Current Concepts for Nerve-Based Procedures to Treat Upper Limb Spasticity.

This article reviews nerve-based surgical approaches for treating upper-limb spasticity, focusing on their anatomic targets and functional outcomes. Central procedures, like selective dorsal rhizotomy and contralateral C7 transfer, reduce tone at the root level but lack muscle specificity. In contrast, peripheral techniques-including hyperselective neurectomy, neurotomy, and nerve transfers-enable targeted modulation of spastic muscles while preserving strength or restoring volition. These interventions provide durable, functionally meaningful results and can be combined with other therapies a

What doesn't kill you makes you stronger and weaker: how childhood trauma relates to temperament traits.

Childhood trauma arises from damaging experiences and the absence of a secure and reliable environment for child development. Despite many studies on the risk for developing psychiatric disorders, much less is known on the relationship between traumatic experiences and personality traits. A total of 10,800 participants (mean age 27.7 ± 7.8 years old, 69.8% women) anonymously answered the Childhood Trauma Questionnaire (CTQ) and the Affective and Emotional Composite Temperament Scale (AFECTS) in the Brazilian Internet Study on Temperament and Psychopathology (BRAINSTEP). The results showed that

Tourette's Syndrome as a vehicle in the search for the neural correlates of consciousness.

The neurological underpinnings of Tourette's Syndrome offer an avenue to identify the neural correlates of consciousness. Neuroimaging of Tourette's Syndrome patients during the volitional control of premonitory urges highlight the involvement of the non-language dominant prefrontal portion of the frontal operculum of the inferior frontal gyrus and the anterior insular cortex in tic inhibition and decision-making processes. Further studies reveal these regions are crucial for intention, volitional control, self-awareness, perception of tangible reality, and attentional selectivity, suggesting

Gait initiation with electromyographically triggered electrical stimulation in people with partial paralysis.

Functional electrical stimulation (FES) facilitates ambulatory function after paralysis by activating the muscles of the lower extremities. Individuals with incomplete spinal cord injury (iSCI) retain partial volitional control of muscles below the level of injury, necessitating careful integration of FES with intact voluntary motor function for efficient walking. The FES-assisted stepping can be triggered automatically at a fixed rate (autotrigger), by a manual switch (switch-trigger), or by an electromyogram-based gait-event-detector (EMG-trigger). It has been postulated that EMG may be a mo

Sex differences in the latent structure of suicide risk among patients with mood disorders: taxometric analyses using the ideation-to-action framework.

BACKGROUND: Previous taxometric studies have yielded inconsistent findings regarding the empirical support for the common clinical practice of categorizing patients into discrete suicide risk groups (low versus high risk). Furthermore, potential sex differences in these latent structures have not been adequately explored. This study aimed to investigate the latent structure of suicide risk based on motivational and volitional phase symptoms from the ideation-to-action framework, and to explore potential sex differences in these latent structures, in order to determine whether the clinical prac

Feasibility of predicting improvements in motor function following SCI using the SCAR outcome measure: a retrospective study.

STUDY DESIGN: A retrospective study. OBJECTIVES: To assess improvement in volitional motor function after SCI, using The Spinal Cord Ability Ruler (SCAR) as a metric and investigate participant characteristics and recovery of motor functioning. SETTING: A highly-specialized SCI rehabilitation unit (Spinal Cord Injury Centre of Western Denmark, SCIWDK). METHODS: Retrospectively, data on all SCI patients admitted to SCIWDK between 1 January 1997 and 1 November 2018 were extracted from a database. The SCAR score (range: 0-100) was calculated by combining items from ISNCSCI and SCIM. RESULTS:

Finite-State Impedance and Direct Myoelectric Control for Robotic Ankle Prostheses: Comparing Their Performance and Exploring Their Combination.

Non-volitional control, such as finite-state machine (FSM) impedance control, does not directly incorporate user intent signals, while volitional control, like direct myoelectric control (DMC), relies on these signals. This paper compares the performance, capabilities, and perception of FSM impedance control to DMC on a robotic prosthesis for subjects with and without transtibial amputation. It then explores, using the same metrics, the feasibility and performance of the combination of FSM impedance control and DMC across the full gait cycle, termed Hybrid Volitional Control (HVC). After calib

Couples' daily self-regulation: The Health Action Process Approach at the dyadic level.

OBJECTIVE: Regulating health behavior change often occurs in a dyadic context of romantic relationships. Dyadic approaches to standard health behavior change models are, however, barely considered. We investigated volitional processes of the Health Action Process Approach model for two health behaviors within a dyadic context of romantic couples. Specifically, we tested whether day-to-day volitional self-regulation predicted one's own and one's partner's cigarettes smoked (Study 1) and physical activity (Study 2). METHODS: In two dyadic intensive longitudinal studies (Study 1: 83 dual-smoker

Kindled non-convulsive behavioral seizures, analogous to primates. A 24th case of 'limbic psychotic trigger reaction': bizarre parental infanticide--might nonvoluntariness during LPTR become objectified by primate model?

Limbic psychotic trigger reaction (LPTR) includes paroxysmal, out-of-character, motiveless, unplanned felonies (or similarly bizarre social misbehavior), all committed during flat affect, autonomic arousal and a fleeting de novo psychosis. A transient limbic hyperactivation is implicated that impairs prefrontal monitoring (judgment, planning, intent, volition, emotional participation) but preserves memory for the acts. It is hypothesized that LPTR implicates an atavistic regression to a limbic 'paleo-consciousness', exemplified by a 24th patient (parental infanticide), presented herein. He had

Voluntary action influences visual competition.

Converging lines of evidence point to a strong link between action and perception. In this study, we show that this linkage plays a role in controlling the dynamics of binocular rivalry, in which two stimuli compete for perceptual awareness. Observers dichoptically viewed two dynamic rival stimuli while moving a computer mouse with one hand. When the motion of one rival stimulus was consistent with observers' own hand movements, dominance durations of that stimulus were extended and, remarkably, suppression durations of that stimulus were abbreviated. Additional measurements revealed that this

Concerning mind.

The nature of the human mind is addressed from a neurological viewpoint. First from reported observations on subjects born blind with congenital cataract, who acquired vision after surgery, the concept of the primacy of somatosensation is developed. Therefrom some principles of the organization of higher sensory functions in the dominant parietal, occipital and temporal lobes are deduced. Sentience is traced to somatosensation. Some characteristics of the mind and of thinking are described. The non-existence of the unconscious is inferred. The question of free will is approached. Using neurolo

sEMG Signal Acquisition Strategy towards Hand FES Control.

Due to damage of the nervous system, patients experience impediments in their daily life: severe fatigue, tremor or impaired hand dexterity, hemiparesis, or hemiplegia. Surface electromyography (sEMG) signal analysis is used to identify motion; however, standardization of electrode placement and classification of sEMG patterns are major challenges. This paper describes a technique used to acquire sEMG signals for five hand motion patterns from six able-bodied subjects using an array of recording and stimulation electrodes placed on the forearm and its effects over functional electrical stimula

Combined multi-metric assessment of diaphragm contractile function in healthy humans: Feasibility, validity and reliability.

The combined use of subcostal ultrasonography and respiratory manometry represents a novel, integrative method for quantifying diaphragm contractile function (force, velocity and power). We evaluated the technical feasibility, construct validity and within-day test-retest reliability of this method during non-volitional, volitional and reflexive respiratory perturbations in healthy adults. Two independent cohorts were studied. In Experiment 1 (n = 10), diaphragm excursion (subcostal ultrasonography) and transdiaphragmatic pressure (Pdi, manometry) were measured during unilateral magnetic phren

Preoperative Bladder Capacity Predicts Social Continence following Bladder Neck Reconstruction in Children Born with Exstrophy-Epispadias Complex.

INTRODUCTION: The aim of the study is to review the continence and volitional voiding rate in a single center cohort of exstrophy-epispadias patients following Young-Dees-Leadbetter bladder neck reconstruction and to explore factors which predict continence. MATERIALS AND METHODS: Children who underwent Young-Dees-Leadbetter bladder neck reconstruction as a final stage of repair in a large single low-volume center in a small-population country between 1997 and 2019 were included. Demographic and clinical details were extracted from the patients' charts. The primary end point was continence an

[A pilot study of sexual problems in chronic schizophrenia: a report on 51 cases].

In order to explore the sexual problems of chronic schizophrenics, 51 cases were investigated. The result showed that 42 cases (82.3%) had various kinds of sexual problems. The problems were erectile impotence, frigidity, premature ejaculation, no ejaculation, vaginismus and pain on coitus. Only 4 cases had increased sexual desire. The authors found that the main sexual problems of chronic schizophrenics were inhibited sexual desire and excitement. The authors suggested that the sexual problems of chronic schizophrenics were related to their conditions in the body, rapport with their wives or

Basal forebrain infarction. A clinicopathologic correlation.

Following the repair of a ruptured anterior communicating artery aneurysm, a patient had a severe anterograde amnesia with sparing of other intellectual functions, apathy and loss of volition, altered arousal, and partial diabetes insipidus. Postmortem examination of the brain revealed bilateral destruction of the septal gray, nucleus accumbens, and nucleus of the diagonal band of Broca. Also involved in the lesion were inferior portions of the anterior limb of the internal capsule and globus pallidus. Discrete, microinfarcts were present in the paraventricular hypothalamic gray. Interlimb conditioning of lumbosacral spinally evoked motor responses after spinal cord injury.

OBJECTIVE: The importance of subcortical pathways to functional motor recovery after spinal cord injury (SCI) has been demonstrated in multiple animal models. The current study evaluated descending interlimb influence on lumbosacral motor excitability after chronic SCI in humans. METHODS: Ulnar nerve stimulation and transcutaneous electrical spinal stimulation were used in a condition-test paradigm to evaluate the presence of interlimb connections linking the cervical and lumbosacral spinal segments in non-injured (n=15) and spinal cord injured (SCI) (n=18) participants. RESULTS: Potentiatio
