Vigabatrin; its effect on the electrophysiology of vision.

Vigabatrin is known to induce visual field defects in approximately one third of patients treated with the drug. It is apparent from electrophysiological studies that the cause of this defect is at retinal level probably as a result of the build up of GABA. Studies of electrophysiological retinal parameters such as the EOG and photopic, scotopic and 30-Hz flicker ERG have revealed changes in Arden Index, photopic a and b wave latency and amplitude, changes in oscillatory potentials, and changes in latency and amplitude of the 30Hz response. However, many of these changes such as the Arden Inde

Functional ability profiles for driver licensing. Exemplification by visual profile.

The impaired driver is evaluated for driver licensing according to a functional ability profile. This test correlates, in increments, the physical or medical ability of the driver with the risk categories and the vehicles the driver is permitted to operate. The person is responsible for his fitness to drive. The physician may help in patient evaluation by participation in a medical advisory board or in public education. Guidelines for 11 areas of potential driving impairments were developed in the state of Utah. These principles are illustrated with visual standards.

Assessment of visual function in autistic children.

Children with autism demonstrate "atypical" gaze or social "looking" and frequently manifest such sterotypies as eye pressing, hand flicking, and light gazing. This study's purpose was to evaluate autistic children for visual dysfunction that may be related to the manifested visual signs and symptoms. Thirty-four autistic children, ages 2 to 11 years (median age = 7 years, 6 months) were evaluated for ocular alignment, refractive error, visual acuity, oculomotility skills, and stereopsis. None of the children manifested ocular disease, known seizure disorders, or dysmorphic features. Their dev

Concurrent visual conversion reaction and simulated colour vision defects in a 12-year-old child.

Visual conversion reaction (VCR) is a psychosomatic anomaly which occasionally manifests in children, most commonly in the form of reduced vision and visual field defects. Malingering or conscious simulation for secondary gain would appear to be rare in children but occasionally VCR and simulation coexist. A case history is presented of a 12-year-old child with behavioural and reading difficulties who manifested reduced vision most probably attributable to VCR and severe colour deficiency, which was best explained in terms of simulation or malingering.

Impact of type 1 diabetes on binocular vision: Evidence from a comparative analysis.

Binocular vision function was assessed in individuals with type 1 diabetes (T1D) without retinopathy and compared to a control group to identify potential nonstrabismic binocular vision disorders. This cross-sectional study included 80 participants (40 T1D, 40 controls) without systemic or ocular diseases, visual impairments, medications, or prior ocular surgery. Highly repeatable tests evaluated heterophoria, the accommodative convergence/accommodation (AC/A) ratio, near point of convergence (NPC), fusional vergences, vergence facility, and stereoacuity. Diagnosed dysfunctions included conver

Binocular Vision Anomalies in Scotland at Age 3.5-5.5 Years: An Epidemiological Study.

PURPOSE: Scotland has comprehensive vision screening at age 3.5-5.5 years, with ~85% participation (40,000-50,000 episodes annually). Orthoptists deliver the screening, including presenting vision and tests for binocular vision anomalies (BVAnom). The aims were to investigate (1) changes in the prevalence of BVAnom, 2013-2022 and (2) whether less comprehensive screening of solely presenting vision would detect BVAnom. METHODS: Data from eight Scottish Health Boards were available for 2013-2014, 2014-2015, 2015-2016, 2020-2021 and 2021-2022. Binocular vision tests included cover test, ocular m

A school of red herring.

A 66-year-old man with chronic kidney disease presented with painless unilateral vision loss and bilateral optic disk swelling. Vision loss progressed in the affected eye and developed in the fellow eye. Evaluation for infectious, neoplastic, ischemic, and inflammatory disorders was unrevealing. High-dose corticosteroids appeared to stabilize his vision temporarily. Eventually he was diagnosed with papilledema in the setting of transverse venous sinus stenosis and dural arteriovenous fistula. His papilledema resolved, and his vision improved following stenting of the venous sinus stenosis and

Visual Neuroprosthesis - Stimulation of Visual Cortical Centers in The Brain. Design of Non-Invasive Transcranial Stimulation of Functional Neurons.

PURPOSE: The purpose of the article is to present the history and current status of visual cortical neuroprostheses, and to present a new method of stimulating intact visual cortex cells. METHODS: This paper contains an overview of the history and current status of visual cortex stimulation in severe visual impairment, but also highlights its shortcomings. These include mainly the stimulation of currently damaged cortical cells over a small area and, from a morphological point of view, possible damage to the stimulated neurons by the electrodes and their encapsulation by gliotic tissue. Harmonization of Outcomes and Vision Endpoints in Vision Restoration Trials: Recommendations from the International HOVER Taskforce.

Translational research in vision prosthetics, gene therapy, optogenetics, stem cell and other forms of transplantation, and sensory substitution is creating new therapeutic options for patients with neural forms of blindness. The technical challenges faced by each of these disciplines differ considerably, but they all face the same challenge of how to assess vision in patients with ultra-low vision (ULV), who will be the earliest subjects to receive new therapies. Historically, there were few tests to assess vision in ULV patients. In the 1990s, the field of visual prosthetics expanded rapidly

[LAMINAR LOCATION OF NEURONS PROVIDING INTERHEMISPHERIC CONNECTIONS IN THE VISUAL CORTEX IN CATS WITH IMPAIRMENTS OF BINOCULAR VISION].

The distributio of cells in the visual cortical layers of intact cats (n=7) and cats with experimentally induced strabismus (n=10) and monocular deprivation (n=5) was studied after microiontophoretic injection of horseradish peroxidase into the ocular-dominance columns in areas 17, 18 and the transition zone 17/18. It was found that in cats with impaired binocular vision, the callosal cells were located deeper in layers of I/II, and higher - in layer IV, as compared to those in intact cats. Also in cats with impaired binocular vision, the proportion of callosal cells in layer IV was increased,

Screening for correctable visual acuity deficits in school-age children and adolescents.

BACKGROUND: Although the benefits of vision screening seem intuitive, the value of such programmes in junior and senior schools has been questioned. In addition to this there exists a lack of clarity regarding the optimum age, and frequency at which to carry out screening. OBJECTIVES: The objective of this review is to evaluate the effectiveness of vision screening programmes carried out in schools in reducing the prevalence of undetected, correctable visual acuity deficits due to refractive error in school-age children. SEARCH STRATEGY: We searched the Cochrane Central Register of Controlle

A review of ocular malingering and hysteria for the flight surgeon.

Malingering is the conscious creation of a functional defect or denial of a true defect. Positive malingering is the former, and negative malingering is the latter. Hysteria is similar to malingering except that it is unconscious. Ocular malingering is common among draft evaders. The flight surgeon or military ophthalmologist will more commonly see the negative malingerer in peacetime because of the high physical standards for flying. Wartime creates a sudden shift towards positive malingering, however, for obvious reasons. This paper carefully defines the distinctions between malingering, hys
