BACKGROUND: Cardiogenic shock is the main cause of death in hospitalized patients with acute coronary syndromes, with a high mortality rate. The management of graft thrombosis after coronary artery bypass graft (CABG) surgery is challenging and the best revascularization strategy is not well defined. In patients who develop cardiac arrest due to graft thrombosis, the benefits of mechanical support during advanced cardiac life support are uncertain. Rescue extracorporeal cardiac bypass resuscitation has been used in the context of cardiopulmonary arrest, with survival rates of around 34.7% of w

Fulminant myocarditis (FM) is a severe, rapidly progressive and life-threatening condition of myocarditis associated with infectious or autoimmune etiologies. Here, we present three patients with an invasive thymoma who had FM, which eventually led to sudden death. The most frequent clinical presentations at onset include palpitations, orthopnea, and acute worsening of myasthenia gravis (MG). Highly elevated myocardial biomarkers and positive autoantibodies against cardiac muscle, skeletal muscle, and neuromuscular junctions were detected in serum. The electrocardiograph (ECG) findings progres

The American Heart Association (AHA) revised the Pediatric Basic and Advanced Life Support guidelines in 2025 to reflect on the current evidence and best practices. The key changes include a special emphasis on post-cardiac arrest care, prognostication, and survivorship. Other major changes include the elimination of 2-finger chest compressions in infants with a recommendation of compressing the chest with the heel-of-1-hand technique or 2-thumb-encircling hands technique; the preferential and immediate use of an automated external defibrillator with a pediatric attenuator, if available, for c

Coronary vasospasm is a well-recognized cause of angina (also known as Prinzmetal angina) and a common cause of admissions to the emergency department and coronary intensive care units. It is however an uncommon cause of cardiac arrest. We describe a patient with multiple episodes of chest pain followed by cardiac arrest in pulseless electrical activity (PEA) due to coronary vasospasm. Telemetry and electrocardiography showed ST-segment elevation followed by PEA. Each event was short-lived and resolved after a maximum of six minutes of advanced life support measures. The patient was started on

OBJECTIVES: It is known that tension pneumoperitoneum (TPP) can lead to cardiopulmonary arrest but it does not figure in the advanced life support algorithms. Therefore we discuss a case of TPP together with the literature review of its aetiology and treatment.
PATIENT: We describe an out of hospital cardiac arrest due to a spontaneous TPP secondary to a perforated duodenal ulcer.
CONCLUSION: Emergency personnel should consider the possibility of TPP in a patient with a distended abdomen and a cardiac arrest, even in the absence of known traumatic and iatrogenic causes for TPP. We believe th

BACKGROUND: With increasing dissemination and improved survival after extracorporeal life support, also called extracorporeal membrane oxygenation, the decrease in readmissions after hospitalization involving extracorporeal life support is an emerging priority. The present study aimed to identify predictors of early readmission after extracorporeal life support at a national level.
METHODS: This was a retrospective cohort study using the Nationwide Readmissions Database. All patients ≥18 years who underwent extracorporeal life support from 2010 to 2015 were identified. Patients were stratifie

It is well known that electric shock can both initiate and terminate ventricular fibrillation. Refractory ventricular fibrillation (RVF) may often be an iatrogenic paradoxical result of early, frequent, excessive salvos of DC current countershocks and inappropriate off-label drug use, particularly aggressive epinephrine administration. Evidence suggests that the current advanced cardiac life support pharmacology protocol for cardiac resuscitation may contribute to disappointing survival in patients with out-of-hospital cardiac arrest. Controlled studies and new theoretical consideration sugges

INTRODUCTION: Advances in cardiology and cardiac surgery have significantly improved survival rates among children with CHD, but these gains come with the need for long-term care that heavily involves parents. Understanding their life situation is essential for delivering effective family-centred and holistic healthcare. The study aimed to assess the life situation of parents of children with CHD in relation to sociodemographic factors and to explore their sources of support, emotional experiences, and the role of spirituality in coping.

Although evaluation of the treatment of congestive heart failure is usually based on objective clinical outcomes, patient self-assessment is increasingly recognized as an important component of evaluation. A study was designed to measure the quality of life of 134 patients with symptoms of advanced heart failure who were being evaluated for possible heart transplantation. The patients' quality of life was assessed using a mix of subjective and objective measures, including functional status, physical symptoms, emotional state, and psychosocial adaptation. There was no significant relationship

Temporary mechanical circulatory support (tMCS) has evolved as a first-line treatment of cardiogenic shock refractory to medical therapy. In recent years, tMCS has been applied to bridge and precondition end-stage heart failure patients with structural heart disease for surgery or intervention. This review describes three clinical scenarios: preoperative stabilization and bridge-to-surgery (Type 1), planned intraoperative support in patients at high risk of perioperative low cardiac output (Type 2), and rescue support for failure to wean from cardiopulmonary bypass or postoperative heart failu

BACKGROUND AND OBJECTIVE: Resuscitative therapies for respiratory and cardiac failure are lifesaving and extended by using extracorporeal life support (ECLS) as mechanical circulatory support (MSC). This review informs the debate to identify the life-threatening thoracic emergencies in which patients may be cannulated for ECLS support.
METHODS: An advanced search was performed in PubMed, Embase, Google Scholar, and references query, assessed in June 2022, identified 761 records. Among them, 74 publications in English were included in the current narrative review.

Cardiopulmonary arrest (CPA) is defined as the abrupt and unexpected cessation of spontaneous and effective ventilation and circulation. CPA can be the natural ending of a normal and long life; however, when CPA is the result of a reversible problem in an animal that has a treatable medical condition, rapid recognition and treatment may make the difference between a happy ending and premature death. Cardiopulmonary resuscitation provides artificial ventilation and circulation until advanced cardiac life support can be provided and spontaneous cardiopulmonary function is restored. The term card

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) remains a major public health and emergency medical challenge worldwide, with generally poor survival outcomes. The quality of cardiopulmonary resuscitation (CPR) provided by prehospital emergency personnel plays a critical role in improving outcomes. This study aimed to assess basic and advanced CPR skills among operational staff of the Kerman Emergency and Incident Management Center using a clinical simulation-based approach.
METHODS: This descriptive cross-sectional study was conducted among 267 operational personnel, including emergency me

Cardiac rehabilitation is strongly recommended in secondary cardiovascular prevention. In patients after coronary artery bypass grafting, this intervention is suggested to reduce mortality, morbidity, and disability. In addition, rehabilitation programs improve quality of life and cardiorespiratory fitness. Modern cardiac rehabilitation programs include structured exercise training, education, nutritional counseling, psychosocial support, and management of cardiovascular risk factors, each tailored to the specific needs of post-coronary artery bypass grafting patients who often face a high bur

Acute ST-elevation myocardial infarction (STEMI) is a life-threatening condition often associated with significant cardiac complications, particularly in the presence of underlying multivessel coronary artery disease. Mechanical complications, such as acute mitral regurgitation (MR), can worsen the clinical course, leading to rapid hemodynamic deterioration. Recent advancements in mechanical circulatory support and percutaneous interventions have introduced new therapeutic options, offering viable alternatives to traditional surgery for high-risk patients. In this case, a 67-year-old male with

BACKGROUND: Cardiopulmonary resuscitation (CPR) in special circumstances includes the emergency intervention for special causes, special environments, and special patients. Special causes cover the potential reversible causes of cardiac arrest that must be identified or excluded during any resuscitation act. The special environments section includes recommendations for the treatment of cardiac arrest occurring in specific locations: cardiac surgery, catheterization laboratory, dialysis unit, dental surgery, commercial airplanes or air ambulances, playing field, difficult environment (eg, drown

OBJECTIVES: It remains unclear whether ST-elevation myocardial infarction (STEMI) patients transported by ambulance over long distances are at risk for clinical adverse events. We sought to determine the frequency of clinical adverse events in a rural population of STEMI patients and to evaluate the impact of transport time on the occurrence of these events in the presence of basic life support paramedics.
METHODS: We performed a health records review of 880 consecutive STEMI patients transported to a percutaneous coronary intervention centre. Patients had continuous electrocardiogram and vit

The extreme medical emergency situation in the dental setting is cardiac arrest. The need to provide dental treatment to the medically compromised patients, suffering from very high risk heart diseases at special oral medicine hospital dentistry units, expose the dental and medical teams to the possibility of patients' death. Cardiac and cardiorespiratory arrest in these units faces the dentists with the need to perform basic and/or advanced cardiopulmonary resuscitation (CPR). Various etiologies are responsible for cardiac arrests. This article describes our experience and the outcome of six

Bone cement implantation syndrome (BCIS) is a rare but potentially fatal complication of cemented orthopedic procedures, most commonly hip hemiarthroplasty in elderly patients. It is characterized by acute cardiovascular and respiratory compromise that can progress to cardiac arrest. We present the case of a 90-year-old woman with long-standing hypertension who sustained a left intertrochanteric femoral fracture following a fall. She was scheduled for cemented hemiarthroplasty and underwent spinal anesthesia. Approximately five minutes after insertion of polymethylmethacrylate bone cement, she
