Verapamil, an L-type calcium channel blocker, has been used successfully to treat cardiovascular diseases. Interestingly, we have recently shown that treatment of cancer cells with verapamil causes an effect on autophagy. As autophagy is known to modulate chemotherapy responses, this prompted us to explore the impact of verapamil on autophagy and cell viability in greater detail. Six cases of acute verapamil poisoning are reported. The dose ingested ranged between 1.2 and 9.6 g. In all cases other drugs had also been ingested and especially betablockers in two cases. Twenty patients, seven of them with paroxysmal supraventricular tachycardia; 8 with paroxysmal auricular flutter; 3 with paroxysmal auricular fibrillation and 2 with paroxysmal atrial tachycardia, with stage II of A-V block, were treated intravenously with isoptin, with an average dose of 0.1 mg/kg body mass. We studied the effects of two verapamil analogs, anipamil and ronipamil, in traumatic shock. Noble-Collip drum trauma produced a shock state characterized by an eight-fold increase in plasma cathepsin D activity, a 13-fold increase in the rate of plasma myocardial depressant factor (MDF) accumulation, and a survival time of 1.9 ± 0.1 hours. A patient had continuous paroxysms of the specific ventricular arrhythmia of the "Torsades de Pointes" type during attacks of variant angina. The arrhythmia was refractory to lidocaine and procainamide and only responded dramatically to verapamil, initially intravenous and afterwards oral. The low success rate in the treatment of such arrhythmias by other drugs and available experimental data suggest that verapamil should be further evaluated.

BACKGROUND: Calcium Channel Blockers (CCBs) are now widely employed in the treatment of cardiovascular diseases and peri operative hypertension. It has been reported that calcium channel blockers inhibit neuromuscular transmission. They have been shown to increase the neuromuscular blockade produced by neuromuscular blocking agents in in-vitro muscle nerve preparations. The present study is undertaken to demonstrate the effect of calcium channel blocker, verapamil on neuromuscular transmission in albino rats.
