Effect of the herbal medicine Dai-kenchu-to for serum ammonia in hepatectomized patients.

BACKGROUND/AIMS: Prolonged paralytic ileus occurring in hepatectomized patients may induce hyperammonemia or bacterial translocation, which injures the remnant liver function and sometimes causes post-resection liver failure. We examined the effectiveness of the herbal medicine, Dai-kenchu-to (DKT), on postoperative serum ammonia levels in patients with liver resection and compared it with lactulose. METHODOLOGY: Patients with liver resection were divided into three groups. Lactulose group (n=31), 16g of lactulose was administered orally three times a day from the first postoperative day. DKT

Marked effect of topical application of Chinese medicine combined with moxibustion in a case of refractory malignant ascites in diffuse liver cancer.

BACKGROUND: Although diffuse hepatocellular carcinoma (HCC) is rarely encountered in clinical practice, it frequently leads to the refractory complication of malignant ascites. Conventional treatments such as diuretics, therapeutic paracentesis, and intraperitoneal chemotherapy often yield suboptimal outcomes. CASE PRESENTATION: We report a detailed case of a 55-year-old female patient with diffuse HCC (CNLC stage IV, Child-Pugh C), portal vein tumor thrombus, and malignant ascites refractory to diuretics. Following confirmation of the diagnosis, she was treated with a novel approach: topical

In vitro phytochemical, antioxidant activity and antimycobacterial potentials of selected medicinal plants commonly used for respiratory infections and related symptoms in the Limpopo Province, South Africa.

BACKGROUND: The emergence of drug resistance among Mycobacterium tuberculosis (Mtb) strains, coupled with the detrimental side effects linked to tuberculosis (TB) treatment, underscores the persistence of TB as a significant clinical and public health concern in South Africa, thereby necessitating ongoing research in drug discovery. The use of medicinal plants for the treatment of TB has garnered increasing attention, especially in countries where a significant portion of the population relies on traditional medicine as a primary form of healthcare. METHODS: The crude extracts from nine medic

In vitro and in vivo antiplasmodial activity of three Rwandan medicinal plants and identification of their active compounds.

In our previous study, we reported the interesting in vitro antiplasmodial activity of some Rwandan plant extracts. This gave rise to the need for these extracts to also be evaluated in vivo and to identify the compounds responsible for their antiplasmodial activity. The aim of our study was, on the one hand, to evaluate the antiplasmodial activity in vivo and the safety of the selected Rwandan medicinal plants used in the treatment of malaria, with the objective of promoting the development of improved traditional medicines and, on the other hand, to identify the active ingredients in the pla

A tingling sanshool derivative excites primary sensory neurons and elicits nocifensive behavior in rats.

Szechuan peppers contain hydroxy-α-sanshool that imparts desirable tingling, cooling, and numbing sensations. Hydroxy-α-sanshool activates a subset of sensory dorsal root ganglion (DRG) neurons by inhibiting two-pore potassium channels. We presently investigated if a tingle-evoking sanshool analog, isobutylalkenyl amide (IBA), excites rat DRG neurons and, if so, if these neurons are also activated by agonists of TRPM8, TRPA1, and/or TRPV1. Thirty-four percent of DRG neurons tested responded to IBA, with 29% of them also responding to menthol, 29% to cinnamic aldehyde, 66% to capsaicin, and sub

The herbal medicine Dai-kenchu-to and one of its active components [6]-shogaol increase intestinal blood flow in rats.

The present study investigated the effects of the herbal medicine Dai-kenchu-to (DKCT) and its 4 individual ingredients on intestinal blood flow (IBF) in rats by laser Doppler flowmetry. Intraduodenal administration of DKCT (30, 100 and 300 mg/kg) increased IBF in a dose-dependent manner, whereas the mean arterial blood pressure was not affected. One of the ingredients in DKCT is dried ginger rhizome (150 mg/kg), whose main component is [6]-shogaol (2 mg/kg), both of which showed similar effects to those shown by DKCT, while the other ingredients in DKCT only slightly increased IBF or had no e

Possible involvement of the transient receptor potential vanilloid type 1 channel in postoperative adhesive obstruction and its prevention by a kampo (traditional Japanese) medicine, daikenchuto.

This study focused on the localization of transient receptor potential vanilloid type 1 (TRPV1) in the intestines in postoperative adhesion model rats and investigated the underlying mechanism for the anti-adhesion action of daikenchuto (DKT), especially in relation to TRPV1. Postoperative intestinal adhesion was induced by sprinkling talc in the small intestine. The expression of TRPV1 mRNA was examined by in situ hybridization and real-time RT-PCR. The effects of DKT and its major ingredient, hydroxy sanshool, with or without ruthenium red, a TRP-channel antagonist, on talc-induced intestina

Risk assessment of daikenchuto-induced hepatobiliary injury in colon cancer patients post-colectomy: a retrospective cohort study.

BACKGROUND: Colectomy for colon cancer typically results in gastrointestinal hypomotility. Daikenchuto, a herbal medicine traditionally used in Japan, is administered to alleviate gastrointestinal hypomotility. However, it is suspected to cause hepatobiliary injury. Therefore, in this comparative, retrospective cohort study, we aimed to assess the risk of daikenchuto administration-induced hepatobiliary injury post-colectomy. METHODS: Patients with colon cancer who underwent colectomy, excluding the population with a high risk of postoperative hepatobiliary injury, were included in this study

Safety and efficacy of a class II medical device based on highly purified and standardized plant extracts in the management of post-menopausal patients with vulvar and vaginal atrophy: a single-center prospective observational study.

BACKGROUND: Despite the gold standard treatment for genitourinary syndrome of menopause (GSM) is based on the use of local or systemic estrogen-containing products, the typical long-term side effects of hormonal treatments and, most importantly, the contraindications in patients with history of breast and endometrial neoplasms do limit in some extent its use. As hyaluronic acid and some highly purified botanicals have clearly demonstrated their anti-inflammatory and mucosa-protecting properties, we have tested, in women with GSM, a class II vaginal medical device containing hyaluronate gel and

[Herbalogical study on Liangmianzhen (Zanthoxyli Radix)].

Liangmianzhen(Zanthoxyli Radix) has long been used as medicine. The current medicinal parts are different from those in the ancient. As recorded in the Chinese Pharmacopeia, the medicinal part is root. However, in ancient works, the medicinal parts include root, stem, leaf, and fruit. In an attempt to find the historical basis that stem is a reasonable medicinal part, the herbalogical study was carried out on this medicinal based on the formal names, synonyms, original plant, medicinal parts, habitat of the medicinal plant, producing area, processing and preparation methods, efficacy, and indi
