Optimal hearing is one goal of otologic surgery. This study examines the long-term functional success rate of cartilage as graft material for ossicular chain reconstruction in type III tympanoplasty. A retrospective review of seventy-nine patients having undergone type III tympanoplasty (53 with closed techniques and 26 with open techniques) was made. Lateral graft ("onlay") techniques were used in almost all cases. All operations were performed by one of the authors, a well-established otologic surgeon. Seventy-four percent of the reconstructions were performed in an one-stage procedure. The

OBJECTIVE: To discuss the effect of round window drill-out for otosclerosis cases with radiographic evidence of round window obliteration.
PATIENTS: Four adult patients (5 ears) were included with audiometric findings of a mixed hearing loss and radiographic evidence of otosclerosis causing round window obliteration.
INTERVENTIONS: Round window obliterative disease was addressed with a formal round window drill-out (RWD) by opening the lumen of the basal turn of the cochlea with a micro drill. The opening was then reconstructed with fascia to recreate a 2-window system.

CONCLUSION: We believe that a tensor tympani reflex, in response to loud sound, is present in a minority of people, although its functional significance with regard to sound transmission is questionable. The absence of startle response in our stapedectomized subjects makes us question whether the tensor component of the startle response is large enough to be identified by manometry.
OBJECTIVES: This study was organized to examine reflex activity to stimulation by loud sound or by startle in the tensor tympani. Although many previous studies have been carried out, results have been contradicto

UNLABELLED: WHY IS AN INTERNATIONAL OTOLOGY DATABASE IMPORTANT? There are many reports on the outcome of otological interventions in the literature. However, it is difficult to make direct comparisons between these reports because of the lack of uniformity in reporting outcomes and patient selection. The aim is to create an interactive otology database for surgeons in the UK and Europe.
METHODOLOGY: The proposed project is the creation of a common database on a website. Two levels of data entry are available: (1) level I: minimum database (only main outcomes are included; for any otolaryngolo

BACKGROUND: Using laser in stapedotomy has attracted a lot of attention since the nineties. It aims at minimizing complications due to hand-operated equipment, especially inner ear lesions, and achieving higher precision. We analyzed the clinical effect of Er:YAG laser with the question whether the postulated cochlea protection and foot plate perforation, optimized for better sound conduction, are achieved, compared to conventional stapedotomy.
PATIENTS AND METHODS: We evaluated retrospectively audiometrical data including pre- and postoperative bone and air conduction thresholds of 114 conse

The aim of this study was to evaluate the success of stapedectomy in patients who have previously had a tympanoplasty because of chronic otitis media (COM). Fourteen patients from a private otology practice had undergone tympanoplasty for COM and subsequently underwent stapedectomy. Measurements were taken of the air-bone gap (ABG) closure and pure tone average (PTA) which showed hearing improvement. Patients had a mean 36.9 dB PTA hearing gain with 79 per cent closing the ABG to within 20 dB. The need for stapedectomy alone is a rare occurrence for patients with a history of COM requiring a t

Endolymphatic hydrops (EH) associated with otosclerosis has been noted for many years. However, the causal relationship of these two entities remains controversial. Having reviewed the records of patients with otosclerosis describing fluctuant hearing loss and vertiginous symptoms, the authors found the EH may coexist with otosclerosis preoperatively; they may be two separate diseases that exist coincidentally; or EH may be caused by the otosclerotic process. Secondly, EH may occur with a fistula after surgery. Occurring after stapedectomy, EH may be caused by fistulization of the bony labyrin

This paper is a review of 132 cases of ossicular reconstruction utilizing prefabricated homografts by clinical staff members of the UCLA Division of Head and Neck Surgery (specializing in ear surgery) in their private practice over the past decade. Another 20 cases completed over the past 2 1/2 years in the UCLA teaching programme are also reviewed. Although ossicular homografts have been used for some time in middle ear reconstruction, this report presents the innovation of prefabricating homografts into specific shapes that lend themselves appropriately to reconstruction problems. Abnormalities in the ENT area that are directly related to aerospace operations are largely secondary to man's persistence in operating outside his intended environment. These are spatial disorientation, motion sickness, barotrauma, noise-induced hearing loss, and certain clinical entities which have the capability of producing sudden incapacitation. Pertinent features of these primary problem areas are discussed.

INTRODUCTION: Direct piston-stimulation of the oval window with a middle ear implant, following a stapedotomy or stapedectomy, is one treatment option for patients with otosclerosis. Here, we experimentally investigated whether increasing the surface area of the actuated piston enhances transmission efficiency in oval window (OW) stimulation.
METHODS: Fresh-frozen human cadaveric temporal bones (N = 14) were used to evaluate the output of different oval window couplers (OWCs) of A = 0.38 mm², 1.26 mm2, 3.06 mm2 by intracochlear pressure difference measurements between scala vestibuli (PSV) an
