Thiosemicarbazones derived from acetylpyrazines were prepared by condensing an acetylpyrazine or a ring-substituted acetylpyrazine with thiosemicarbazide. Dichloromethane extracts of the seeds of Nephelium lappaceum afforded two new diastereomeric monoterpene lactones, 1 and 2, and the known butenolide siphonodin (3), as well as kaempferol 3-O-beta-D-glucopyranoside-7-O-alpha-L-rhamnopyranoside. Compounds 1 and 2 represent a new monoterpene skeleton, and their structures were elucidated by extensive 1D and 2D NMR spectral data interpretation. Onychomycosis is caused by infection by fungi, mainly dermatophytes and nondermatophyte yeasts or moulds; it affects the fingernails and, more frequently, the toenails. Dermatophytes are responsible for about 90 to 95% of fungal infections. Trichophyton rubrum is the most common dermatophyte; Candida albicans is the major nondermatophyte yeast. Mycetoma is one of the six Neglected Tropical Diseases that are prevalent in Turkana County (northwest Kenya). BACKGROUND: There are currently three main treatment strategies for onychomycosis: topical, oral and combination. Amorolfine nail lacquer appears to be the most effective form of topical monotherapy. However, the best mycological and clinical cure rates are obtained with combination therapy. Combination therapy increases antifungal spectrum, fungicidal activity and safety. New antifungals (triazoles and echinocandins) were recently developed, enabling new protocols.

Econazole-nitrate is a new potent antifungal drug with a broad spectrum against dermatophytes, yeasts and moulds; in addition it is effective against gram-positive bacteria. Econazole nitrate was tested in-vitro for antifungal and anti-microbial properties. In an open trial 75 patients were treated with a 1 percent econazole cream. Cure was achieved in tinea pedis in 91 percent; in tinea genitocruralis in 100 percent and in tinea corporis in 92 percent. The remainder were greatly improved. 22 patients with erythrasma were cured within 3 weeks.
