[Long-term functional results in ossicular reconstruction with cartilage].

Optimal hearing is one goal of otologic surgery. This study examines the long-term functional success rate of cartilage as graft material for ossicular chain reconstruction in type III tympanoplasty. A retrospective review of seventy-nine patients having undergone type III tympanoplasty (53 with closed techniques and 26 with open techniques) was made. Lateral graft ("onlay") techniques were used in almost all cases. All operations were performed by one of the authors, a well-established otologic surgeon. Seventy-four percent of the reconstructions were performed in an one-stage procedure. The

Active middle ear implants-Enhanced efficiency of oval window stimulation with larger piston size.

INTRODUCTION: Direct piston-stimulation of the oval window with a middle ear implant, following a stapedotomy or stapedectomy, is one treatment option for patients with otosclerosis. Here, we experimentally investigated whether increasing the surface area of the actuated piston enhances transmission efficiency in oval window (OW) stimulation. METHODS: Fresh-frozen human cadaveric temporal bones (N = 14) were used to evaluate the output of different oval window couplers (OWCs) of A = 0.38 mm², 1.26 mm2, 3.06 mm2 by intracochlear pressure difference measurements between scala vestibuli (PSV) an

A Critical Look Into Stapedotomy Learning Curve: Influence of Patient Characteristics and Different Criteria Defining Success.

PURPOSE: To evaluate stapedotomy learning curve with cumulative summation methodology using different success criteria (ie, air-bone gap [ABG] ≤10 dB, ABG ≤15 dB, restoration of interaural symmetry, or hearing threshold gain >20 dB), and to assess patient characteristics influencing or modifying the learning curve. METHODS: Retrospective chart review of primary and revision stapedotomy cases performed by surgeon 1 (S1, n = 78) and surgeon 2 (S2, n = 85). RESULTS: Using the classic criterion for a successful stapedotomy (ABG ≤10 dB), patients with preoperative ABG >34 dB were associated with

CO2 laser stapedotomy safety: influence of laser energy and time on bone-conduction hearing levels.

Total laser energy in CO2 stapedotomy depends on the laser settings and the amount of applications. It is unclear if the amount of total laser energy affects bone-conduction hearing thresholds and if possible effects are temporary or permanent. Alterations of bone-conduction hearing thresholds after single or multiple-shot CO2 laser stapedotomy were analyzed between 1 and 3 weeks and 1.5-6 months after primary (n = 501) or revision surgeries (n = 153) and correlated to time, laser energy, frequency, surgical technique, and pathology encountered in revision stapedotomy. In both time periods, mo

Acoustic stapedius reflex function in man revisited.

OBJECTIVE: The objective of this study was to examine the role of the acoustic stapedius reflex in the protection of speech recognition from the upward spread of masking arising from low-frequency background noise. DESIGN: Speech recognition scores were measured for nine control participants (19-34 years) and six patients with transected stapedius tendons poststapedotomy (39-57 years) as a function of the amplitude of a low-frequency masker, presented at nominal signal to noise ratios of +5 dB, -5 dB, and -15 dB. All participants had pure-tone hearing thresholds in the normal range. Continuou

Osteoprotegerin expression and sensitivity in otosclerosis with different histological activity.

Otosclerosis is a complex bone dystrophy of the human otic capsule leading to conductive and sensorineural hearing loss. Since otosclerosis may, at least in part, be considered as an autoimmune-inflammatory disease, disturbed balance of TNF-alpha and osteoprotegerin (OPG) expression has been implicated in the pathological bone remodeling. It has been supposed that active otosclerosis is characterized by decreased or missing local OPG production with invariable OPG sensitivity of the otosclerotic foci. Ankylotic stapes footplates (n = 41) removed by stapedectomy were processed to histological e

Sex differences in patients with hearing impairments caused by otosclerosis.

The course of hearing in the non-operated ears was examined in 112 female patients and 63 male patients with bilateral otosclerosis who had operations in one ear only. All patients had a follow-up of at least 5 years. The mean annual impairment of the air conduction thresholds in the female patients (2.0-2.5 dB/year) did not differ significantly from that of the male patients (2.0-2.3 dB/ year). At the last follow-up examination, no difference was found between the sexes in the magnitude of the air-bone gaps. However, a sensorineural component developed in the women more frequently than in the

Early post-laser stapedotomy hearing thresholds.

OBJECTIVE: Auditory testing is not routinely performed within 4-6 weeks after stapedotomy, because hearing acuity is thought to be transiently depressed. In rare circumstances, postsurgical auditory and vestibular complaints may lead one to test hearing soon after stapedotomy. The early postoperative effects of carbon dioxide (CO2) and potassium titanyl phosphate (KTP) lasers, which now are routinely used to perform stapedotomies, have not been reported. The purpose of this report is to present normative data for auditory thresholds measured within 2 weeks of laser stapedotomy. STUDY DESIGN:

Hearing results in otosclerosis surgery after partial stapedectomy, total stapedectomy and stapedotomy.

Hearing results in a consecutive series of 407 patients with otosclerosis undergoing primary stapes surgery were analysed (437 operated ears). Partial stapedectomy was performed in 70 ears (16%), total stapedectomy in 205 ears (47%), in both groups using the House steel wire prosthesis on fascia in the oval window. The remaining 162 ears (37%) underwent stapedotomy using the Fisch 0.4 mm teflon-platinum piston. No case of cochlear loss (> 15 dB) occurred in the total series. The comparison between the three groups one year postoperatively showed that the air-bone gap was smaller for partial an

Stapedectomy in the elderly.

Clinical records were reviewed to examine the effectiveness of stapedectomy in patients 70 years and older. A total of 154 patients was studied, including 11 with profound hearing loss with long-standing otosclerosis. Ages at the time of surgery ranged from 70 to 92 years (mean, 76.3 years). The mean pure-tone average (500, 1,000, 2,000, and 4,000 Hz) improved 30.6 dB after surgery for the 143 patients in the main otosclerotic group and 26.8 dB for the patients in the profound-hearing-loss group. The rate of successful stapedectomies for the 70(+)-year-old patients (90.9%) and the younger comp

[Tympanoplasty with incus interposition].

We report on a 5-year experience with 44 patients (1980-1985) with incus interposition using a modelled or sculptured incus, either autograft or homograft, to correct ossicular discontinuity when a functional malleus and stapes are present. After a period of 21 months, 75% of our patients achieved an air-bone gap closure between 1 and 10 dB. The postoperative hearing results of patients with trauma and middle ear atelectases were better than the hearing results of patients with cholesteatomas. These long-term results are dependent upon intact malleus attic ligaments and an intact tensor tympan

Experimental investigations on the suitability of the carbon dioxide laser for stapedotomy.

Surgery of the footplate may cause a number of possible complications such as cochlear hearing defects or even complete loss of hearing. This has made it necessary to look for improved techniques. When a laser is used for stapedotomy, the energy transmitted to the cochlea must be reduced to the lowest possible level. We thus investigated the carbon dioxide laser to determine whether it would prove to be more advantageous than the argon laser. Animal experiments showed that CO2 laser irradiation is well tolerated. Tests performed on isolated petrous bone resulted in the development of a new sur

Stapes prosthesis: evaluation with CT.

We used computed tomography (CT) to evaluate 44 patients who had undergone stapedectomy with insertion of a prosthesis. Several patients had poor surgical results, including recurrent conductive hearing loss, vertigo, and sensorineural hearing loss. Conductive hearing loss occurring immediately after the procedure was most often caused by anatomic limitations or surgical technique. Causes of delayed or recurrent conductive hearing loss included reparative granuloma formation, incus necrosis (at the attachment of the wire), prosthesis subluxation (most often posterior), and regrowth of otoscler
