by measuring LV longitudinal strain and ejection fraction (EF) before surgery (time point 1) and at 5 time points after VSD repair (time point 2: 0 to 2 weeks, time point 3: 2 t

Influence of right ventricular pressure and volume overload on right and left ventricular diastolic function.

BACKGROUND: Ventricular interdependence may account for altered ventricular mechanics in congenital heart disease. The present study aimed to identify differences in load-dependent right ventricular (RV)-left ventricular (LV) interactions in porcine models of pulmonary stenosis (PS) and pulmonary insufficiency (PI) by invasive admittance-derived hemodynamics in conjunction with noninvasive cardiovascular magnetic resonance (CMR). METHODS: Seventeen pigs were used in the study (7 with PS, 7 with PI, and 3 controls). Progressive PS was created by tightening a Teflon tape around the pulmonary ar

Postoperative Recovery of Left Ventricular Function following Repair of Large Ventricular Septal Defects in Infants.

INTRODUCTION: Early postoperative left ventricular (LV) dysfunction is observed following repair of large ventricular septal defects (VSDs), but the frequency and rate of recovery of LV function are unknown. This study aims to characterize the incidence and rate of recovery of postoperative LV dysfunction following repair of large VSDs and to improve understanding of LV mechanics before and after VSD repair. METHODS: Infants who underwent surgical repair of an isolated large VSD were included. Pre- and postoperative echocardiographic images were reviewed, and LV function was assessed by both

Left ventricular end-systolic volume index in patients with ischemic cardiomyopathy predicts postoperative ventricular function.

BACKGROUND: We investigated the usefulness of the preoperative left ventricular end-systolic volume index (LVESVI) as a predictor of postoperative ventricular function. METHODS: We retrospectively reviewed the records of 310 patients who underwent coronary artery bypass grafting and identified 20 patients with ischemic cardiomyopathy with a preoperative ejection fraction less than 0.30. We determined the preoperative and postoperative ejection fraction, LVESVI, and left ventricular enddiastolic volume index using biplane left cineventriculography. Patients were divided into groups depending o

Left ventricular dimensions and function during exercise in dogs with chronic right ventricular pressure overload.

Left ventricular (LV) dimensions and shortening at rest and during treadmill exercise were examined before and after 4 weeks of pulmonary artery (PA) constriction in 6 conscious dogs. The dogs were preinstrumented with LV and right ventricular (RV) catheters, an LV micromanometer, a PA inflatable cuff occluder and ultrasonic crystals to measure an LV anteroposterior, a septal-lateral, an apex-base and a free wall segment chord. With PA constriction, RV pressures increased from 49 +/- 4/2 +/- 1 mm Hg (systolic/end-diastolic) to 104 +/- 5/2 +/- 1 at rest and from 71 +/- 9/2 +/- 1 to 133 +/- 8/14

Use of contrast enhancement for the assessment of left ventricular function.

Endocardial visualization is essential for accurate interpretation of regional wall thickening abnormalities, which constitute the diagnostic hallmark of coronary artery disease, and for reproducible assessment of left ventricular (LV) ejection fraction. Unfortunately, in many cardiac patients, poor image quality does not allow consistent visualization, let alone automated detection, of the endocardial boundary in all segments throughout the cardiac cycle. Continuing efforts directed towards improving endocardial visualization made by both echocardiographic imaging equipment manufacturers and

[Cardiac MRI for determining functional left ventricular parameters].

PURPOSE: To prove the accuracy of MR methods in the determination of left ventricular (LV) functional parameters and anatomy. MATERIALS AND METHODS: At 1.5 T, 20 healthy volunteers and 22 patients with aortic valvular disease (stenosis n = 15, regurgitation n = 7) were examined. Functional parameters like cardiac output, ejection fraction, end-diastolic volume, aortic flow maximum, and time interval from the R-wave to maximum flow were obtained using a velocity encoding 2D FLASH sequence (TR 24 ms, TE 5 ms, venc 250 cm/sec) and segmented breath-hold cine FLASH 2D technique (TR 100 ms, TE 4.8

Acute changes in left atrial appendage function with premature ventricular complexes.

OBJECTIVES: Left atrial appendage (LAA) dysfunction is a risk factor for stroke. Evidence shows that frequent premature ventricular complexes (PVCs) are associated with embolic stroke. Whether left atrial dysfunction is the bridging link between frequent premature ventricular complexes and stroke is unknown. MATERIALS AND METHODS: Patients with a structurally normal heart undergoing elective electrophysiology study were included. Transoesophageal echo was used to measure LAA flow velocities. To simulate PVCs in bigeminal rhythm, single paced beats were delivered from the right ventricle with

Machine Learning-Based Prediction of Early Left Ventricular Function After STEMI.

Background: Left ventricular (LV) function and lactate dynamics are major prognostic markers after ST-segment elevation myocardial infarction (STEMI). Early identification of patients at risk for impaired LV function or systemic hypoperfusion may improve outcomes. Machine learning (ML) can enhance predictive accuracy beyond traditional statistical methods, yet most prior studies were limited by small sample sizes and categorical outcomes. Methods: We retrospectively analyzed 2132 consecutive STEMI patients admitted to LMU Hospital (2014-2023). After preprocessing, 1608 patients with complete d

Left bundle branch area pacing improves right ventricular function and synchrony.

BACKGROUND: The impact of left bundle branch area pacing (LBBAP) on right ventricular (RV) function and tricuspid regurgitation (TR) remains unclear. OBJECTIVE: We aimed to assess the long-term effects of LBBAP on RV performance and on TR. METHODS: RV function was evaluated using RV free wall strain, tricuspid annular plane systolic excursion, fractional area changing, and systolic velocity of the lateral tricuspid annulus. The presence of reverse septal flash (RSF) and basal bulge (BB) was used to assess RV motion pattern. The distance between the lead entry site on the interventricular sep

A Novel Continuous Left Ventricular Diastolic Function Score Using Machine Learning.

BACKGROUND: Unlike left ventricular (LV) ejection fraction, which provides a precise, reliable, and prognostically valuable measure of systolic function, there is no single analogous measure of LV diastolic function. OBJECTIVES: We aimed to develop a continuous score to grade LV diastolic function using machine learning modeling of echocardiographic data. METHODS: Consecutive echo studies performed at a tertiary-care center between February 1, 2010, and March 31, 2016, were assessed, excluding studies containing features that would interfere with diastolic function assessment as well as stud

Left Ventricular Diastolic Function in Subjects Conceived through Assisted Reproductive Technologies.

Subjects conceived through assisted reproductive technologies (ART) potentially suffer from impaired left ventricular (LV) function due to premature vascular aging. This study aimed to evaluate whether subtle differences in LV diastolic function can be observed echocardiographically between young ART subjects and their spontaneously conceived peers. The echocardiographic assessment included the measurement of LV dimensions, mitral inflow velocities, and myocardial velocity at early diastole (E', cm/s) at the LV wall and the interventricular septum (IVS). An average from E/E'LV and E/E'IVS (E/E

Determinants of left ventricular function improvement for cardiac resynchronization therapy candidates.

AIMS: A waiting period of more than 3 months is recommended for patients before undergoing cardiac resynchronization therapy (CRT). However, due to an anticipated high mortality rate, early implementation of CRT might be beneficial for some patients. We aimed to evaluate the rate and the probability of left ventricular (LV) function improvement and their predictors in patients with heart failure (HF) with indications for CRT. METHODS AND RESULTS: From March 2011 to February 2014, a total of 5625 hospitalized patients for acute HF were consecutively enrolled in 10 tertiary hospitals. Among the

Longitudinal Assessment of Right Ventricular Function in Hypoplastic Left Heart Syndrome.

Overall survival of patients with hypoplastic left heart syndrome (HLHS) has shown continued improvement. Right ventricular (RV) dysfunction, in the long term, adversely affects prognosis in these patients. This study examines changes in echocardiographic markers of RV function in a longitudinal cohort. We retrospectively reviewed patients with HLHS managed at our institution from 7/1994 to 1/2016. Follow-up included surgical and clinical data, and echocardiographic measures. Measures of RV function preceding and following all three stages of single ventricular palliation were collected. Freed

Left Ventricular Function After Repair of Totally Anomalous Pulmonary Venous Connection.

BACKGROUND: Poor left ventricular (LV) growth and diastolic dysfunction long after simple total anomalous pulmonary venous connection (TAPVC) repair has been well documented and is believed to originate from insufficient preoperative volume preload. The objective of the study was to confirm these findings. METHODS: Of 61 patients undergoing simple TAPVC repair between 1996 and 2016, 42 patients undergoing postoperative catheter examinations were enrolled. The mean age at the time of repair was 39 ± 117 days. Postoperative catheter examinations were conducted at a mean duration of 1.1 years af

[Chronic heart failure with impaired left ventricular function (systolic heart failure)].

Chronic heart failure is a common public health problem. The disease has a poor prognosis with high mortality rate and the incidence increases continuously. Prognosis of chronic systolic heart failure can be improved by several different medications as well as by special cardiac interventions based on the newly-published European and American guidelines. In case of severe systolic dysfunction, hospitalization and mortality can be reduced using angiotensin converting enzyme inhibitors, angiotensin receptor blocking drugs, beta-receptor blocking agents and aldosterone antagonists, as evidenced i
