Associations between Proprioceptive Neural Pathway Structural Connectivity and Balance in People with Multiple Sclerosis.

Mobility and balance impairments are a hallmark of multiple sclerosis (MS), affecting nearly half of patients at presentation and resulting in decreased activity and participation, falls, injuries, and reduced quality of life. A growing body of work suggests that balance impairments in people with mild MS are primarily the result of deficits in proprioception, the ability to determine body position in space in the absence of vision. A better understanding of the pathophysiology of balance disturbances in MS is needed to develop evidence-based rehabilitation approaches. The purpose of the curre

[Minor functional disorders after surgery of acoustic nerve neuroma].

UNLABELLED: Acoustic neuromas must be removed surgically, otherwise serious complications may occur. Removal of an acoustic neuroma most often leads to disorders of balance, hearing, and sometimes facial motor function. Many earlier studies have approached the hearing and facial motor function. However, the facial nerve also comprises sensory and secretory fibers which, when injured, may produce minor disorders affecting patient's quality of life. The purpose of this work was to evaluate the quality of life, secretory and taste disorders after acoustic neuroma surgery. PATIENTS AND METHOD: Am

Treatment of Vestibular Disorders (Inner Ear Balance Problems): How Does Your Physical Therapist Treat Dizziness Related to Inner Ear Balance Problems?

Dizziness is very common, but it is never normal. Dizziness can make performing daily activities, work, and walking difficult. Inner ear balance problems can make people dizzy when they turn their head, which can cause problems during walking and make people more likely to fall. Most of the time dizziness is not from a life-threatening disease. Often, dizziness is related to a problem of the vestibular (or inner ear balance) system. Vestibular disorders can be caused by infections in the ear, problems with the immune system, medications that harm the inner ear, and rarely from diabetes or stro

Heart transplantation on the first day of life from an anencephalic donor.

Heart transplantation on the first day of life, and graft harvesting from anencephalic donors, have been very rare events in the history of transplantation. At Bambino Gesù Hospital (Rome), heart transplantation was performed on a newborn 9 h after birth, using a graft harvested from an anencephalic donor. This graft achieved a good cardiocirculatory function, but the recipient died of necrotizing enterocolitis (NEC) on post-operative day (POD) 10. Despite failure, this case and other reports support the concept that hearts from anencephalic donors can work normally, and indicate that heart tr

Effects of effort-reward imbalance on emergency nurses' health: a mediating and moderating role of emotional exhaustion and work-family conflict.

BACKGROUND: Effort-reward imbalance (ERI) is a prevalent issue in the healthcare sector, particularly in the high-intensity, high-risk, and high-stress environment of the emergency department. This results in emergency department nurses bearing heavier workloads, responsibilities, and time commitments compared to their counterparts in other departments. ERI poses significant risks to their physical, psychological, and sleep quality. Therefore, it is essential to explore the mechanisms through which ERI influences the health of emergency department nurses. OBJECTIVE: The aim of this investigat

Clinical Movement Toward Integration With Obsessive Personality Disorder: The Fear of Losing Love and the Need for Control.

Case material is used to illustrate a Kleinian-informed psychoanalytic approach to working with a patient captured by obsessive cycles of controlling others along with the threat of losing their love. Obsessive disorders involve either controlling the self and the other or feeling confined and controlled by the self and the other. These phantasy states bring on intense fear of difference, fear of the unknown, and fear of abandonment. During treatment, as the patient improves, there is often a fluctuation between more disturbed and more balanced mental experiences of life. The Kleinian-informed

Environmental, Health, and Psychological Factors Predict Alcohol Sipping in Childhood: A Machine Learning Analysis of the ABCD Study.

OBJECTIVE: Extant research has focused on the risk factors for alcohol use in adolescence, but little work has examined these in childhood. Early alcohol sipping, defined here as sipping alcohol by ages 9 to 10, may be a unique and informative developmental precursor to future problematic alcohol use. This study employed machine learning to rank risk factors linked to early alcohol sipping by importance. METHOD: Data from a subset of the Adolescent Brain Cognitive Development℠ (ABCD) Study sample were used (n = 10,707 children ages 9-10 years old; 48.1% girls). Two state-of-the-art machine le

"Are You Okay, Honey?": Recognizing Emotions Among Couples Managing Diabetes in Daily Life Using Multimodal Real-World Smartwatch Data.

Couples generally manage chronic diseases together and the management takes an emotional toll on both patients and their romantic partners. Consequently, recognizing the emotions of each partner in daily life could provide insight into their emotional well-being in chronic disease management. Currently, the process of assessing each partner's emotions is manual, time-intensive, and costly. Despite the existence of works on emotion recognition among couples, none of these works have used data collected from couples' interactions in daily life. In this work, we collected 85 h (1021 5-min samples

Comprehensive Physiotherapy Rehabilitation in a Patient With Cerebellar Ataxia and Dysphagia: A Case Report Investigating Symptomatology, Management, and Outcomes.

A cerebellar infarct occurs when blood flow to the cerebellum, located in the posterior cranial fossa, is disrupted. This diminished blood supply leads to decreased oxygen delivery, resulting in motor and balance control impairments. One prevalent sign of neurodegenerative diseases is dysphagia, which is typically linked to a higher death rate. No systematic and uniform assessment of dysphagia is used in the clinical care environment of individuals with ataxia. Its effect on the quality of life associated with health in patients is little understood. Therefore, this case report works to addres

A Review of Movement and Functional Impairments in Fibromyalgia: Implications for Targeted Treatment.

Fibromyalgia (FM) is a complex and disabling chronic pain condition characterized by widespread musculoskeletal pain, persistent fatigue, cognitive dysfunction, and sleep disturbances. It affects approximately 1-10% of the general population, with a strong female predominance. Beyond pain, patients often experience hypersensitivity to stimuli such as light, touch, and temperature - a phenomenon attributed to central sensitization. Despite its high prevalence, FM remains poorly understood. The condition is believed to arise from abnormal central sensory processing in interaction with peripheral

Vestibular rehabilitation in a patient with whiplash-associated disorders.

Whiplash-associated disorders are characterized by multiple physical complaints after a flexion-extension trauma to the neck. They are difficult to treat, and they often result in great impact on the patient's quality of life. In this paper, the comprehensive treatment of a patient with whiplash-associated disorders is presented. The purpose is to highlight the importance of accurate diagnosis and appropriate treatment plans to improve patients' quality of life. This 23-year-old woman experienced a traffic accident which caused severely painful neck disability, numbness over bilateral upper li

Legislation for advancing women's leadership in the health sector in India and Kenya: a 'law cube' approach to identify ways to strengthen legal environments for gender equality.

OBJECTIVES: This paper examines the availability of legal provisions, or the lack thereof, that support women to progress equitably into leadership positions within the health workforce in India and Kenya. METHODS: We adapted the World Bank's Women, Business and Law framework of legal domains relevant to gender equality in the workplace and applied a 'law cube' to analyse the comprehensiveness, accountability and equity and human rights considerations of 27 relevant statutes in India and 11 in Kenya that apply to people in formal employment within the health sector. We assessed those laws aga

Impacts of hearing history, etiology, vestibular and balance function, and socioeconomic marginalization on developmental outcomes in children with cochlear implants.

Children with bilateral cochlear implants (BCIs) remain vulnerable to developmental challenges relative to typically developing peers (TD), even when access to sound through acoustic or amplified hearing is achieved within the first year of life. This study evaluated contributions of hearing history, etiology, vestibular/balance function, and socioeconomic marginalization to language, working memory, and academic outcomes. Ninety-six children aged 4.65-17.85 years participated: 66 with BCIs (mean [SD] age, 11.54 [3.57]) and 30 typically developing (TD) peers (mean [SD], 11.69 [2.68]). Standard

Anosognosia, neglect and quality of life of right hemisphere stroke survivors.

BACKGROUND AND PURPOSE: Anosognosia and neglect may coexist in stroke patients. Neglect patients often report poor quality of life (QOL), whereas patients suffering from other cognition disorders with poor insight report better QOL. This study investigates the relationship between anosognosia, neglect and QOL amongst stroke survivors. METHODS: Stroke survivors who met the criteria were used as a sampling pool. Sixty stroke patients were observed in this study, amongst whom 20 patients with anosognosia and neglect (A+N+), 20 patients with neglect but not anosognosia (A-N+) and 20 patients with
