Case Report: Postpartum diagnosis and microsurgical management of a cervicothoracic intramedullary spinal ependymoma clinically manifesting during the third trimester of pregnancy.

BACKGROUND: Intramedullary spinal cord ependymomas are the most common intramedullary tumours in adults. However, clinical manifestation during pregnancy is exceptionally rare, and diagnosis may be delayed because pain, sensory disturbances, and muscle weakness are often mistakenly attributed to the common musculoskeletal changes associated with pregnancy. CASE DESCRIPTION: A 29-year-old multiparous woman developed progressive back pain, numbness, and weakness in both lower extremities at 30 weeks of gestation. She was initially managed conservatively. Following an uncomplicated spontaneous d

Real-world evaluation of gnomAD variant co-occurrence information for haplotype phasing in autosomal recessive disorders.

BACKGROUND: Haplotype phasing plays an essential role in the analysis of variants associated with autosomal recessive disorders, particularly in verifying compound heterozygosity. Recently, Genome Aggregation Database (gnomAD) introduced a population-based method to infer cis or trans configurations using variant co-occurrence information; however, its routine clinical utility remains unclear. RESULTS: A retrospective review was performed on 2,783 targeted gene panel tests conducted at Asan Medical Center, Korea, from 2017 to 2024. Through this analysis, 299 unrelated patients were identified

Motor phenotypes of amyotrophic lateral sclerosis - a three-determinant anatomical classification based on the region of onset, propagation of motor symptoms, and the degree of upper and lower motor neuron dysfunction.

BACKGROUND: In amyotrophic lateral sclerosis (ALS), heterogeneity of motor phenotypes is a fundamental hallmark of the disease. Distinct ALS phenotypes were associated with a different progression and survival. Despite its relevance for clinical practice and research, there is no broader consensus on the classification of ALS phenotypes. METHODS: An expert consensus process for the classification of ALS motor phenotypes was performed from May 2023 to December 2024. A three-determinant anatomical classification was proposed which is based on the (1) region of onset (O), (2) the propagation of

Defining diagnostic cutoffs in neurological patients for serum very long chain fatty acids (VLCFA) in genetically confirmed X-Adrenoleukodystrophy.

X-linked Adrenoleukodystrophy (X-ALD) is caused by mutations in the ABCD1 gene resulting in the accumulation of very long chain fatty acids (VLCFA). X-ALD is the most common peroxisomal disorder with adult patients (male and female) presenting with progressive spastic paraparesis with bladder disturbance, sensory ataxia with impaired vibration sense, and leg pain. 80% of male X-ALD patients have an adrenal failure, while adrenal dysfunction is rare in women with X-ALD. The objective of this study was to define optimal serum VLCFA cutoff values in patients with X-ALD-like phenotypes for the dif

Dietary Intake and Serum Selenium Levels Influence the Outcome of HTLV-1 Infection.

Human T cell leukemia virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP), as the most common neurological emersion related to HTLV-1, is a debilitating and lifelong treating disease with no definitive treatment. Furthermore, it has been determined that dietary compositions (inflammatory and anti-inflammatory) and some micronutrients (such as vitamin D and selenium) have an effect on inflammatory and immune processes and with this background; the study was done to compare the nutritional status between age- and sex-matched with infected and non-infected HTLV-1. HTLV-1 contains a high CG dinucleotide content and is susceptible to the host antiviral protein ZAP.

BACKGROUND: Human T cell leukaemia virus type 1 (HTLV-1) is a retrovirus associated with human diseases such as adult T-cell leukaemia/lymphoma and HTLV-1 associated myelopathy/tropical spastic paraparesis. In contrast to another human retrovirus, human immunodeficiency virus type 1 (HIV-1), HTLV-1 persists in the host not via vigorous virus production but mainly via proliferation and/or long-term survival in the form of silent proviruses in infected host cells. As a result, HTLV-1-infected cells rarely produce virus particles in vivo even without anti-retroviral treatment. That should be an a

Evaluation of Line Immunoassay to Detect HTLV-1 Infection in an Endemic Area, Southwestern Japan; Comparison with Polymerase Chain Reaction and Western Blot.

BACKGROUND: Human T-lymphotropic virus type 1 (HTLV-1) has been recognized as a cause of adult T-cell leukemia/lymphoma, HTLV-1-associated myelopathy/tropical spastic paraparesis, and HTLV-1-associated uveitis. HTLV-1 infection is normally detected by screening for HTLV-1 antibodies, and positive samples are confirmed by Western blot (WB). However, WB fails to confirm some samples that were positive for HTLV-1 antibodies on screening. Line immunoassay (LIA) is commonly used in Europe and Brazil, but not in Japan. Therefore, we evaluated the performance of LIA as a method of confirming HTLV-1 a

Family Aggregation of Human T-Lymphotropic Virus 1-Associated Diseases: A Systematic Review.

Human T-lymphotropic virus 1 (HTLV-1) is a retrovirus that produces a persistent infection. Two transmission routes (from mother to child and via sexual intercourse) favor familial clustering of HTLV-1. It is yet unknown why most HTLV-1 carriers remain asymptomatic while about 10% of them develop complications. HTLV-1 associated diseases were originally described as sporadic entities, but familial presentations have been reported. To explore what is known about family aggregation of HTLV-1-associated diseases we undertook a systematic review. We aimed at answering whether, when, and where fami

HTLV-1 modulates the frequency and phenotype of FoxP3+CD4+ T cells in virus-infected individuals.

BACKGROUND: HTLV-1 utilizes CD4 T cells as the main host cell and maintains the proviral load via clonal proliferation of infected CD4+ T cells. Infection of CD4+ T cells by HTLV-1 is therefore thought to play a pivotal role in HTLV-1-related pathogenicity, including leukemia/lymphoma of CD4+ T cells and chronic inflammatory diseases. Recently, it has been reported that a proportion of HTLV-1 infected CD4+ T cells express FoxP3, a master molecule of regulatory T cells. However, crucial questions remain unanswered on the relationship between HTLV-1 infection and FoxP3 expression. RESULTS: To i

Cytokine induction in HTLV-I associated myelopathy and adult T-cell leukemia: alternate molecular mechanisms underlying retroviral pathogenesis.

The human T-cell lymphotropic virus type I (HTLV-I) is capable of inducing a variety of host cellular genes including many of the cytokines responsible for immune regulation and osteoclast activation. This derangement in cytokine expression may contribute to the panoply of disease states associated with HTLV-I infection such as the adult T-cell leukemia (ATL) and HTLV-I associated myelopathy/tropical spastic paraparesis (HAM/TSP). We wished to determine if there was a correlation between the expression of an array of cytokines and the diverse clinical manifestations of ATL and HAM/TSP. Utilizi

HTLV-I associated retinochoroidal degeneration.

A review of 100 patients with HTLV-I associated myelopathy revealed 9 cases of retinochoroidal degeneration. The ocular disease was characterized by late onset, usually bilateral, progressive degenerative changes of the retina and choroid, which were preceded by, concurrent with, or followed by HTLV-I associated myelopathy. Ophthalmoscopic features were consistent with diffuse or localized retinochoroidal degeneration, the resultant functional disturbance being dependent on the extent of disease. It is noteworthy that the clinical picture in 3 cases was indistinguishable from primary retinitis

Characterization of T cells transmigrating through human endothelial cells in patients with HTLV-I-associated myelopathy.

We investigated the surface markers as well as the expression of beta 2-integrin (LFA-1 beta/CD 18), in T cells migrating through human umbilical vein endothelial cells (EC) in patients with HTLV-I-associated myelopathy (HAM). No significant differences were found in the percentages of both HLA-DR+ T cells and total CD4+ cells and in the expression of beta 2-integrin between the EC-transmigrated and the EC adherent T cells. However, the percentages of HLA-DR+CD4+ cells in the transmigrated cells were significantly higher than those in the adherent cells. These results suggest that activated or

Establishment and characterization of an HTLV-I cell line from a Taiwanese patient with HTLV-I-associated myelopathy.

We describe a Taiwanese woman with chronic progressive myelopathy, in whom Western blot analysis of the serum and cerebrospinal fluid (CSF) displayed positive reactions to human T-lymphotropic virus type I (HTLV-I) proteins, p19, p24, p28, p36, gp46 and p53. HTLV-I proviral genomes were detected in the peripheral blood mononuclear cells (PBMC) and CSF cells by nested polymerase chain reaction and Southern blot hybridization. HTLV-I was successfully isolated from PBMC stimulated with interleukin-2 (IL-2). The established cell line, named THAM-1, was an IL-2-independent T-cell line with CD2+, CD

Detection of virus-specific T cells and CD8+ T-cell epitopes by acquisition of peptide-HLA-GFP complexes: analysis of T-cell phenotype and function in chronic viral infections.

Antigen-specific CD8+ T cells acquire peptide-major histocompatibility complex (MHC) clusters through T-cell receptor (TCR)-mediated endocytosis after specific antigen stimulation. We generated an antigen-presenting cell (APC) expressing human leukocyte antigen (HLA)-A*201 coupled to the enhanced green fluorescent protein (GFP), which delivered GFP to an antigen-specific T cell when pulsed with antigenic peptide. We quantitatively identified human T-cell lymphotropic virus type I (HTLV-I) Tax(11-19) peptide-specific T-cell populations in peripheral blood mononuclear cells (PBMCs) from patients

Sequence analysis of an immunogenic and neutralizing domain of the human T-cell lymphoma/leukemia virus type I gp46 surface membrane protein among various primate T-cell lymphoma/leukemia virus isolates including those from a patient with both HTLV-I-associated myelopathy and adult T-cell leukemia.

Human T-cell lymphoma/leukemia virus type I (HTLV-I) causes adult T-cell leukemia/lymphoma and HTLV-I-associated myelopathy. Specific regions within the outer envelope proteins of other retroviruses, e.g., human immunodeficiency virus type 1, are highly immunogenic and, because of the selective pressure of the host immune system, quite variable. Mutations in the external envelope protein gene of murine retroviruses and human immunodeficiency virus type 1 influence cellular tropism and disease pathogenesis. By contrast, no disease-specific viral mutations have been identified in HTLV-I-infected

Hybrid Machine Learning Models for Predicting Types of Human T-cell Lymphotropic Virus.

Life threatening diseases like adult T-cell leukemia, neurodegenerative diseases, and demyelinating diseases such as HTLV-1 based myelopathy/tropical spastic paraparesis (HAM/TSP), hypocalcaemia, and bone lesions are caused by a group of human retrovirus known as Human T-cell Lymphotropic virus (HTLV). Out of the four different types of HTLVs, HTLV-1 is most prominent in scourging over 20 million people around the world and still not much effort has been made in understanding the epidemiology and controlling the prevalence of this virus. This condition further worsens when most of the infected
