Gentamicin tympanoclysis: effects on the vestibular secretory cells.

Gentamicin application to the middle ear may relieve Meniere's disease, presumably by reducing endolymph secretion by the dark cells. To explore this possibility, the ears of adult cats were treated with daily intratympanic infusions of gentamicin until ataxia occurred. The temporal bones of these cats were then examined using electron microscopy. A 3 per cent solution of gentamicin resulted in ataxia after four treatments. Acute dark cell damage to basal infoldings seen after one month persisted at six months. Treatment with a 0.3 per cent solution required 15 and 21 days to effect ataxia and

A method of stapedectomy in the fenestrated ear.

Two consequences of a previous fenestration militate against successful stapledectomy. They are the lessened depth of the middle ear over the stapes, and the missing incus. Twelve years' experience of using the stapedectomy for fenestrated ears in 62 patients has given generally disappointing results until, in 1962, technique of using cartilage blocks to increase the depth of the middle ear was devised.

Value of CT and three-dimensional reconstruction revealing specific radiological signs for screening causative high jugular bulb in patients with Meniere's disease.

BACKGROUND: The aim of the present study was to investigate the pathological features of vestibular aqueduct (VA) related high jugular bulb (HJB) and explore the possible cause-consequence relation between HJB and endolymphatic hydrops (EH), and the potential specific radiological signs for screening causative HJB in Meniere's disease (MD). METHODS: High-resolution computed tomography (HRCT) and three-dimensional reconstruction (3DRC) were used to detect the anatomical variables associated with VA and jugular bulb (JB) in hydropic and non-hydropic ears. The presence or absence of EH in the in

Radiation exposure of normal temporal bone structures during stereotactically guided gamma knife surgery for vestibular schwannomas.

OBJECT: The dosimetry of radiation exposure of healthy inner, middle, and external ear structures that leads to hearing loss, tinnitus, facial weakness, dizziness, vertigo, and imbalance after gamma knife surgery (GKS) for vestibular schwannomas (VSs) is unknown. The authors quantified the dose of radiation received by these structures after GKS for VS to assess the likelihood that these doses contributed to postradiosurgery complications. METHODS: A retrospective study was performed using a prospectively acquired database of a consecutive series of 54 patients with VS who were treated with G

Quantifying Inner Ear Function in Children With a Diagnosis of Cancer: Assessment of Vestibular and Audiometric Function.

OBJECTIVES: While hearing loss is a known late effect of childhood cancer treatment, the impact of childhood cancer treatment on vestibular function has not been comprehensively explored. Vestibular impairment is associated with delayed gross motor development and impaired balance, and thus is important to measure in children. Few studies have examined peripheral vestibular end organ function in this population, and relationships between vestibular and audiometric function in children with cancer are limited. The purpose of this study was to (1) determine if tests of peripheral vestibular func

Changes in caloric responses after temporal bone surgery with posterior tympanotomy.

OBJECTIVE: To elucidate the role of facial recess bony plate in the thermal transmission route from external auditory canal to lateral semicircular canal during caloric stimulation test, we performed the test on patients before and after removal of the plate, i.e. posterior tympanotomy. In the present study, we adopted facial nerve decompression (FND) as posterior tympanotomy without surgery-induced inner ear damages and cochlear implantation (CI) as posterior tympanotomy with surgery-induced inner ear damages. METHODS: Between 1999 and 2003, we performed FND on 19 patients with unilateral fa

Sensorineural Hearing Loss Negatively Impacts Cognition in Older Subjects with Normal Lateral Semi-Circular Canal Function.

INTRODUCTION: Worldwide around 400 million people suffer from hearing loss. There is increasing evidence that hearing loss is a modifiable risk factor for cognitive decline. However, several risk factors for cognition are common in hearing-impaired individuals, including vestibular dysfunction, anxiety, and depression. This study aimed to explore the effect of hearing loss on cognitive functioning in older adults, considering the effects of vestibular function, anxiety, and depression on cognitive performance. METHOD: A cross-sectional study was performed on 42 subjects with moderate-to-sever

Superior Canal Dehiscence Syndrome: Relating Clinical Findings With Vestibular Neural Responses From a Guinea Pig Model.

HYPOTHESIS: In superior canal dehiscence (SCD), fluid displacement of the endolymph activates type I vestibular hair cells in the crista of the affected canal and thus irregular superior canal (SC) neurons in Scarpa's ganglion, which provides the neurophysiological basis for the clinical presentation of SCD. BACKGROUND: Patients with SCD display sound- and vibration-induced vertigo/nystagmus and increased amplitudes of vestibular evoked myogenic potentials. METHODS: Extracellular recordings from n = 25 primary vestibular neurons of 16 female guinea pigs were analyzed. We recorded from the sa

Vestibulo-ocular reflex deficits with medial longitudinal fasciculus lesions.

The medial longitudinal fasciculus (MLF) is the final common pathway for all conjugate adducting horizontal eye movements, as well as for the vertical-torsional vestibulo-ocular reflex (VOR). MLF lesion causes adduction paresis of ipsilesional (adducting) eye with dissociated nystagmus of contralesional (abducting) eye-the well-known clinical syndrome of internuclear ophthalmoplegia (INO). We measured the VOR stimulation and also any catch-up saccades, from individual semicircular canal (SCC) evoked by the head impulse test (HIT), using head and binocular 3-dimensional scleral search coils in

Sympathetic arousal to a vestibular stressor in high and low hostile men.

The aim of the present experiment was to extend the literature on hostility and a cerebral systems based model of sympathetic arousal to a vestibular-based stress. Several authors have concluded that autonomic stress reactivity in high hostile individuals must be interpersonally based, whereas healthy vestibular system functioning does not depend on interpersonal features. Utilizing a vestibular activation paradigm, skin conductance levels of 15 high hostile and 15 low hostile men were recorded after brief passive rotation about the vertical neuroaxis. It was expected that hostile individuals

Expression of BDNF and TrkB mRNAs in the crista neurosensory epithelium and vestibular ganglia following ototoxic damage.

Following ototoxic lesion with the aminoglycoside gentamicin, the vestibular neurosensory epithelia undergo degeneration and then limited spontaneous regeneration. The spatio-temporal expression of brain-derived neurotrophic factor (BDNF) and of its high affinity receptor (trkB) mRNA was investigated in the vestibular end organs and ganglia of chinchillas following gentamicin ototoxicity. In the vestibular ganglia of untreated chinchillas, the level of expression of BDNF mRNA is low. At 1 and 2 weeks after intraotic treatment with gentamicin, BDNF mRNA levels in the vestibular ganglia were ele

Scanning electron microscopic evaluation of age-related changes in the rat vestibular epithelium.

An ultrastructural study was performed to assess age-related changes in the vestibular end organs of Fischer 344 rats. The surfaces of the maculae and cristae from 3-, 12-, and 24-month-old Fischer 344 rats were observed by use of scanning electron microscopy. Age-related changes in the morphology of the vestibular neuroepithelium included a substantial loss of hair cells, as well as a reduction in the number of kinocilia and stereocilia on those that remained. These changes were greatest in the central upper regions of the three ampullae. In aged animals a greater area of the neuroepithelial

Computed tomography of local complications of temporal bone cholesteatomas.

The preoperative temporal bone CT scans of 13 patients with surgically proven acquired cholesteatoma were retrospectively reviewed for bony erosions and for extension beyond the temporal bone. A correlation with the clinical and surgical findings was also carried out. The CT findings are discussed and illustrated as well as some pitfalls in the evaluation of bony erosions. The appearance of "autoantrectomy" caused by a dry, self-cleaning, well ventilated cholesteatoma is also discussed and illustrated.

Aeromedical aspects of otolaryngology.

Abnormalities in the ENT area that are directly related to aerospace operations are largely secondary to man's persistence in operating outside his intended environment. These are spatial disorientation, motion sickness, barotrauma, noise-induced hearing loss, and certain clinical entities which have the capability of producing sudden incapacitation. Pertinent features of these primary problem areas are discussed.

[Human visual functions during mild vestibular exposures].

Human experiments have shown that after mild stimulation of semi-circular receptors, which induces no motion sickness symptoms, the time of recovery of the central vision acuity shortens and after stimulation of otolith receptors tends to increase. Thresholds of color discrimination as applied to the red, green and violet colours as well as the course of back adaptation after mild stimulation of otolith receptors remain unchanged. The time of appearance of the Purkinje phenomenon after stimulation of receptors of semi-circular canals often remains unaltered and sometimes increases. It is assum
