The Concept of Treatment-refractory Addiction: Drawing on Psychiatric Insights to Improve Addiction Research.

The concept of treatment-refractory addiction (TRA), while relatively new to the addiction field, is a well-established concept in other areas of medicine and psychiatry. Recent commentaries have highlighted the need for clearer conceptualization and definition of TRA. This commentary argues that conceptual frameworks established in other psychiatric domains (eg, depression, schizophrenia, obsessive-compulsive disorder) can and should inform the development of criteria and strategies for identifying and addressing TRA in substance use disorders (SUDs). Central to this effort is the recognition

The experience of drug-related client loss for healthcare professionals who support people in addiction.

INTRODUCTION: Addiction support and recovery is a multi-faceted support context in which practitioners work with clients who present with increased mortality risks. Drug-related deaths are understood to be a risk factor for complicated grief-reactions but, to date, research has neglected to explore the intricacies of drug-related client loss for practitioners who work with clients experiencing addiction. Due to wider expectations of professional endurance and the demanding nature of health care, grief-related reactions associated with the loss of a client may go unprocessed and, therefore, res

[Deep brain stimulation in the context of addiction--a literature-based systematic evaluation].

BACKGROUND: Deep Brain Stimulation (DBS) is established as option to treat Parkinson's Disease and essential tremor by now. Successful treatments of small samples suffering from dystonia, cluster headache or Tourette's Syndrome or from obessive-compulsive disorders and depression make a future extension of indication seem probable. In this context the efficiency of DBS for the treatment of addiction is being discussed. AIM AND METHODS: To assess the potential effects of DBS of different target areas on addiction a keyword-related research in Pubmed (National Library of Medicine, Washington) w

Treatment of hepatitis C infection in injection drug users.

Chronic hepatitis C is the most common infectious disease among injection drug users (IDUs). Because of the allegedly poor compliance of IDUs with treatment requirements and conditions, hepatologists recommend treatment only if former IDUs have spent 6 to 12 months drug free. The aim of this prospective study was to investigate whether opiate-dependent IDUs with chronic hepatitis C virus (HCV) infection can be treated successfully with interferon. Eligibility for the study meant IDUs had to be HCV-RNA positive by polymerase chain reaction. Subsequently 50 inpatients were enrolled during detoxi

[Present status and future of hypnotic drug treatment for insomnia].

Pharmacological treatments of insomnia have become safer since the first benzodiazepine receptor agonist (BzRA) hypnotic was introduced in the 1960's. Though BzRAs could hardly cause a fatal condition even in cases of overdosing, they had inherited the arguments on addiction and withdrawal from the prior studies of barbiturate hypnotics that indicated they are strongly addictive. In the 2000s, it was repeatedly demonstrated that insomnia as well as sleep deprivation underlie the development and deterioration of comorbid diseases such as hypertension, cardiovascular diseases, diabetes and depre

Assessment of knowledge and awareness among North Indian populations about oral precancerous lesions (OPL): A cross-sectional survey study.

BACKGROUND: A precancerous lesion is a morphologically altered tissue in which oral cancer is more likely to occur than its apparently normal counterpart. They are quite common in the Indian population due to the habitual habit of chewing tobacco. The aim of this study was to evaluate the awareness of oral precancerous lesions by a questionnaire-based survey among the study subjects having been diagnosed with it. METHODS: In this descriptive study, a structured questionnaire having 11 survey questions has been used to record the response from study subjects who have been diagnosed and reporte

Suicide in prison in the North of Italy (1993-2022): a case-control study examining differences between suicides inside and outside prison.

Prisoners constitute a group at suicide risk, showing higher relative rates of suicides than the general population. However, there is limited knowledge about the characteristics of those who die by suicide in Italian prisons. Based on the total sample of suicides of the Institute of Forensic Medicine of Milan (1993-2022), suicides in prison ( N  = 120) were matched by age and gender with cases that occurred outside prison ( N  = 300) and compared with them. The considered variables were sociodemographic, clinical, and suicide-related. Univariate analyses and logistic regression model were per

The Clinical Relevance of Impulsivity in Substance Use Disorder Treatment: Examining Within-Treatment Changes and Relationship to Psychiatric Symptoms and Cravings in a Large Inpatient Sample.

OBJECTIVE: Impulsivity is a multidimensional construct consistently associated with problematic substance use, but less is known about its relevance to clinical outcomes. The current study examined whether impulsivity changed over the course of addiction treatment and whether the changes were associated with changes in other clinical outcomes. METHOD: Participants were patients in a large inpatient addiction medicine program (N = 817; 71.40% male). Impulsivity was assessed using a self-report measure of delay discounting (i.e., overvaluation of smaller immediate rewards) and the UPPS-P Impuls

[Contraindications to health education in hospitalized patients in general medicine departments].

Among 408 patients discharged from a hospital unit of internal medicine (as defined in France), education appeared desirable and possible in 28%, but contraindicated in 72%. Personal care was satisfactory in 7%; stay was too short in 7%; short-term prognosis was fatal in 13%; psychic contraindications were found in 45%. This last group included impenitent alcoholics, drug addicts and tramps; other non-complaint patients; patients with learning disabilities or various mental diseases. In subjects classified as "internal medicine patients", results were identical. Other obstacles were intervals

Large-scale behavioral characterization of oxycodone self-administration in heterogeneous stock rats reveals initial analgesic effects are associated with addiction-like behaviors.

Family and twin studies indicate that 20-60% of the vulnerability to opioid use disorder (OUD) is influenced by genetic factors, but the specific genes driving addiction-like behaviors, including sensitivity to opioid analgesia, tolerance, dependence, and escalation of oxycodone self-administration, remain unidentified, limiting precision medicine approaches. To address this, we phenotyped over 500 heterogeneous stock (HS) rats, an outbred population with high genetic diversity, to characterize traits associated with OUD vulnerability and resilience. Rats self-administered oxycodone (150 µg/kg

A qualitative study of professionals' perspectives on the ethics of medically-delivered safer injection education for people who inject drugs.

BACKGROUND: In this qualitative analysis we aimed to explore addiction physicians' perspectives on safer injection education for people who inject drugs, especially: (1) on possible means of introducing safer injection education in the medical environment, (2) on the compatibility of safer injection education with each physician's core values and goals, and (3) on possible reasons for the ethical dilemma in safer injection education. METHODS: We conducted semi-structured interviews with eleven physicians practicing addiction medicine in France in clinical and harm reduction settings. RESULTS

Medical cannabis - the Canadian perspective.

Cannabis has been widely used as a medicinal agent in Eastern medicine with earliest evidence in ancient Chinese practice dating back to 2700 BC. Over time, the use of medical cannabis has been increasingly adopted by Western medicine and is thus a rapidly emerging field that all pain physicians need to be aware of. Several randomized controlled trials have shown a significant and dose-dependent relationship between neuropathic pain relief and tetrahydrocannabinol - the principal psychoactive component of cannabis. Despite this, barriers exist to use from both the patient perspective (cost, ad

Animal models for opioid addiction drug discovery.

INTRODUCTION: Since ancient times, the opium poppy has been used in a variety of settings, including pain management. Natural and synthetic derivatives of opium are commonly used in medicine today and include drugs, such as morphine, codeine, hydromorphone and oxycodone. Although excellent at inhibiting pain, these narcotics often produce a state of euphoria leading to misuse and abuse by the general population, particularly in young adults. The misuse of prescription opiates has continually increased over the past 10 years despite associated negative outcomes, resulting in opiate psychologica

The Use of Prazosin in Treatment of Drug Dreams in Adolescents with Substance Use Disorder: Two Case Reports.

~ Nightmares are psychologically distressing events that may cause significant effects on mental health, physical well-being and overall quality of life, in addition to their negative impact on quality and duration of sleep. Drug dreams are a variant of nightmares and a known phenomenon in addiction medicine, and have been studied as a potential causative factor in relapse of substance use. Recent studies have noted that addictive behaviors and drug dreams share a similar neurobiological pathway, and that certain neurotransmitters, most notably those of the noradrenergic system, may underlie t
