Revision rate and long-term outcome after partial wrist denervation in wrist osteoarthritis.

INTRODUCTION: This prospective, one-group, interventional study assessed 5 year survival and outcomes after partial wrist denervation for painful wrist osteoarthritis. The primary outcome was the proportion of patients who did not require further surgery (survival rate). Secondary outcomes included patient-reported measures, objective function and radiographic progression. METHODS: Sixty consecutive patients who underwent neurectomy of the posterior and anterior interosseous nerves between 2018 and 2020 were included. Survival was analysed using Kaplan-Meier and Cox regression. Patient-report

Single-Trial Electroencephalography Discrimination of Real, Regulated, Isometric Wrist Extension and Wrist Flexion.

Improved interpretation of electroencephalography (EEG) associated with the neural control of essential hand movements, including wrist extension (WE) and wrist flexion (WF), could improve the performance of brain-computer interfaces (BCIs). These BCIs could control a prosthetic or orthotic hand to enable motor-impaired individuals to regain the performance of activities of daily living. This study investigated the interpretation of neural signal patterns associated with kinematic differences between real, regulated, isometric WE and WF movements from recorded EEG data. We used 128-channel EEG

Outcome of carpal tunnel release--correlation with wrist and wrist-palm anthropomorphic measurements.

Wrist and wrist-palm measurements have been associated with the diagnosis of carpal tunnel syndrome. We found no reported study about how this correlation affects the outcome after surgery. We investigated the role of the measurements in predicting outcome after open carpal tunnel release. A total of 131 patients (88 female, 43 male) responded to our postal questionnaire using the Boston Carpal Tunnel assessment (65% response rate) at a minimum of 9 months post-operatively. Symptom and functional scores showed a strong correlation. There was no statistical difference in the outcome between wri

Abnormal Wrist Sagittal Kinematics in Gymnasts With Dorsal Wrist Pain: A New Syndrome.

BACKGROUND: The pathology of dorsal wrist pain in gymnasts without abnormal radiographic findings remains unclear. PURPOSE/HYPOTHESIS: The purpose of this study was to identify abnormal wrist sagittal kinematics in gymnasts with dorsal wrist pain. It was hypothesized that gymnasts with dorsal wrist pain would show abnormal sagittal kinematics with reversible hypermobility of the intercarpal joint. STUDY DESIGN: Controlled laboratory study. METHODS: Participants included 19 wrists in male gymnasts with dorsal wrist pain, 18 wrist in male gymnasts without wrist pain, and 20 adult men without

Total Wrist Arthroplasty for Posttraumatic Wrist Osteoarthritis: A Cohort Study Comparing Three Indications.

Scapholunate ligament ruptures and scaphoid nonunion with consecutive advanced collapse (SLAC and SNAC wrists) as well as intra-articular distal radius fractures (DRF) are prone to cause posttraumatic wrist osteoarthritis. The aim of this study was to compare the outcomes of these indications for total wrist arthroplasty. We included 13, 11, and 8 patients with an overall mean age of 60 ± 9 years in the SLAC, SNAC, and DRF cohort, respectively. After an average follow-up period of 6 ± 3 years, we found no difference between our groups regarding pain levels and functional scores, although these

The effects of wrist position and radioulnar wrist compression on median nerve longitudinal mobility.

BACKGROUND: Carpal tunnel syndrome is an entrapment neuropathy at the wrist characterized by compromised median nerve mobility. The purpose of this study was to investigate the effect of wrist position on median nerve longitudinal mobility in healthy subjects and the effect of radioulnar wrist compression on the median nerve mobility under non-neutral wrist positions. METHODS: Dynamic B mode ultrasound images captured longitudinal median nerve motion in the carpal tunnel in 10 healthy subjects at wrist neutral position, 30-degree flexion, and 30-degree extension. In each position, RWC of 0, 5

Wrist shortening arthrodesis with volar plate in a dorsal position for spastic wrist contracture.

Total wrist arthrodesis in severe wrist flexion deformities (greater than 60°) due to spasticity represents a valid therapeutic option. It aims to improve the hand's appearance, hygiene, function and to prevent the deformity from getting worse. The objective of this study is to evaluate the clinical and anatomical results of wrist shortening arthrodesis using a classic volar plate in the dorsal position in functional surgery for central spastic hands. We conducted a single-center analysis of a series of patients who underwent this shortening arthrodesis. The review at a minimum 1-year follow-u

Wrist Pain in Gymnasts: A Review of Common Overuse Wrist Pathology in the Gymnastics Athlete.

Injury rates among gymnasts are among the highest of any sport at the high school and collegiate level per athletic exposure. The wrist has increased injury risk due to repetitive physical stresses predisposing it to acute injury, overuse, and degenerative damage. This article will review the most common overuse wrist injuries seen in gymnasts. Prompt evaluation and management is necessary to avoid the negative sequelae that can often accompany these injuries. Little is known about effective sport-specific injury prevention strategies, but general guidelines for overuse injury prevention inclu

Motor control impairment of the contralateral wrist in patients with unilateral chronic wrist pain.

OBJECTIVE: Assessment of the quality of fine motor control in patients with unilateral chronic wrist pain seldom focuses on the possibility that control of movements is effector independent at the cerebral level. This mechanism may be involved in an impairment of motor function in the unaffected wrist. We studied the possible motor impairment in the unaffected wrist in patients with chronic wrist pain. DESIGN: Eighteen patients with chronic wrist pain in their dominant hand and 20 healthy controls performed, using their nondominant hand, back-and-forth, left-to-right stroke patterns with a pe

The spaghetti wrist. Simultaneous laceration of the median and ulnar nerves with flexor tendons at the wrist.

The outcome of 15 patients who sustained simultaneous laceration of the median and ulnar nerves with flexor tendons at the wrist is described. Primary nerve repair yielded satisfactory results, the median nerve achieving a better outcome than the ulnar nerve. Most patients regained a functional range of wrist movement. Flexor tendons yielded the poorest results. A functional, albeit impaired outcome can usually be anticipated following this severe injury.

Effect of Total Wrist Arthroplasty Alignment on Wrist Range of Motion and Component Impingement.

PURPOSE: The purpose of this study was to evaluate whether total wrist arthroplasty (TWA) component alignment affects wrist range of motion (ROM), component impingement, and tendon forces. METHODS: Six fresh frozen cadaver wrists were moved through five wrist motions before and after implantation of the KinematX TWA using a "refined alignment" strategy: the radial component was aligned parallel to the longitudinal axis of the forearm in the coronal plane, and the carpal component colinear with the ulnar cortex of the long finger metacarpal shaft. Comparisons were made to a prior study that te

Relation between the dash score and radiographic evaluation of the wrist in patients with wrist fracture.

Traditionally, the assessment of distal radius fracture outcomes has been based on radiological measurements and self-evaluation scores. However, there is uncertainty regarding how accurately these measurements reflect the patient's perception of their outcome. In this study, we examined the correlation between radiological measurements and patient-perceived outcomes using the Disabilities of the Arm, Shoulder, and Hand outcome (DASH) score. 140 individuals who had recovered from a distal radius fracture. and had been treated with DVR, Kapandji, percutaneous pinning or closed reduction were in

Management of Patients With Post-Traumatic Wrist Flexor Contracture by Carpectomy and Wrist Fusion.

Post-traumatic wrist flexor contracture is most commonly seen in major trauma affecting the hand, wrist, and forearm. It produces debilitating complications affecting the quality of life, often requiring multiple staged surgeries, and prolonged rehabilitation with physiotherapy to yield functional improvement. Wrist flexion contracture correction is the first surgery performed to reduce the deformity and improve the functional status of the hand. Releasing the wrist flexion contracture due to stretch on the contracted structures could cause a vascular compromise of the hand and skin deficit, w
