Finite element analysis of three patterns of internal fixation of fractures of the mandibular condyle.

The most stable pattern of internal fixation for fractures of the mandibular condyle is a matter for ongoing discussion. In this study we investigated the stability of three commonly used patterns of plate fixation, and constructed finite element models of a simulated mandibular condylar fracture. The completed models were heterogeneous in the distribution of bony material properties, contained about 1.2 million elements, and incorporated simulated jaw-adducting musculature. Models were run assuming linear elasticity and isotropic material properties for bone. This model was considerably large

Development and application of reflexodent in the quantitative functional evaluation of chewing control in patients with temporomandibular joint dysfunction and a control group.

BACKGROUND: There has been controversy with respect to the diagnostic value of the inhibitory masseteric reflex in temporomandibular joint dysfunction (TMJD) because the whole reflex response was not considered. The purpose of this study was to characterize the reflex changes that occur in patients with different levels of TMJD and in a control group. METHODS: Eighty-nine patients (ages 31.14 +/- 12.74 years) divided into three groups were studied and compared. The control group was without TMJD (n = 30), with moderate symptoms (n = 30), and with severe symptoms (n = 29). Using an instrument

Prevalence, patterns, and impact of myofascial pain in patients with head and neck cancer after cancer treatment - a single-center cross-sectional study in India.

PURPOSE: Head and neck cancer is the seventh most prevalent cancer, with over 660,000 new cases and 325,000 annual fatalities, accounting for 30% of all cancer cases. Chronic cancer-related pain affects 15-75% of patients, with myofascial pain being especially common in those with head and neck cancers, ranging from 11.9 to 44.8%. Surgery and radiotherapy, the primary treatments for these cancers, contribute to myofascial pain development. Additionally, head and neck cancer patients face higher psychological distress, with rates up to 50%. This study estimates the prevalence, topography of the

Functional and Molecular Characterization of a Novel Traumatic Peripheral Nerve-Muscle Injury Model.

Traumatic injuries to human peripheral nerves are frequently associated with damage to nerve surrounding tissues including muscles and blood vessels. Currently, most rodent models of peripheral nerve injuries (e.g., facial or sciatic nerve) employ surgical nerve transection with scissors or scalpels. However, such an isolated surgical nerve injury only mildly damages neighboring tissues and weakly activates an immune response. In order to provide a rodent nerve injury model accounting for such nerve-associated tissue damage and immune cell activation, we developed a drop tower-based facial ner

Anesthetic considerations for masticatory muscle tendon-aponeurosis hyperplasia: a report of 24 cases.

Masticatory muscle tendon-aponeurosis hyperplasia (MMTAH) is a new disease entity characterized by limited mouth opening due to contracture of the masticatory muscles, resulting from hyperplasia of tendons and aponeuroses. In this case series, we report what methods of airway establishment were conclusively chosen after rapid induction of anesthesia. We had 24 consecutive patients with MMTAH who underwent surgical release of its contracture under general anesthesia. Rapid induction of anesthesia with propofol and rocuronium was chosen for all the cases. In 7 cases, intubation using the Macinto

Ring fibers visualized by electron microscopy in a Japanese patient with malignant hyperthermia.

This report describes an episode of malignant hyperthermia (MH) in a 53-year-old Japanese man during general anesthesia that was triggered by isoflurane and succinylcholine. His past history and family history were unremarkable. From our analysis of ten exons, he had no recognizable mutation in the ryanodine receptor gene, but compatible with his MH reaction, he showed positive sensitivity to the Ca-induced Ca-release test. Histochemical and electron microscopic studies of muscle biopsy tissue demonstrated unusual "ring fibers," which have never before been reported to be associated with MH. T

Peptide 19 in the rat superior cervical ganglion.

Peptide 19 is a 7.6 kDa polypeptide which can bind to calmodulin and inhibit calcium-calmodulin signaling. In this study, peptide 19-immunoreactivity was examined in the rat superior cervical ganglion. In the ganglion, 54.8% of postganglionic sympathetic neuron profiles were immunoreactive for peptide 19. These neuron profiles were small- to medium-sized and measured 87-845 microm(2) (mean+/-SD = 343+/-111 microm(2)). Double immunofluorescence method revealed that 99.9% of peptide 19-containing neurons had neuropeptide Y in the superior cervical ganglion. Retrograde neuronal tracing and immuno

Incorrect orofacial functions until 5 years of age and their association with posterior crossbite.

INTRODUCTION: In addition to heredity, sucking habits, impaired nasal breathing, and atypical swallowing patterns are considered important factors in the etiology of posterior crossbite. The purpose of this study was to assess irregular orofacial functions to determine their correlation with posterior crossbite. METHODS: Two hundred forty-three children were examined at the ages 3, 4, and 5. Irregular orofacial functions and morphologic traits of malocclusion were clinically evaluated. The prevalence of posterior crossbite and the relationship with incorrect orofacial functions was determined

[An autopsy case with lower motor neuron disease showing a transient-appearance of anti-GM1 antibody and an improvement of conduction block after gamma-globulin administration].

We report a 63-year-old man who died of respiratory failure. He was well until 1992 (57 years of his age), when he had an onset of progressive weakness of the bilateral upper limbs. He showed no improvement with TRH administration in other hospital. On January 12, 1994, he admitted to our department because of the progressive muscle weakness. Neurologic examination revealed a muscular atrophy associated with severe weakness and hyporeflexia in both upper limbs, and fasciculation were seen in his tongue. Electrophysiological studies revealed mild conduction block in the left medial nerve, and F

[Clinical experience in interscalene brachial plexus block combined with Ho's method of C3-4 block for shoulder and proximal upper extremity surgeries].

BACKGROUND: Brachial plexus block, first performed in 1889 by Halsted, has been widely used for surgery of shoulder and upper third of upper extremity. But the level of block is inadequate for surgery of the deeper tissue. If high volume of local anesthetic (40 ml) is used to block C3-4, complications like Horner's syndrome and phenic nerve palsy would be frequent. The landmark of C-3 and C-4 nerve root is difficult to identify. The purpose of this study was to design a new method to block easily the C-3 and C-4 nerve roots for surgery of shoulder deep tissue. METHODS: Sixty-five patients wit
