Comparison of Short and Mid-Term Mortality and Morbidity in Patients With Severe Aortic Stenosis (Intermediate/High Risk) Who Underwent Transcatheter Aortic Valve Replacement and Surgical Aortic Valve Replacement.

Treatment protocols for severe aortic valve stenosis include surgical aortic valve replacement (SAVR), balloon valvuloplasty, transcatheter aortic valve replacement (TAVR), and medical  treatment. Because the success rates are getting higher with both SAVR and TAVR, making the right treatment decision is important. This study retrospectively shows the short- (1 month) and mid-term (6 months) mortality and morbidity rate differences between 2 groups of patients, who arrived to our hospital from January 2014 through October 2018. The first group consists of 54 patients who underwent mid-high ris

The impact of the development of transcatheter aortic valve implantation on the management of severe aortic stenosis in high-risk patients: treatment strategies and outcome.

OBJECTIVES: Transcatheter aortic valve implantation (TAVI) has reoriented the treatment of aortic stenosis (AS) for high-risk patients. Little is known about late outcome after TAVI, surgical aortic valve replacement (AVR) or medical treatment in a single centre. We report patients' characteristics, early and 6-year survival rates after the three therapeutic strategies, and the evolution over time. We also analysed predictive factors of mortality after TAVI or surgical AVR. METHODS: Between October 2006 and December 2010, 478 high-risk consecutive patients were referred for severe symptomatic

Surgical strategy for the bicuspid aortic valve: tricuspidization with cusp extension versus pulmonary autograft.

OBJECTIVE: The congenitally bicuspid aortic valve is the most common etiologic factor associated with clinically significant aortic stenosis and/or regurgitation in pediatric patients. Beyond infancy, surgical intervention typically involves valve repair with cusp thinning and commissurotomy or valve replacement, primarily with pulmonary autograft in the current era. An aortic valve repair technique using tricuspidization with cusp extension was introduced in 1999. This study compares the midterm clinical outcome in patients undergoing valve repair by tricuspidization with cusp extension with

Balloon Versus Self-Expandable TAVI in Patients With Left Ventricular Dysfunction: A Real-World Comparative Study.

BACKGROUND: In severe aortic stenosis with reduced left ventricular ejection fraction (LVEF), transcatheter aortic valve implantation (TAVI) improves systolic function. Self-expanding valves (SEV) show haemodynamic advantages over balloon-expandable valves (BEV), but whether this yields greater recovery of left ventricular function remains uncertain. OBJECTIVES: To compare change in ejection fraction between BEV and SEV in adults with LVEF < 40% undergoing TAVI, and to assess haemodynamics and clinical outcomes at 30 days and 12 months. METHODS: Observational cohort using a national registry

3-Year Outcomes of Mitral Valve-in-Valve Therapy Using Balloon-Expandable Transcatheter Valves in the United States.

BACKGROUND: Mitral valve-in-valve (MViV) is a safe and effective therapy for severe bioprosthetic mitral degeneration; however, longer-term outcomes are not well defined. OBJECTIVES: This study aimed to evaluate 3-year outcomes following MViV. METHODS: Outcomes of all-cause mortality, stroke, and reintervention were collected in patients undergoing transseptal MViV with the SAPIEN 3 valve family for failed surgical bioprostheses from June 2015 to March 2024 in the TVT (Transcatheter Valve Therapy) Registry, and Centers for Medicare and Medicaid Services data linkage was performed. Importance of Preoperative Computed Tomography Assessment of the Membranous Septal Anatomy in Patients Undergoing Transcatheter Aortic Valve Replacement With a Balloon-Expandable Valve.

BACKGROUND: Cardiac conduction disturbance (CD) is the most frequent complication following transcatheter aortic valve replacement (TAVR). This study examined whether the anatomy of the membranous septum (MS) could provide useful information about the risk of CD following TAVR with a balloon-expandable valve (BEV). METHODS AND RESULTS: Among 132 consecutive patients, 106 (mean age, 85.6±5.1 years; 75 females) were included in the study. Using preoperative CT and angiography, MS length and implantation depth (ID) were assessed. The MS length minus the prosthesis ID was calculated (∆MSID). Corr

Postprocedural aortic regurgitation in balloon-expandable and self-expandable transcatheter aortic valve replacement procedures: analysis of predictors and impact on long-term mortality: insights from the FRANCE2 Registry.

BACKGROUND: Significant postprocedural aortic regurgitation (AR) is observed in 10% to 20% of cases after transcatheter aortic valve replacement (TAVR). The prognostic value and the predictors of such a complication in balloon-expandable (BE) and self-expandable (SE) TAVR remain unclear. METHODS AND RESULTS: TAVR was performed in 3195 consecutive patients at 34 hospitals. Postprocedural transthoracic echocardiography was performed in 2769 (92%) patients of the eligible population, and these patients constituted the study group. Median follow-up was 306 days (Q1-Q3=178-490). BE and SE devices

A novel generation balloon-expandable versus supra-annular self-expanding trans-catheter heart valve in patients with severe aortic stenosis and calcified left ventricle outflow tract.

INTRODUCTION: Severe left ventricular outflow tract (LVOT) calcification remains a challenging anatomy for transcatheter aortic valve replacement (TAVR), with increased risk of paravalvular leak (PVL), annular injury, and conduction disturbances. Evidence on the performance of new-generation balloon-expandable (BE) valves in this setting is limited. METHODS: An international, multicenter, cohort analysis comparing outcomes of patients with severe LVOT calcification treated with the novel Octacor BE versus two self-expanding (SE) transcatheter heart valves (THVs: Acurate Neo2 and Evolut Pro/Pr

Alternative access for balloon-expandable transcatheter aortic valve replacement: comparison of the transaortic approach using right anterior thoracotomy to partial J-sternotomy.

OBJECTIVES: For transcatheter aortic valve replacement (TAVR), transaortic (TAo) and transapical (TA) approaches are major alternatives in cases unsuitable for the transfemoral approach. Partial J-sternotomy is a widely used access for TAo. However, redo sternotomy or right-sided aorta may preclude this access, and right anterior thoracotomy is potentially beneficial in these cases. This study sought to evaluate the TAo approach using thoracotomy (T-TAo) and compare it to the TAo approach using a sternotomy (S-TAo) and a TA approach. METHODS: In a large single-center series, consecutive TAVR
