HYPOTHESIS: Histopathologic examination of temporal bones from patients who had undergone stapedectomy may provide information concerning the causes of both residual and recurrent conductive hearing loss (CHL).
BACKGROUND: Although closure of the air-bone gap to within 10 dB occurs in approximately 90% of primary stapedectomies, a residual CHL occurs in approximately 10% and recurrent CHL may occur in up to 35% of cases. Putative causes of failure of surgery as determined during revision include erosion of the incus, bony regrowth at the oval window, and displacement of the prosthesis. Most r

Hearing results are presented for 627 ears with chronic otitis media that were operated on radically and obliterated (Palva flap) and in which a tympanoplasty was performed. The ears were examined annually for five to 14 years (mean, 8.8 years). The long-term improvement (five to 14 years after the surgery) was the greatest in ears with an intact ossicular chain and in ears with ossicular reconstruction using autograft or homograft ossicles or autogenous cortical bone columellae. As a whole, the early (one year after the operation) improvement in the air-bone gap was 8.0 dB when compared with

Studies of the type presented here have rarely been undertaken and should be useful in surgery and pathology of the inner and middle ear. Samples of perilymph were collected during stapedectomy in patients with otosclerosis. In view of the very small volume of perilymph obtainable from each patient's vestibule (2 to 4 microliter), the only assay method that could be used to measure drug levels was thin layer chromatography on silica gel. Despite pooling of the perilymphs of 5 patients, trimethoprim (TMP) levels could not be measured but the authors were able to demonstrate that sulfamethoxazol

This paper presents the authors' experiences with one method of reconstruction of the tympanic membrane when it is totally absent or when there is a fixed, retracted, defective, or absent malleus. Ten patients undergoing surgery for chronic suppurative otitis media between April, 1971, and January, 1973, had homograft tympanic membranes and/or ossicles used to reconstruct the defect in the tympanic membrane or ossicular mechanism. The patients had certain clinical and surgical characteristics in common: 1. all had either an absent tympanic membrane or almost total perforation of it; 2. all wer

Otology & neurotology : official publication of the American Otological Society, American Neurotology Society [and] European Academy of Otology and Neurotology2014eng
BACKGROUND: In the further development of alloplastic prostheses for use in middle ear surgery, the Dresden and Cologne University Hospitals, working together with a company, introduced a new partial ossicular replacement prosthesis in 2011. The ball-and-socket joint between the prosthesis and the shaft mimics the natural articulations between the malleus and incus and between the incus and stapes, allowing reaction to movements of the tympanic membrane graft, particularly during the healing process.

INTRODUCTION: The diagnosis of perilymphatic fistula (PLF) is difficult since no single clinical situation gives the diagnosis for sure. The goal of this study is to clarify the clinical situations where you must suspect a PLF.
METHODS: Retrospective study of 20 patients that had an exploratory tympanotomy with a PLF confirmed peroperatively. An analysis of the symptoms, signs and complementary exams was done. The surgical findings and the postoperative evolution were noted.
RESULTS: 100% of patients reported a hearing loss, 80% vertigo, 70% a tinnitus and 35% equilibrium problems. Every pat

HYPOTHESIS: The spatial and temporal surface temperature distribution was measured after laser irradiation in fresh porcine otic capsule and calvarial bone tissue using an HgCdTe (mercury-cadmium-tellurium) infrared camera.
BACKGROUND: Carbon dioxide (CO2) (lambda = 10.6 mm), argon (lambda = 514 nm), and Potassium-Titanyl-Phosphate Neodynium: Yttrium-Aluminum-Garnet (KTP[Nd:YAG]) (lambda = 532 nm) lasers are used for stapes surgery and in the treatment of chronic ear disease. Despite extensive clinical use, little is known about the thermal perturbations in otic capsule calcified tissues and

INTRODUCTION: The influence of age on threshold changes of bone conduction after stapedectomy has not been thoroughly studied. Improvement of the Carhart notch by stapes surgery might be related to age as well as an increase in cochlear sensitivity to surgical trauma.
PATIENTS AND METHODS: A retrospective study on the outcome of stapedectomy was undertaken. The results of surgery performed on 387 ears in 315 patients for otosclerosis between 1962 and 1989 were obtained. Stapedectomy, performed by a single surgeon, consisted of total stapedectomy, seal of the oval window with tragal perichondr

Introduction: Stapedotomy improves air conduction thresholds and restores one of the components of bone conduction (BC): sound transmission to the inner ear via vibrations of the ossicular chain. This results in a measurable improvement in BC thresholds. Audiometric confirmation of pseudoperceptual hearing loss is the loss of the Carhart's notch (CN) observed after surgery. Sensorineural hearing loss is characterized by varying degrees of speech incomprehension impairment, particularly clinically apparent when the impairment affects frequencies essential for communicati

Conductive hearing loss (CHL) commonly results from middle ear fluid and inflammation (otitis media). Less commonly in patients with CHL, the middle ear cleft is well aerated or 'dry' with absence of soft tissue or fluid clinically and on imaging. There are numerous causes for this but they can be clinically challenging to diagnose. This pictorial review aims to illustrate and discuss the CT features of both common and less common causes of CHL in patients with a "dry middle ear cavity".

To report and analyse our results and complications of argon laser assisted stapedotomy for primary otosclerosis. A retrospective analysis of 135 consecutive cases of primary otosclerosis operated by the senior author (JH) has been performed. The air-bone gap was calculated by using the pure tone average at 500, 1000, 2000 and 4000 Hz. A separate analysis of air-bone gap at 500, 1000 and 2000 Hz was carried out to assess the effects of a small diameter piston on low frequency hearing. Hearing at high frequencies (4000 and 8000 Hz) was also assessed to evaluate effects of small fenestra techniq

Controversy exists concerning stapedectomy for patients with small air-bone gaps. The purpose of this study was to examine the results for patients who had a stapedectomy to correct a small (10 dB or less) air-bone gap. One hundred fifty-four patients with suspected otosclerosis were explored and a stapedectomy was performed in 136 (88.3%) of these cases. The mean pure-tone average (PTA) improved 16.7 dB and overdosed the preoperative bone conduction PTA by 8.1 dB. The majority of the stapedectomy patients (89.7%) had a PTA closure greater than or equal to 0 dB. These results showed that stape
