[Subcutaneous tenotomy of Achille's tendon in adults for ankle stiffness. A review of 80 cases].

PURPOSE OF THE STUDY: Lengthening of Achille's tendon is part of surgical program for ankle stiffness in equinus deformity. Usually this lengthening is done by opened surgery with all the well-knowned advantages in term of adjustment. We used a percutaneous method for Achille lengthening with a two stages tenotomy. The purpose of this study is to evaluate advantages, inconvenients and efficiency of the subcutaneous method versus the open method. MATERIALS AND METHODS: We present a retrospective study of a 80 percutaneous lengthening of Achille's tendon in 78 patients done between August 85 an

Retrograde flow in the deep veins of subjects with normal venous function.

BACKGROUND: Duplex ultrasonography has become the optimal non-invasive method of assessing the deep venous system of the leg. The stimuli used to promote reverse flow, subject positioning, and the veins scanned have varied between studies. Reverse flow of less than 0.5 s is accepted as 'normal', however, all published studies have demonstrated 'normal' subjects with deep vein reverse flow exceeding this time. The aim of this study was to establish the duration of reverse flow in all deep vein segments, and to determine the optimal method of eliciting reverse flow in individuals with proven nor

[Dolichoectatic intracranial arteries. Advances in images and therapeutics].

Dolichoectasia of intracranial arteries is an infrequent disease with an incidence less than 0.05% in general population. It represents 7% of all intracranial aneurysms. Commonly seen in middle age patients with severe atherosclerosis and hypertension, the affected arteries include the basilar artery, supraclinoid segment of the internal carotid artery, middle, anterior and posterior cerebral arteries; males are more frequently affected. The clinical features of these fusiform aneurysms are divided in three categories: ische-mic, cranial nerve compression and signs from mass effect. Hemorrhage

Intracranial aneurysms.

The modern history of the management of intracranial aneurysm encompasses little more than a quarter century. These are seen in 5% routine autopsies and those under 2 mm exist in 17% normal adult brain vasculature. One third of strokes are SAH and aneurysms account for 70% of these. Aneurysms rupture at a rate of 12/100.000 population/year, mostly on the 5th, and 6th decades. Given the high morbidity and mortality of these (43% from first haemorrhage if untreated), surgeons can attempt to avoid rebleeding for a week or more until surgery becomes safe and expand the ability to deal safely with

Factors predicting toxicity and response following isolated limb infusion for melanoma: An international multi-centre study.

INTRODUCTION: Isolated limb infusion (ILI) is a minimally-invasive procedure for delivering high-dose regional chemotherapy to treat melanoma in-transit metastases confined to a limb. The aim of this international multi-centre study was to identify predictive factors for toxicity and response. METHODS: Data of 687 patients who underwent a first ILI for melanoma in-transit metastases confined to the limb between 1992 and 2018 were collected at five Australian and four US tertiary referral centres. RESULTS: After ILI, predictive factors for increased limb toxicity (Wieberdink grade III/IV limb

Pharmacokinetics of Zilurgisertib With and Without Food from Single and Multiple Ascending Dose Phase 1 Studies in Healthy Adults.

BACKGROUND AND OBJECTIVES: The oral, potent, and highly selective activin receptor-like kinase 2 (ALK2) inhibitor zilurgisertib (INCB000928) is in development as a treatment for fibrodysplasia ossificans progressiva (FOP), and for anemia due to myelofibrosis, myelodysplastic syndromes, and multiple myeloma. Saliva is an attractive alternative to blood for drug monitoring and pharmacokinetic analysis, as it is non-invasive to retrieve. This is beneficial for patients, such as those with FOP, for whom blood draws can be challenging due to soft tissue damage susceptibility that can cause progress

A Pilot Study on the Role of Experience and Patient Gender on MARCH Treatment Sequence.

INTRODUCTION: The brevity of training for soldiers and combat medics to learn how to provide treatment on the battlefield may restrict optimal performance for treating chest and airway injuries, particularly when treating female soldiers. The present study tested treatment performance on patient simulators by battlefield medic trainees to determine whether there is a need for more extensive training on chest and airway procedures on female soldiers. MATERIALS AND METHODS: Battlefield medic trainees treated male and female patient simulators in counterbalanced order. The assessment considered

Blood Flow Restriction Therapy Preserves Lower Extremity Bone and Muscle Mass After ACL Reconstruction.

BACKGROUND: Muscle atrophy is common after an injury to the knee and anterior cruciate ligament reconstruction (ACLR). Blood flow restriction therapy (BFR) combined with low-load resistance exercise may help mitigate muscle loss and improve the overall condition of the lower extremity (LE). PURPOSE: To determine whether BFR decreases the loss of LE lean mass (LM), bone mass, and bone mineral density (BMD) while improving function compared with standard rehabilitation after ACLR. STUDY DESIGN: Randomized controlled clinical trial. METHODS: A total of 32 patients undergoing ACLR with bone-pat

Minimally Invasive Dorsal Cheilectomy and Hallux Metatarsal Phalangeal Joint Arthroscopy for the Treatment of Hallux Rigidus.

BACKGROUND: Hallux rigidus (HR) is a common source of forefoot pain and disability. For those who fail nonoperative treatment, minimally invasive dorsal cheilectomy (MIDC) is an increasingly popular alternative to the open approach with early positive results. Early failures may be due to lose bone debris from the MIDC as well as other intra-articular pathology that cannot be addressed with MIDC alone. Metatarsophalangeal (MTP) arthroscopy can be used in addition to MIDC to assess the joint after MIDC and address any intra-articular pathology while still maintaining the benefits of minimally i

Venomous snakebites.

More than 5 million people are bitten by venomous snakes annually and more than 100,000 of them die. In Europe, one person dies due to envenomation every 3 years. There is only one venomous snake species in Lithuania--the common adder (Vipera berus)--which belongs to the Viperidae family; however, there are some exotic poisonous snakes in the zoos and private collections, such as those belonging to the Elapidae family (cobras, mambas, coral snakes, etc.) and the Crotalidae subfamily of the Viperidae family (pit vipers, such as rattlesnakes). Snake venom can be classified into hemotoxic, neurot

[Ten years' experience with our method of wrist joint arthrodesis].

PURPOSE OF THE STUDY: Total arthrodesis of the carpal joint is a complex operative procedure that results in the restoration of joint stability in frontal and sagittal planes and improvement in function of the tendons of digits' extensors and flexors. We developed our own method based on the use of a special implant; in this study we report our long-term results. MATERIAL: Since 1992 we have carried out 54 total carpal arthrodeses, using our method, on 51 patients; three patients were treated bilaterally. Both wrists were treated in. This group comprised 34 women and 19 men, with the average

[Treatment of patellar cartilage defects by solid chondral graft: first experience].

PURPOSE OF THE STUDY: To present the results of a new method for treatment of cartilage defects of the patella, using a solid graft produced by chondrocytes on a three-dimensional matrix. MATERIAL AND METHODS: Chondrocyte transplantation is indicated in deep and extensive chondral and osteochondral defects of all large joints in patients at 18 to 40 years of age. When, on arthroscopic examination, a defect of patellar cartilage was found, healthy cartilage was collected from a non-weight-bearing surface, i. e., the intercondyllar fossa. In the tissue bank, the cartilage was fragmented, chondr

Carpal tunnel syndrome.

Carpal tunnel syndrome is one of the most common peripheral neuropathies. It affects mainly middle aged women. In the majority of patients the exact cause and pathogenesis of CTS is unclear. Although several occupations have been linked to increased incidence and prevalence of CTS the evidence is not clear. Occupational CTS is uncommon and it is essential to exclude all other causes particularly the intrinsic factors such as obesity before attributing it to occupation. The risk of CTS is high in occupations involving exposure to high pressure, high force, repetitive work, and vibrating tools.

Superior survival, hemodynamics, and metabolic recovery using PEG-20k-based low volume resuscitation in a polytrauma hemorrhagic shock model in rats.

BACKGROUND: Hemorrhagic shock is a major cause of preventable civilian and military trauma death. Although whole blood and blood products remain the standard of care, field‑stable alternatives are needed for resource‑limited environments and supply‑chain disruptions. Blood‑based resuscitation may also fail to optimally restore microvascular perfusion despite improved macrohemodynamics, contributing to ongoing ischemia-reperfusion injury, dysfunctional molecular cell dynamics, and capillary cell swelling. Prior controlled hemorrhage studies in rats and swine demonstrated superior survival, mean

Blood Flow Restriction Training for Meniscus Repair Surgery.

BACKGROUND: Blood flow restriction (BFR) is a training tool that involves wearing a tourniquet or occlusive device during exercise. Data support that low-load training with BFR may produce muscle hypertrophy similar to standard high-load training. Because of the weight-bearing and range of motion (ROM) restrictions after meniscal repair, patients encounter substantial atrophy of lower extremity musculature. We perform BFR for these patients to limit atrophy postoperatively with the goal of facilitating their return to prior function and sports. INDICATIONS: We incorporate BFR in the postopera

Lacertus Syndrome: an Outcome Analysis After Lacertus Release.

PURPOSE: The lacertus fibrosus or bicipital aponeurosis is a sheet of ligamentous tissue just distal to the elbow joint and can be a compression point for the median nerve. Essentially, lacertus syndrome is a subset of pronator syndrome and an uncommon diagnosis by itself. Surgical release of the lacertus consists of a small 2-cm incision that can be performed under local anesthesia. This study aimed to evaluate the outcome of lacertus release in resolving median nerve symptoms. METHODS: This retrospective study was performed at Prince Court Medical Centre, Kuala Lumpur, Malaysia, from Januar
