[Severe upper gastrointestinal hemorrhage of a rare cause: aorto-esophageal fistula successfully treated].

Aorto-esophageal fistula is a rare entity, determining huge digestive bleeding with a very poor prognostic most often. We present the case of a patient with aorto-esophageal fistula of unknown origin, treated successfully in the Center of Surgery and Liver Transplantation from Fundeni Clinical Institute. A young man, 21 years old, was admitted in our clinic from another hospital for repetitive severe upper gastrointestinal bleeding, with hematemesis and melena, pale, asthenia, without any pain; the bleeding stopped when he was admitted. The upper digestive endoscopy and esophago-gastro-duodena

[Initial empirical antimicrobial therapy with piperacillin/tazobactam in intra-abdominal infections due to perforation of the large intestine and rectum and in postoperative complications after resection of the large intestine and rectum].

One of the basic therapeutic procedures in surgical intraabdominal infections is early administration of antimicrobial drugs. The basic requirement for the selection of antimicrobial drugs is a broad-spectum bactericide action with low toxicity. Within the framework of a retrospective non-comparative investigation the action of the betalactam antibiotic piperacillin combined with the betalactamase inhibitor tazobactam was tested which was used in the initial empirical antimicrobial treatment of 33 patients with intraabdominal infectious complications caused by perforation of the large bowel or

The newborn exstrophy bladder inadequate for primary closure: evaluation, management and outcome.

PURPOSE: The surgical approach to the small newborn exstrophy bladder inadequate for primary closure remains undetermined. Various methods for long-term management have been implemented. We evaluated our experience with late primary closure of the small exstrophied bladder template. MATERIALS AND METHODS: Our institutional database of patients treated and followed for the exstrophy-epispadias complex was reviewed. Of these patients 19 had a bladder template that was too small to close in the newborn period. The treatment and outcome of these 19 patients were reviewed. RESULTS: Of the 19 chil

Quality of life and functional outcomes after low anterior rectal resection with total mesorectal excision: a propensity score-matched comparison of robotic-assisted versus open surgery.

BACKGROUND: Robotic-assisted surgery (RAS) for rectal cancer is increasingly adopted. However, a recent systematic review highlighted a significant gap in the literature regarding quality of life (QOL) after different total mesorectal excision (TME) techniques, with open TME being particularly underrepresented. This study aimed to compare QOL and anorectal function between robotic-assisted and open low anterior rectal resection with TME. METHODS: Patients who underwent low anterior rectal resection with TME and primary anastomosis between 2006 and 2021 at a single tertiary centre were identif

Robotic Versus Laparoscopic Surgery for Crohn's Disease: A Systematic Review and Meta-Analysis with Trial Sequential Analysis.

BACKGROUND: Laparoscopic surgery (Lap) is widely recognized as the standard minimally invasive method for colorectal surgeries. Although robotic surgery (Rob) has seen increased adoption, its application in inflammatory bowel disease remains uncertain. METHODS: A systematic search was conducted across PubMed, Scopus, and the Cochrane Central Register of Controlled Trials up to February 2026. Pooled odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (CIs) were calculated using random-effects models. Heterogeneity was assessed via the inconsistency (I2) statistic. Impact of an ERAS-Based Surgical Care Bundle Implementation for Preventing Anastomotic Leakage in Minimally Invasive Low Anterior Resection for Rectal Cancer: A Retrospective Cohort Study.

BACKGROUND: Anastomotic leakage (AL) remains a severe complication after low anterior resection (LAR) for rectal cancer, despite advances in minimally invasive (MI) techniques. This study aimed to evaluate the impact of a surgery-focused care bundle, implemented on an enhanced recovery after surgery (ERAS)-based perioperative protocol, on preventing AL and improving postoperative outcomes in patients with MI-LAR. METHODS: In this retrospective historically controlled cohort study, a total of 306 patients who underwent MI-LAR between 2011 and 2024 were included. A late-phase cohort (n = 81) re

Vesicostomy in the Rat Model for Low-Pressure Urinary Drainage in Neurogenic Bladder Research.

The rat is the primary animal model for preclinical research on spinal cord injury (SCI)-induced neurogenic lower urinary tract dysfunction (nLUTD). Owing to its anatomical and functional similarity to humans, rats closely recapitulate key pathological consequences of SCI. However, a crucial translational gap remains: whereas standard bladder management in humans typically relies on continuous or intermittent catheterization, comparable approaches are not feasible in rodents. The common practice of manual bladder expression generates exceptionally high intravesical pressures, which may introdu

[Treatment of obstructive colorectal cancer].

Obstructive colorectal cancer is a common malignant bowel obstruction. Colostomy or colostomy following tumor resection may be the first choice for emergency surgery. The intestinal and systemic conditions of patients undergoing emergency surgery are often poor, and patients need to undergo multiple operations, which increase the surgical risk and economic burden and reduce the quality of life of patients. Poor intraoperative visualization may also affect the radical operation of emergency surgery. Transanal decompression tube (TDT) can rapidly decompress and drain the obstructed bowel, effect

Robotic versus laparoscopic low anterior resection following neoadjuvant chemoradiation therapy for stage II-III locally advanced rectal cancer: a single-centre cohort study.

Neoadjuvant chemo-radiotherapy (nCRT) of locally advanced rectal cancer is associated with challenging surgical treatment and increased postoperative morbidity. Robotic technology overcomes laparoscopy limitations by enlarged 3D view, improved anatomical transection accuracy, and physiologic tremor reduction. Patients with UICC stage II-III rectal cancer, consecutively referred to our institution between March 2015 and June 2020 (n = 102) were treated with robotic (Rob-G, n = 38) or laparoscopic (Lap-G, n = 64) low anterior resection (LAR) for total meso-rectal excision (TME) following highly
