Attenuated or partial 21-hydroxylase deficiency is one of several biochemical defects in steroid metabolism that can lead to hirsutism in young women after puberty. The diagnosis is made through the exaggerated response of 17 alpha-hydroxyprogesterone (17-OH) to adrenocorticotrophic hormone (ACTH). To provide reference data 72 mild to moderately hirsute patients aged 18 to 35 were studied (over two years) with the ACTH test. Four patients with an exaggerated response were found. The roles of, and interactions between, steroids and naloxone, an opioid antagonist, in the reversal of experimental hypotensive shock were studied in normal and adrenalectomized rats. In normal rats treated with dexamethasone or deoxycorticosterone or 17-hydroxyprogesterone the hypotension and shock caused by 1% bodyweight and 2% bodyweight haemorrhage could be substantially reversed by naloxone in a dose-related manner. Detection of heterozygote carriers for congenital adrenal hyperplasia by use of a modified tetracosactide (a synthetic corticotropin) stimulation test with prior overnight dexamethasone suppression proved to have a diagnostic accuracy of 95%. For the purpose of studying the metabolism of adrenocortical steroids in primary aldosteronism, diurnal variation and the effect of dexamethasone were examined in four patients. The circadian rhythm of cortisol was observed in plasma aldosterone and corticosterone in all cases in the preoperative state. The plasma 17 alpha-hydroxyprogesterone (17-OHP) concentration was determined in the basal state and 60 minutes after cosyntropin, 0.25 mg, in 139 patients with idiopathic hirsutism (IH) and polycystic ovarian disease (PCOD). Acne vulgaris may be a manifestation of hyperandrogenism. An appropriate, thorough history and physical exam will identify evidence of androgen excess. An understanding of normal androgen production and conditions of androgen excess is critical when evaluating for hyperandrogenism. We report a 6-year-old girl with striking signs of virilization, as well as elevated concentrations of testosterone and 17-hydroxyprogesterone. Although the elevated 17-hydroxyprogesterone concentration initially suggested late-onset 21-hydroxylase deficiency, she was found to have an ovarian mass by sonography which at surgery proved to be a hilus cell subtype of steroid cell tumor. This intra-ovarian tumor appears to be the smallest tumor of its type ever reported. The testosterone concentration was normal three days after tumor resection and has remained so two years later.

Hirsutism is a common medical condition that in most women is due to the polycystic ovary syndrome or is idiopathic. For a few women, hirsutism signals a serious underlying disorder such as an ovarian or adrenal tumor, congenital adrenal hyperplasia, or Cushing's syndrome. A detailed medical history and examination can identify women in whom a serious disease is suspected and for whom laboratory evaluation is warranted. Measurements of serum testosterone, dehydroepiandrosterone, and 17 alpha-hydroxyprogesterone levels, and 24-hour urinary cortisol concentrations are important screening tests.
