Planovalgus Foot Deformity in Patients Undergoing Total Hip Arthroplasty Is Associated With Increased Risk of Falls, Implant-Related Complications, and Revisions: A Case-Control Analysis.

BACKGROUND: Pes planus occurs due to the loss of the longitudinal arch of the foot, resulting in altered gait mechanics. This may lead to increased complications following total hip arthroplasty (THA). Thus, the aim of this study was to assess the effects that pes planus has on rates of falls, implant complications, fall-related injuries, and times to revision among THA patients. METHODS: A retrospective review of a private insurance claims database was conducted from 2010 to 2021. Patients who had a diagnosis of congenital or acquired pes planus and cases of THA were identified. Patients und

Results of resection for middle facet tarsal coalitions in adults.

The optimum surgical procedure for talocalcaneal coalitions has not been definitively determined. The authors performed this study to evaluate the results achieved with talocalcaneal tarsal coalition resection with regard to preoperative radiographic findings in relation to postoperative outcomes. They reviewed the medical records of 7 patients and conducted telephone interviews using a modified American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Scale. The average age at surgery was 31 years (range, 15-56 years), and the follow-up period averaged 17.4 months (range, 7-36 months). In 6

Forefoot flexibility and medial tibial stress syndrome.

PURPOSE: To investigate the association between medial tibial stress syndrome (MTSS) and morphology and flexibility of the foot arches. METHODS: 131 feet from 74 healthy subjects and 31 feet from 27 patients with MTSS were classified as normal feet (n=78 in 40 subjects), flat feet (n=53 in 34 subjects), or MTSS feet (n=31 in 27 patients). The medial longitudinal arch (MLA) ratio and the transverse arch length (TAL) were measured in both rearfoot and forefoot loading positions. The difference between the 2 positions indicated the flexibility of the MLA (diff-MLA ratio) and the transverse arch

The ground reaction force thresholds for detecting postural stability in participants with and without flat foot.

Foot mobility commonly contributes to injury via altered motion of the lower extremities. However, there is a lack of understanding of sensitive kinetic changes with flat foot. The purpose of this study was to investigate the threshold that best distinguishes between participants with and without flat foot. The kinetic stability index was developed based on the three-dimensional data from the ground reaction force (GRF) during one leg standing. In total, 34 control participants and 30 participants with flat foot were asked to maintain one leg standing for 25s with the contra lateral hip and kn

The incorporation of allogeneic and autogenous bone graft in healing of lateral column lengthening of the calcaneus.

Lateral column calcaneal lengthening as described by Mosca is a widely accepted technique for the correction of hindfoot valgus and pes planus. It is performed with both allogeneic and autogenous bone graft. The purpose of this study was to evaluate the incorporation of these 2 types of bone graft in this procedure. A retrospective review of all lateral calcaneal lengthenings performed by 4 pediatric orthopaedic surgeons over a 10-year period was conducted. Radiographs at the last follow-up visit were independently examined by 3 reviewers. Incorporation of the bone graft was determined by 2 cr

Free-flap salvage of soft tissue complications following the lateral approach to the calcaneus.

Soft tissue complications including wound slough and/or infection after lateral foot incisions used to approach the calcaneus are notoriously frequent. Simpler wound management may be possible when early, aggressive treatment is deployed. If definitive coverage has been delayed, local options typically become unreliable. Free tissue transfers may then become the mainstay for limb salvage. Over the past 2 decades in our joint experience in managing such complications referred to us, 8 free flaps were necessary in 7 patients who all had had some form of traumatic injury to the calcaneus. The mea

[Influence of the posterior tibial tendon on the medial arch of the foot: an in vitro kinetic and kinematic study].

INTRODUCTION: The respective contributions of the active and passive structures of the foot to the stability of the medical arch were investigated using an in vitro kinetic and kinematic model. The effect of the tibialis posterior tendon on foot and ankle movements, and plantar pressure distribution of the foot were tested in a cadaveric human foot. METHOD: The stance phase from heel-contact to toe-off of normal walking gait and after tibialis posterior tendon rupture was simulated in eight roentenographically normal human feet (age 66 +/- 19 years, males). Ground reaction force and tibial in

The Smith STA-peg: a 7-year retrospective study.

The Smith design subtalar joint arthroeresis has been performed at The Podiatry Hospital of Pittsburgh since 1982 for the surgical correction of hypermobile flatfeet in children. The authors review the historical background of the procedure with emphasis on surgical procedure, indications, and complications. The results of a 7-year retrospective study of over 40 cases revealed a 95% success rate with limited complications. Some interesting radiographic changes in the talus and calcaneus are described.

Surgical Repair of Traumatic Rupture of Tibialis Posterior Tendon in 32-Year-Old Male.

INTRODUCTION: This article reports a case of surgical repair of traumatic rupture of tibialis posterior (TP) tendon in a young healthy male after alleged history of grinder (heavy machine) injury over the foot, with the help of a suture anchor and running whip stitch followed by immobilization in a below knee slab postoperatively. The acute rupture of the TP tendon (TPT), compared to an acute rupture of the Achilles tendon, is a quite uncommon disease to be diagnosed in the emergency department setting. In most cases symptoms related to a TP dysfunction, like weakness, pain along the course of

Pilot Study: Human Adipose Tissue Allograft for Fat Pad Defects in Patients With Preulcerative Lesions.

BackgroundLoss or displacement of a fat pad on the foot increases plantar pressure, leading to pain and plantar ulcers. These ulcers, especially in patients with diabetic neuropathy, have high recurrence rates, often resulting in amputations. Standard of care focuses on reducing plantar pressure with shoe padding or orthotic devices, leaving the restoration of the fat pad as an unmet medical need. To address this, a human cryopreserved adipose tissue (hCAT) allograft has been developed to repair adipose tissue defects.MethodsScientific characterization of hCAT included assessments of its struc
