Do patients receiving workers' compensation who undergo arthroscopic rotator cuff repair have worse outcomes than non-recipients? Retrospective case-control study.

PURPOSE: We compared preoperative and postoperative measures among workers' compensation board (WCB) recipients and non-recipients and determined the impact of WCB receipt on the 1- and 2-year outcomes of rotator cuff repair. METHODS: We retrospectively reviewed patients with full-thickness rotator cuff tears who underwent arthroscopic repair between September 2011 and September 2014. Patients were divided into two groups based on WCB status: WCB recipients and non-recipients. All patients returned for follow-up functional evaluations at 1 and 2 years after the operation. Four outcome measure

The Relationship Between Work-Disability Duration and Claimant's Expected Time to Return to Work as Recorded by Workers' Compensation Claims Managers.

Purpose This research sought to determine whether there is a relationship between claimants' expected time to return to work (RTW) as recorded by claims managers and compensated days of work disability. Methods We utilized workers' compensation data from a large, United States-based insurance company. RTW expectations were collected within 30 days of the claim being reported and these were compared with the termination of total temporary indemnity payments. Bivariate and hierarchical regression analyses were conducted. Results A significant relationship between expected time to RTW and compens

Effects of the 2016 CDC opioid prescription guidelines on opioid use and worker compensation case length in patients with back pain.

BACKGROUND: Narcotic consumption in the workers' compensation population contributes to prolonged case duration, worse clinical outcomes, and opioid dependence. In 2016, the CDC provided recommendations guiding clinicians on prescribing opioids to adult patients with chronic pain. The objective of our study was to evaluate a cause-and-effect relationship between narcotic consumption and worker compensation claim length before and following guideline revision. METHODS: An administration database was retrospectively queried to identify patients evaluated for spine-related workers' compensation

Arthroscopic lateral release for anterior knee pain: a study comparing patients who are claiming worker's compensation with those who are not.

The purpose of this retrospective study was to assess the outcome of arthroscopic lateral retinacular release comparing patients who are claiming worker's compensation with those who are not. Twenty-three consecutive patients with anterior knee pain caused by injuries on the job were matched with an equal number of patients with other kinds of injuries on the basis of age, sex, and medical history. A numeric scoring scale, physical examination, isokinetic strength analysis, performance in a one-leg hop test, and radiography of the knee formed the data base. Evaluation of the outcome was based

Does Symptom Duration Prior to Anterior Cervical Discectomy and Fusion for Disc Herniation Influence Patient-Reported Outcomes in a Workers' Compensation Population?

OBJECTIVE: To evaluate the influence of symptom duration before anterior cervical discectomy and fusion (ACDF) on patient-reported outcomes (PROs) in workers' compensation patients. METHODS: A prospective registry was searched for workers' compensation patients who underwent ACDF for herniated disc. Two cohorts based on symptom duration were formed: lesser duration (LD) (<6 months) and prolonged duration (PD) (≥6 months). PROs were collected preoperatively and at 6 weeks, 12 weeks, 6 months, and 1 year postoperatively. PROs were compared within and between groups. Rates of minimum clinically

Symptomatic cervical disc herniation following a motor vehicle collision: return to work comparative study of workers' compensation versus personal injury insurance status.

BACKGROUND CONTEXT: Patients with approved workers' compensation injuries receive guaranteed compensation for the duration of their injury, whereas patients with personal injury claims are only compensated, if at all, at the time of a successful settlement or trial verdict at a time point distant from their injury. PURPOSE: This study compares the financial impact and loss of work patterns due to a workers' compensation (WC) claim or personal injury in patients with a symptomatic cervical disc herniation resulting from a motor vehicle collision. STUDY DESIGN: A prospective study of patients

Costs and cost-effectiveness of spinal cord stimulation (SCS) for failed back surgery syndrome: an observational study in a workers' compensation population.

STUDY DESIGN: Prospective cohort study. OBJECTIVE: We estimated the cost-effectiveness of spinal cord stimulation (SCS) among workers' compensation recipients with failed back surgery syndrome (FBSS). SUMMARY OF BACKGROUND DATA: Randomized controlled trial (RCT) evidence suggests that SCS is more effective at 6 months than medical management for patients with FBSS. However, procedure costs are high and workers' compensation claimants often have worse outcomes than other patients. METHODS: We enrolled 158 FBSS patients receiving workers' compensation into three treatment groups: trial SCS wi

Changes in Pregabalin Dispensing to Australians with Workers' Compensation Claims for Low Back Pain Following the Listing of Pregabalin on the Pharmaceutical Benefits Scheme.

OBJECTIVES: We sought to identify whether the inclusion of pregabalin on the list of medicines subsidised by the Australian government in 2013 resulted in changes to the dispensing of pregabalin to Australians with workers' compensation claims for low back pain. METHODS: Using a sample of workers' compensation claims and payments data (2010-2018), we measured the prevalence of pregabalin dispensing and time to first pregabalin dispensing in workers whose claim began before, during or after pregabalin was listed on the Pharmaceutical Benefits Scheme (PBS) with binary logistic and Cox proportio

Single-level lumbar fusion for degenerative disc disease is associated with worse outcomes compared with fusion for spondylolisthesis in a workers' compensation setting.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: Compare lumbar fusion outcomes, return-to-work (RTW) status in particular, between workers' compensation (WC) subjects undergoing single-level posterolateral fusion for either spondylolisthesis or degenerative disc disease (DDD). SUMMARY OF BACKGROUND DATA: Lumbar fusion for spondylolisthesis tends to yield more consistent outcomes than fusion for DDD and discogenic low back pain. Within the clinically distinct WC population, relatively few studies exist that evaluate lumbar fusion outcomes. METHODS: A total of 889 Ohio WC subjects were i

Medical Care Provided Under California's Workers' Compensation Program: Effects of the Reforms and Additional Opportunities to Improve the Quality and Efficiency of Care.

Since 2004, significant changes have been made to the California workers' compensation (WC) system. The Commission on Health and Safety and Workers' Compensation (CHSWC) asked the RAND Corporation to examine the impact that these changes have on the medical care provided to injured workers. This study synthesizes findings from interviews and available information regarding the implementation of the changes affecting WC medical care and identifies areas in which additional changes might increase the quality and efficiency of care delivered under the WC system. To improve incentives for efficien

Increased mortality risk for workers with a compensated, permanent occupational disability of the upper or lower extremities: a 21-year follow-up study.

This 1986-2006 study sought to determine whether specific causes led to increased mortality risks for Taiwanese workers with an approved compensation claim for permanent occupational disability (ACCPOD) of the upper or lower extremities. All cases of ACCPOD between 1986 and 2006 were collected from the database of compensation claims at the Bureau of Labor Insurance. Standardized mortality ratios and 95% confidence intervals were calculated for different causes of death among workers with an ACCPOD of the upper or lower extremities. A total of 800,047 person-years were accrued for 71,001 worke

Early prognostic factors for duration on temporary total benefits in the first year among workers with compensated occupational soft tissue injuries.

AIMS: To develop a model of prognosis for time receiving workers' compensation wage replacement benefits in the first year. METHODS: A prospective cohort of 907 injured workers off work because of soft tissue injuries was followed for one year through structured telephone interviews and administrative data sources. Workers were recruited at workers' compensation claim registration. Only those still off work at four weeks post-registration were included in the analysis. Data from several domains (demographics, clinical factors, workplace factors, recovery expectations) were collected at approx

How do coverage policies influence practice patterns, safety, and cost of initial lumbar fusion surgery? A population-based comparison of workers' compensation systems.

BACKGROUND CONTEXT: In response to increasing use of lumbar fusion for improving back pain, despite unclear efficacy, particularly among injured workers, some insurers have developed limited coverage policies. Washington State's workers' compensation (WC) program requires imaging confirmation of instability and limits initial fusions to a single level. In contrast, California requires coverage if a second opinion supports surgery, allows initial multilevel fusion, and provides additional reimbursement for surgical implants. There are no studies that compare population-level effects of these po

[Recording Trauma Care in German Hospital Discharge Data: Services Provided by Hospitals Owned by Workers' Compensation Funds and Financed through Statutory Accidental Insurance].

BACKGROUND: In Germany, the Diagnosis-Related Group Statistics (DRG Statistics) represent an almost complete discharge data-based registry of inpatient services in acute care hospitals. However, services of hospitals owned by workers' compensation funds and financed through the statutory insurance for occupational accidents are excluded from the obligation of submitting hospital discharge data. Hence, the DRG statistics might be incomplete regarding inpatient services for trauma care. METHODS: In order to illustrate trauma and post-trauma care in acute care hospitals, groups of specific inpat

Editorial Commentary: Preoperative Exercise, Workers' Compensation, Diabetes, Lateral Extra-Articular Tenodesis, and Return to Sport Determine Patient Satisfaction After Anterior Cruciate Ligament Reconstruction.

Patient-reported outcome measures (PROMs) have been developed and used as the primary determinant of successful patient-centered results. The patient acceptable symptomatic state delineates an absolute value for PROMs indicating that patients are satisfied with their outcome. When this metric is used for anterior cruciate ligament reconstruction, patients reach a satisfactory outcome at between 6 and 8 months postoperatively, and more than 90% reach a satisfactory outcome at 12 months. Preoperative variables such as preoperative exercise, Workers' Compensation, and diabetes impact patient outc
