Creating healthy habits for Maryland preschoolers (CHAMP): a cluster-randomized controlled trial among childcare centers.

BACKGROUND: Risks for chronic health problems are embedded in preschoolers' dietary and physical activity habits. Childcare centers are a potential venue to establish healthy habits. We hypothesized that health-promoting center plus parent interventions would improve preschoolers' dietary and physical activity outcomes, including body weight, over control. METHODS: We made local modifications to the 30-week Food Friends® curriculum to develop a center intervention, Creating Healthy Habits Among Maryland Preschoolers (CHAMP), and a parent website (CHAMP+), aligned with the CHAMP intervention.

Wearable sensors in patient acuity assessment in critical care.

Acuity assessments are vital for timely interventions and fair resource allocation in critical care settings. Conventional acuity scoring systems heavily depend on subjective patient assessments, leaving room for implicit bias and errors. These assessments are often manual, time-consuming, intermittent, and challenging to interpret accurately, especially for healthcare providers. This risk of bias and error is likely most pronounced in time-constrained and high-stakes environments, such as critical care settings. Furthermore, such scores do not incorporate other information, such as patients'

Low carbohydrate intake correlates with trends of insulin resistance and metabolic acidosis in healthy lean individuals.

INTRODUCTION: Both obesity and a poor diet are considered major risk factors for triggering insulin resistance syndrome (IRS) and the development of type 2 diabetes mellitus (T2DM). Owing to the impact of low-carbohydrate diets, such as the keto diet and the Atkins diet, on weight loss in individuals with obesity, these diets have become an effective strategy for a healthy lifestyle. However, the impact of the ketogenic diet on IRS in healthy individuals of a normal weight has been less well researched. This study presents a cross-sectional observational study that aimed to investigate the eff

Effect of Timing and Coordination Training on Mobility and Physical Activity Among Community-Dwelling Older Adults: A Randomized Clinical Trial.

IMPORTANCE: Standard exercise interventions targeting underlying physiologic system impairments have limited success in improving walking. Augmenting standard interventions with timing and coordination training, which incorporates the principles of motor learning and integrates multiple systems, may be more successful. OBJECTIVE: To determine whether a standard strength and endurance program incorporating timing and coordination training (standard-plus) improves gait speed more than strength and endurance training alone. DESIGN, SETTING, AND PARTICIPANTS: The Program to Improve Mobility in A

Effect of Inpatient and Outpatient Pneumonia on Mobility Disability, Gait Speed, and Physical Activity in Older Adults.

Pathophysiological changes caused by pneumonia may influence physical functioning in older adults. This study was a secondary analysis of the Lifestyle Interventions and Independence for Elders (LIFE) Study. The LIFE Study included 1635 individuals over an average follow-up of 2.6 years at eight clinical sites during 2010-2013. Adults ≥70 years-old with mobility limitations (Short Physical Performance Battery score ≤9) were randomized to a physical activity (exercise) intervention or health education control arm. This analysis evaluated the association between pneumonia events and major mobili

Matrisome, innervation and oxidative metabolism affected in older compared with younger males with similar physical activity.

BACKGROUND: Due to the interaction between skeletal muscle ageing and lifestyle factors, it is often challenging to attribute the decline in muscle mass and quality to either changes in lifestyle or to advancing age itself. Because many of the physiological factors affecting muscle mass and quality are modulated by physical activity and physical activity declines with age, the aim of this study is to better understand the effects of early ageing on muscle function by comparing a population of healthy older and young males with similar physical activity patterns. METHODS: Eighteen older (69 ±

Early Postoperative Actigraphy Poorly Predicts Hypoactive Delirium.

STUDY OBJECTIVES: Delirium is a postoperative complication accompanied by disturbances in attention, cognition, arousal, and psychomotor activity. Wrist actigraphy has been advocated to study inactivity and inferred sleep patterns during delirium. We hypothesized that altered patterns of motor activity or immobility, reflective of disordered sleep and wakefulness patterns, would serve as predictive markers of hypoactive postoperative delirium. METHODS: Eighty-four elderly surgical patients were classified into three groups based on the timing of hypoactive delirium following surgery: intact w

Do COVID-19 recovered individuals differ from healthy controls in accelerometer-measured physical activity and self-reported sleep? a UAE-based study.

The COVID-19 pandemic has significantly impacted global health, with recent studies suggesting long-term aftermath effects on physical activity (PA) and sleep among recovered individuals. Pre-existing challenges, such as high rates of physical inactivity and sleep deprivation in the United Arab Emirates (UAE), may exacerbate these effects. This cross-sectional study compared accelerometer-measured PA and self-reported sleep between 65 individuals who had recovered from COVID-19 within the past year (mean age 23.28 ± 6.05 years; 56 had acute COVID-19; 6 had ongoing symptoms up to 12 weeks, and

Accelerometer-based portable navigation system shows no superior accuracy over pelvic alignment guide for acetabular cup placement in total hip arthroplasty in lateral decubitus position.

BACKGROUND: Total hip arthroplasty (THA) using a portable navigation system (PNS) incurs costs per procedure. However, it does not require a large console. This study aimed to compare the accuracy of acetabular cup placement using a pelvic alignment guide (PAG) attached to the pelvis and an accelerometer-based PNS in THA performed in the lateral decubitus position. METHODS: We retrospectively analyzed 100 hips that underwent primary THA in the lateral decubitus position between July 2018 and January 2021. The PAG was used in 50 hips, whereas the PNS was used in the other 50. Cup placement acc

Traditional versus dynamic sitting: Lumbar spine kinematics and pain during computer work and activity guided tasks.

Dynamic sitting may mitigate low back pain during prolonged seated work. The current study compared pelvis and lumbar spine kinematics, pain, and work productivity, in traditional and dynamic sitting. Sixteen participants completed three 20-min blocks of computer work and activity guided tasks in a traditional office chair or backless and multiaxial rotating seat pan while kinematics were measured from accelerometers on the low back. Pain ratings were recorded on a visual analogue scale every 10 min. Similar pelvis and lumbar kinematics emerged when performing computer work in traditional and

Cardiovascular risk and functional burden at midlife: Prospective associations of isotemporal reallocations of accelerometer-measured physical activity and sedentary time in the CARDIA study.

Cardiovascular risk and functional burden, or the accumulation of cardiovascular risk factors coupled with functional decline, may be an important risk state analogy to multimorbidity. We investigated prospective associations of sedentary time (ST), light intensity physical activity (LPA), and moderate to vigorous intensity physical activity (MVPA) with cardiovascular risk and functional burden at midlife. Participants were 1648 adults (mean ± SD age = 45 ± 4 years, 61% female, 39% Black) from Coronary Artery Risk Development in Young Adults (CARDIA) who wore accelerometers in 2005-2006 and 20

Quantitative assessment of cerebellar ataxia, through automated limb functional tests.

BACKGROUND: Cerebellar damage can often result in disabilities affecting the peripheral regions of the body. These include poor and inaccurate coordination, tremors and irregular movements that often manifest as disorders associated with balance, gait and speech. The severity assessment of Cerebellar ataxia (CA) is determined by expert opinion and is likely to be subjective in nature. This paper investigates automated versions of three commonly used tests: Finger to Nose test (FNT), test for upper limb Dysdiadochokinesia Test (DDK) and Heel to Shin Test (HST), in evaluating disability due to C

Cardiovascular fitness in narcolepsy is inversely related to sleepiness and the number of cataplexy episodes.

OBJECTIVE: Cardiopulmonary fitness depends on daily energy expenditure or the amount of daily exercise. Patients with narcolepsy spent more time being sleepy or asleep than controls; thus we may speculate that they have a lower quantity and quality of physical activity. The aim of the present study was thus to test the hypothesis that exercise tolerance in narcolepsy negatively depends on sleepiness. PATIENTS AND METHODS: The cross-sectional study included 32 patients with narcolepsy with cataplexy, 10 patients with narcolepsy without cataplexy, and 36 age- and gender-matched control subjects

Sedentary Time, Cardiorespiratory Fitness, and Cardiovascular Risk Factor Clustering in Older Adults--the Generation 100 Study.

OBJECTIVE: To determine whether meeting physical activity (PA) recommendations and/or having high age-specific cardiorespiratory fitness (CRF) attenuate the adverse effect of prolonged sedentary time on cardiovascular risk factor (CV-RF) clustering in older adults. PATIENTS AND METHODS: We conducted a cross-sectional study of Norwegian women (495) and men (379) aged 70 to 77 years from August 22, 2012, through June 30, 2013. Sedentary time and PA were assessed by accelerometers and CRF by directly measured peak oxygen uptake (VO2peak). Logistic regression was used to estimate adjusted odds ra

Ecologically optimizing exercise maintenance in men and women post-cardiac rehabilitation: Protocol for a randomized controlled trial of efficacy with economics (ECO-PCR).

BACKGROUND: Exercise-based cardiac rehabilitation (CR) participation results in increased cardio-metabolic fitness, which is associated with reduced mortality. However, many graduates fail to maintain exercise post-program. ECO-PCR investigates the efficacy and cost-effectiveness of a social ecologically-based intervention to increase long-term exercise maintenance following the completion of CR. METHODS/DESIGN: A three-site, 2-group, parallel randomized controlled trial is underway. 412 male and 192 female (N=604) supervised CR participants are being recruited just before CR graduation. Use of a force-sensing automated open field apparatus in a longitudinal study of multiple behavioral deficits in CAG140 Huntington's disease model mice.

Behavioral testing of mouse models of Huntington's disease (HD) is a key component of preclinical assessment for potential pharmacological intervention. An open field with a force plate floor was used to quantify numerous spontaneous behaviors in a slowly progressing model of HD. CAG140 (+/+, +/-, -/-) male and female mice were compared in a longitudinal study from 6 to 65 weeks of age. Distance traveled, wall rears, wall rear duration, number of low mobility bouts, in-place movements, number of high velocity runs, and gait parameters (stride rate, stride length, and velocity) were extracted f

Cytokines and their association with insulin resistance in obese pregnant women with different levels of physical activity.

BACKGROUND: Cytokines contribute to insulin resistance in pregnancy, but the role of distinct cytokines is not fully understood. OBJECTIVES: To study whether cytokines produced by tissues other than skeletal muscle are associated with glucose and insulin metabolism activity in overweight and obese women and to study whether these associations can be modified by physical activity. METHODS: A longitudinal study with 44 overweight and obese pregnant women was conducted. Changes in cytokines levels (IFN-γ, IP-10, IL1-α, MIP1-α, adiponectin and leptin) and ICAM1 from early (15wk) to late (32wk) p

Gender differences in children's voice use in a day care environment.

BACKGROUND: The prevalence of dysphonia is higher in boys than in girls before puberty. This could be because of the differences in boys' and girls' voice use. Previous research on gender differences in prepubescent children's voice parameters has been contradictory. Most studies have focused on examining fundamental frequency. OBJECTIVES: The purpose of this study was to investigate voice use in boys and girls in a day care environment based on the voice parameters fundamental frequency (Hz), vocal intensity (dB SPL), and phonation time (%) and to ascertain whether there were any significant

Heel-Tapping-Based Parkinson's Disease Progression Monitoring Using Smart Insoles.

Parkinson's Disease (PD) is a progressive neurodegenerative disorder marked by motor and non-motor symptoms, which complicate both diagnosis and disease monitoring. Traditional subjective assessments, such as the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) and Hoehn and Yahr scale, often fall short of capturing subtle motor variations across PD stages as well as require expertise and thus the accessibility is limited in daily life. In this study, we evaluated the feasibility of using in-phase (IP) and anti-phase (AP) heel- and toe-tapping as biomarkers to monitor PD progression object
