Developing prosthetics to treat cognitive disabilities resulting from acquired brain injuries.

Persistent cognitive disabilities represent the most troublesome consequences of acquired brain injury. Although these problems are widely recognized, few neuroprosthetic efforts have focused on developing therapeutic strategies aimed at improving general cognitive functions such as sustained attention, intention, working memory or awareness. If possible, effective modulation of these neuropsychologic components might improve recovery of interactive behaviors. The emerging field of neuromodulation holds promise that technologies developed to treat other neurological disorders may be adapted to

Involvement of the cranial nerves and their nuclei in spinocerebellar ataxia type 2 (SCA2).

Although the cranial nerves, their nuclei and related fiber tracts are crucial for a variety of oculomotor, somatomotor, somatosensory, auditory, vestibular-related, autonomic and ingestion-related functions, knowledge regarding the extent of their involvement in spinocerebellar ataxia type 2 (SCA2) patients is incomplete. Accordingly, we performed a pathoanatomical analysis of these structures in six clinically diagnosed SCA2 patients. Unconventionally thick serial sections through the brainstem stained for lipofuscin pigment (aldehyde-fuchsin) and Nissl material (Darrow red) showed that all

Self-motion perception is sensitized in vestibular migraine: pathophysiologic and clinical implications.

Vestibular migraine (VM) is the most common cause of spontaneous vertigo but remains poorly understood. We investigated the hypothesis that central vestibular pathways are sensitized in VM by measuring self-motion perceptual thresholds in patients and control subjects and by characterizing the vestibulo-ocular reflex (VOR) and vestibular and headache symptom severity. VM patients were abnormally sensitive to roll tilt, which co-modulates semicircular canal and otolith organ activity, but not to motions that activate the canals or otolith organs in isolation, implying sensitization of canal-oto

A reduction of vestibulo-visual integration during transdermally administered scopolamine and dimenhydrinate. A presentation of gain control theory in motion sickness.

The effect of transdermally administered scopolamine (TTS scopolamine) (release rate 5 micrograms/h) and dimenhydrinate (100 mg) was examined on optovestibular nystagmus in 16 volunteers in a randomized double-blind trial. A statistically significant decrease in the optokinetic part of nystagmus was observed during all treatments. Most profound reduction was found during treatment with two TTS scopolamine. The vestibular part was reduced by treatment with two TTS scopolamine, only. The results indicate that scopolamine and dimenhydrinate exert their effect in motion sickness by reducing the ve

Mechanisms underlying the antimotion sickness effects of psychostimulants.

The major conclusions of this review are: 1) that selective enhancement of dopaminergic transmission, not noradrenergic, is sufficient to account for amphetamine-induced resistance and perhaps natural resistance to motion sickness; 2) the site of this enhanced dopaminergic transmission is probably within the basal ganglia; and 3) the neuropharmacology of the basal ganglia, but not the brainstem vestibular areas, can account for the therapeutic synergism of scopolamine and amphetamine. The therapeutic actions of psychostimulants may be dissociable from some of their side effects, particularly c

Observations on the secondary vestibulocerebellar projections in the macaque monkey.

The distribution of retrogradely labeled cells in the nuclei of the vestibular nuclear complex following injections of horseradish peroxidase in various parts of the cerebellar cortex (except the nodulus and paraflocculus) has been mapped in the macacus rhesus monkey. In the main the findings correspond to those made in other mammalian species (cf. Table 1). The flocculus receives afferents bilaterally from the superior, medial and descending vestibular nucleus, group y, the interstitial nucleus of the vestibular nerve and also from the abducent nucleus. The projection to the posterior vermis

Vestibulotrigeminal pathways in the frog, Rana esculenta.

The aim of this study was to investigate whether primary vestibular afferent fibers establish direct connections with the motor and sensory trigeminal system in the brainstem of the frog. The experiments were carried out on Rana esculenta. In anaesthetized animals the trigeminal and vestibular nerves were prepared, and their proximal stumps were labeled either with fluorescein binding dextran amine (trigeminal nerve) or tetramethylrhodamine dextran amine (vestibulocochlear nerve). With a confocal laser scanning microscope we could detect close connections between the vestibular fibers and bran

The vestibular system: Unsteadiness, dizziness, and vertigo.

This chapter provides a detailed overview of the vestibular system's anatomy, physiology, and clinical relevance, emphasizing how inner-ear sensory structures and brainstem pathways interact to maintain balance, gaze stability, and spatial orientation. It begins with the peripheral vestibular apparatus, describing the semicircular canals and otolith organs in detecting angular and linear accelerations. These signals are transmitted via the vestibulocochlear nerve to the vestibular nuclei at the pontomedullary junction, where they integrate with visual and somatosensory inputs to drive coordina

Postvibration changes in the spike activity of neurons in the medial vestibular nucleus.

Changes in the baseline spike activity in the medial vestibular nucleus were studied after exposure to vibration for 5, 10, and 15 days. Studies addressed the distributions of neurons within this structure in terms of the regularity of their spike activity, the dynamics of neural flows, and the modalities of interspike histogram modalities. Mean values were calculated for the major statistical measures of baseline spike activity. Statistically significant changes in the major characteristics of neuron activity were seen in the medial vestibular nucleus at different periods of vibration exposur

Projections of purkinje cells in the translation and rotation zones of the vestibulocerebellum in pigeon (Columba livia).

Previous electrophysiological studies have shown that the pigeon vestibulocerebellum (ventral uvula, nodulus, and flocculus) can be divided into two parasagittal zones based on responses to optic flow stimuli. The medial zone (ventral uvula and nodulus) responds best to optic flow resulting from self-translation, whereas the lateral zone (flocculus) responds best to optic flow resulting from self-rotation. In this study we investigated the projections of the Purkinje cells in the translation and rotation zones of the vestibulocerebellum by using the anterograde tracer biotinylated dextran amin

Reversed postoptokinetic nystagmus: a model of plasticity in the vestibuloocular system.

A simple manifestation of the memorial processes is the so called reversed postoptokinetic nystagmus (RPN), a trace phenomenon elicited in the rabbit by prolonged (60 min) optokinetic (OK) stimulation. Electrophysiological analysis of RPN indicated that the underlying neural trace is weakened, but not suppressed by spreading depression in the cerebral cortex or superior colliculus. An asymmetry of RPN is brought about by unilateral 6-OHDA lesion on substantia nigra. Electroconvulsive shock applied immediately after a period of OK stimulation blocks the subsequent RPN without interfering with O

Projections from the supragenual nucleus to the lateral mammillary and dorsal tegmental nuclei.

An organism's sense of direction depends on vestibular input to thalamic and forebrain structures. The supragenual nucleus (SGN) sits at a prime location in the network to convey vestibular signals forward to update the representation of head direction (HD) with ongoing head movement. The SGN receives anatomical projections from the medial vestibular nuclei and nucleus prepositus hypoglossi and prior reports have suggested that the SGN sends projections to both the dorsal tegmental nucleus (DTN) and lateral mammillary nucleus (LMN), two midbrain nuclei crucial in generating a representation of

Brainstem representation of vestibular evoked myogenic potentials.

OBJECTIVE: Vestibular evoked myogenic potentials (VEMPs) are caused by a short-latency reflex recorded from averaged electromyography from the sternocleidomastoid muscle evoked by intense auditory clicks. Besides peripheral vestibulopathy, abnormal VEMPs can be caused by lesions of the brainstem. The aim of this study was to analyze the topology of ischemic brain lesions generating pathological VEMPs. METHODS: Twenty-nine patients with brainstem infarcts were prospectively studied using VEMPs and MR imaging to evaluate the brainstem representation of the VEMP reflex. Individual brainstem lesi

Vestibular nucleus projections to nucleus tractus solitarius and the dorsal motor nucleus of the vagus nerve: potential substrates for vestibulo-autonomic interactions.

Autonomic effects of vestibular stimulation are important components of phenomena as diverse as acute vestibular dysfunction and motion sickness. However, the organization of neural circuits mediating these responses is poorly understood. This study presents evidence for direct vestibular nucleus projections to brain stem regions that mediate autonomic function. One group of albino rabbits received injections of Phaseolus vulgaris leucoagglutinin into the vestibular nuclei. The tracer was visualized immunocytochemically with standard techniques. Anterogradely labeled axons from the caudal medi

Subjective postural vertical in peripheral and central vestibular disorders.

The perception of subjective postural vertical was assessed in normals and patients with peripheral and central vestibular disorders and spasmodic torticollis. The subjects were seated in a motorized gimbal with the head and torso restrained and their eyes closed. The gimbal executed 7-10 cycles of tilt around the vertical at 1.5 degrees/s in either pitch or roll. Subjects indicated when they began to feel upright and again when they began to feel tilted by an analogous 3-position joystick. Normal subjects felt upright within a sector of 5-6 degrees around vertical in pitch and roll. Five pati

Morphology of the facial motor nuclei in a rat model of autism during early development.

The development of facial nuclei in animal models of disease is poorly understood, but autism is sometimes associated with facial palsy. In the present study, to investigate migration of facial neurons and initial facial nucleus formation in an animal model of autism, rat embryos were treated with valproic acid (VPA) in utero at embryonic day (E) 9.5 and their facial nuclei were analyzed by in situ hybridization at E13.5, E14.5 and E15.5. Signals for Tbx20, which is expressed in early motor neurons, appeared near the floor plate at the level of the vestibular ganglion and extended caudolateral

Calcitonin gene-related peptide in the audiovestibular system.

OBJECTIVE: This study aimed to summarize and analyze the current literature related to calcitonin gene-related peptide (CGRP), the vestibular system, and vestibular migraine. BACKGROUND: CGRP is a neuropeptide that has been implicated in the pathophysiology of migraine. Vestibular migraine (VM) is a subtype of migraine that causes recurring episodes of vestibular symptoms like vertigo and is often associated with migrainous symptoms. Although the pathophysiology of VM is not completely understood, CGRP expression in the central and peripheral vestibular systems has suggested that it may also

Spontaneous unilateral brainstem infarction in Swiss mice.

Spontaneous vestibular syndrome in mice, characterized clinically by head tilt, circling or rolling, can be caused by otitis media, arteritis or central nervous system lesions. Postmortem examination of eleven non-inbred Swiss mice submitted for necropsy due to acute onset of vestibular signs revealed lesions consistent with brainstem infarction. The lesions were characterized by unilateral, well-demarcated areas of necrosis, malacia, and gliosis, with variable amounts of hemorrhage, in the lateral aspect of the medulla and caudal pons. The affected area included the medial, lateral and superi
