Acetabular deficiency in spastic hip subluxation.

BACKGROUND: The direction of spastic hip subluxation is generally agreed to be posterolateral; however, the location of acetabular deficiency is still controversial. This study aims to define the degree and location of acetabular deficiency that is crucial in choosing acetabuloplasty method. METHODS: Twenty-five children with spastic cerebral palsy, average aged 7.1 years (range, 4.4 to 9.6 y), underwent 3-dimensional computed tomography (CT) studies of the pelvis. Mean Reimers migration percentage of the 50 hips was 32.6% (range, 16% to 66%). Twenty-eight hips were nonsubluxated and the othe…

The Success of Hip Arthroscopy in an Active Duty Population.

PURPOSE: To examine the outcomes of arthroscopic treatment of the hip in a young, active military population. Specifically, the ability to r … most common diagnosis was femoroacetabular impingement, and the most common procedure performed was acetabuloplasty. Twenty-two percent of patients underwent evaluation by the medical retention board after hip arthroscopy and were separated from military service. Seventy-eight percent of soldiers were maintained on active duty after hip arthroscopy. The overall c …

Intrapelvic protrusion of the ceramic head resulting from forte ceramic liner fracture without a significant trauma: a case report.

In this article, we present a 48-year-old female case who had an unsuccessful previous forte ceramic-on-ceramic total hip arthroplasty for early coxarthrosis secondary to acetabular dysplasia. She had fracture and protrusion of the ceramic liner through the titanium acetabular shell without any apparent trauma, possibly resulting from the mistake during the implantation of the ceramic liner into the metal shell in the initial operation. The patient had no other risk factors (component malposition, trauma, malalignment) for ceramic liner fracture. She underwent revision surgery with allograft,

Editorial Commentary: Primary Treatment of Hip Labral, Osseous, Chondral, Capsular, and Extra-Articular Pathology Is Essential: Outcomes Following Revision Are Less Predictable.

Appropriate labral management is one of many procedures during hip arthroscopy that affects postoperative outcomes and revision rates. Both … sseous deformities, with care taken to avoid under- and over-resection during both femoroplasty and acetabuloplasty; management of chondral injury; and management of the hip capsule with repair or plication. Residual dysplasia of the hip after successful ultrasound-monitored treatment: how does an infant's hip evolve?

Despite that normal values for the hip joint are reached at the end of ultrasound-monitored-treatment, the development of the acetabulum can … uated 'severely dysplastic', in the third follow-up only 1. On the other hand, three hips underwent acetabuloplasty. Even after normal values have been achieved at the end of ultrasound-monitored treatment, there remains a risk of residual dysplasia of the hips. Particularly, when the first radiographic examination shows nonphysiological findings, further close-m …

Acetabular reconstruction after pelvic tumor resection: are biological methods justified?

BACKGROUND: Acetabular reconstruction after pelvic tumor resection represents one of the most demanding challenges in orthopedic oncology. While non-biological methods are widely applied, their long-term durability and complication profiles remain concerning. Biological reconstruction using recycled bone offers the potential for durable and stable outcomes, though its role in the acetabulum remains debated. This study aimed to evaluate the radiological, functional, and oncological outcomes of biological acetabular reconstruction and to analyze associated complications in a single-center cohort

The role of the acetabulum in Perthes disease.

The purpose of this study is to evaluate the changes in the shape of acetabulum from the onset of the disease to the skeletal maturity in Legg-Calvé-Perthes disease. The study population consisted of 243 patients with unilateral involvement who had reached skeletal maturity at last follow-up. All hips were treated by containment methods (which included bed rest and traction in abduction, Petri cast, brace, varus osteotomy, Salter osteotomy, and shelf acetabuloplasty). Necrosis of the femoral head was estimated according to the lateral pillar classification and late results according to the Stu…

Does Adding Femoral Lengthening at the Time of Rotation Hip Transposition After Periacetabular Tumor Resection Allow for Restoration of Limb Length and Function? Interim Results of a Modified Hip Transposition Procedure.

BACKGROUND: Reconstruction after pelvic tumor resection of the acetabulum is challenging. Previous methods of hip transposition after acetabular resection have the advantages of reducing wound complications and infections of the allograft or metal endoprosthesis but were associated with substantial limb length discrepancy. We therefore developed a modification of this procedure, rotation hip transposition after femur lengthening, to address limb length, and we wished to evaluate its effectiveness in terms of complications and functional outcomes. QUESTIONS/PURPOSES: In this study, we asked: (

Surgical Treatments for Legg-Calvé-Perthes Disease: Comprehensive Review.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is a common public health problem that usually occurs between the ages of 4 and 8 years, but it can occur between the ages of 2 and 15 years. This condition occurs due to the interruption of blood supply to the femoral head. Up to now, different surgical and nonsurgical treatments, including femoral varus osteotomy, innominate osteotomy, pelvic osteotomies, triple osteotomy, Chiari osteotomy, and shelf acetabuloplasty, have been suggested for noncontainable LCPD hips. OBJECTIVE: The aim of this comprehensive review was to investigate the various s…

Multimodality Intraoperative Neurophysiological Monitoring (IONM) in Anterior Hip Arthroscopic Repair Surgeries.

Arthroscopic hip surgery is performed routinely for the treatment of various hip disorders. Leg traction during labral tear repair, femoroplasty, and acetabuloplasty for hip stabilization can stretch the peripheral nerves. This may cause temporary or permanent nerve injury. This study illustrates the benefit of utilizing multimodality Intraoperative Neurophysiological Monitoring (IONM) during hip surgical procedures. We performed a retrospective review of 10 arthroscopic hip surgeries with neurophysiological monitoring at one medical center. The patients consisted of six females and four males…

Cementless Acetabular Reconstruction for Arthropathy in Old Acetabular Fractures.

This study was conducted to identify the factors influencing mechanical failure of cementless acetabular reconstruction for arthropathy after operative treatment of acetabular fractures. Fifty-six patients (56 hips) undergoing cementless total hip arthroplasty were enrolled and followed for a mean of 120 months (range, 60-180 months). The 10-year survival rate, with mechanical failure (radiographic loosening or revision due to aseptic loosening) as the endpoint, was analyzed with respect to sex, age, body mass index (BMI), acetabular bone deficiency, sclerotic changes to the acetabulum, and us

PURPOSE: To investigate the impact of social determinants of health (SDOH) disparities on 30-day emergency department (ED) visits, 90-day postoperative complications, and 5-year secondary surgery rates after primary hip arthroscopy using a large national database. METHODS: A national administrative claims database was used to identify patients who underwent primary hip arthroscopy with femoroplasty, acetabuloplasty, and/or labral repair between 2015 and 2022.
