CONTEXT: Type 2 diabetes is a growing global concern with serious complications, including kidney damage and cardiovascular morbidity and mortality. Monitoring albuminuria, which is associated with these complications, is crucial in optimal diabetes management. Gut microbiota composition has been suggested to impact albuminuria, but large studies with granular data are lacking.
METHODS: We investigated the relationship between 1002 gut microbial species, 1308 plasma metabolites, and albuminuria in 752 participants with type 2 diabetes from the Swedish CArdioPulmonary BioImage Study (SCAPIS).

IMPORTANCE: Sodium-glucose cotransporter 2 (SGLT2) inhibitors reduce chronic kidney disease (CKD) progression in individuals with type 2 diabetes, CKD, or heart failure. However, their effects in those with stage 4 CKD or little to no albuminuria remain uncertain.
OBJECTIVE: To assess whether estimated glomerular filtration rate (eGFR) or degree of albuminuria, measured by urinary albumin to creatinine ratio (UACR), modifies the effects of SGLT2 inhibitors on kidney outcomes.
DATA SOURCES: SGLT2 inhibitor trials participating in the SGLT2 Inhibitor Meta-Analysis Cardio-Renal Trialists' Conso

Obesity-related renal dysfunction is largely attributed to hemodynamic alterations and insulin resistance. Among individuals with overweight or obesity, albuminuria has been shown to be independently and inversely associated with circulating adiponectin levels in the absence of diabetes. Accordingly, this study aimed to evaluate and quantify the independent association between obesity and the risk of kidney disease, as assessed by the presence of albuminuria in the Asian Population. This systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and

Albuminuria is routinely used to assess kidney damage in diabetes, but it also conveys cardiovascular risk across the urinary albumin-to-creatinine ratio (UACR) distribution, including within A1. Whether sex modifies this relationship remains uncertain. This narrative review prioritizes longitudinal studies in type 2 diabetes mellitus (T2DM), analyses within A1 particularly, and distinguishes this direct evidence from the general population or chronic kidney disease cohorts, surrogate-endpoint studies, and experimental mechanisms. A few studies report formal sex-by-UACR interactions for select

The high disease burden and bidirectional relationship of chronic kidney disease (CKD), heart failure (HF) and other cardiovascular disease (CVD) necessitate the need for early diagnosis of these diseases. While current screening and detection methods are recommended by CKD and CVD guidelines, their adoption in practice is low. Urine albumin-to-creatinine ratio (uACR) is recognized as a diagnostic marker for CKD and a prognostic marker for CKD progression, HF and CVD outcomes, therefore albuminuria changes have been accepted as a surrogate outcome for kidney and cardiovascular endpoints. Background and Objectives: Heart failure (HF) and chronic kidney disease (CKD) frequently coexist and are closely interrelated, significantly affecting clinical outcomes. Among CKD-related markers, albuminuria and estimated glomerular filtration rate (eGFR) have emerged as key prognostic indicators in HF. However, their specific predictive value across different HF phenotypes-namely HF with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF)-remains incompletely understood. This systematic review aims to evaluate the prognostic significance of albuminuria and eGFR

Diabetes mellitus and chronic kidney disease (CKD) are prevalent conditions affecting a significant portion of the Australian population. Urinary albumin testing, particularly through the urine albumin-to-creatinine ratio (uACR), is essential for screening, prognostication, and guiding treatment in patients with diabetes and CKD. Despite recommendations for annual screening among high-risk populations such as those with diabetes, recent studies indicate disturbingly low rates of albuminuria testing. This narrative literature review aims to examine rates of uACR testing in these populations, fo

A 70-year-old woman was diagnosed with multiple myeloma in 2014. She achieved complete remission (CR) after bortezomib, cyclophosphamide, dexamethasone (VCD) therapy and lenalidomide, dexamethasone (Rd) therapy; however, she relapsed in 2017. Although she achieved second CR by carfilzomib, dexamethasone (Kd) therapy, serum creatinine levels increased with urinary protein after 17 courses of Kd therapy. Urinary protein test revealed albuminuria, whereas M-protein was undetectable. Carfilzomib-induced renal impairment was suspected due to absence of other causes, such as progression of myeloma o

OBJECTIVES: Inhibitors of the renin-angiotensin system are recommended for the management of albuminuria in patients with hypertension and diabetes mellitus, but there is little consensus about alternative therapies. Calcium channel blockers are recommended for the management of hypertension, but the data are controversial regarding their role in patients with albuminuria. This review was designed to assess the efficacy of calcium channel blockers compared with inhibitors of the renin-angiotensin system in decreasing albuminuria in diabetic, hypertensive patients with nephropathy.

Albuminuria in nephrotic states is thought to arise from the formation of large pores in the glomerular capillary wall as large hydrodynamic probes, like Ficoll, have increased fractional clearance. In the present study, we tested for large pore formation in a novel manner. We accounted for the rates of plasma elimination as determined for tritium-labeled tracers of uncharged polydisperse Ficoll (radii range: 35-85 Å) and two globular (14)C-labeled proteins, albumin (radius: 36 Å) and IgG (radius: 55 Å), in control and puromycin aminonucleoside nephrotic (PAN) Sprague-Dawley rats. To evaluate the impact of systemic blood pressure and albuminuria on the progression of diabetic nephropathy, we followed 41 IDDM patients with persistent albuminuria (> 300 mg/24 h) by measuring glomerular filtration rate (51Cr-EDTA technique), blood pressure, and albuminuria. None of the patients were taking drugs other than insulin. Arterial blood pressure, albuminuria, and blood glucose were measured four to eight times/yr, whereas glomerular filtration rate was determined twice yearly. During the median investigation period of 36 (15-66) mo, glomerular filtration rate decreased from 102 +

BACKGROUND: Sodium-glucose co-transporter 2 (SGLT2) inhibitors reduce CKD progression in adults; but pediatric evidence remains limited. We evaluated dapagliflozin in children with non-diabetic CKD.
METHODS: In this multicenter, randomized, open-label, endpoint-blinded controlled trial, 114 children with CKD stages 1-4 were assigned to dapagliflozin plus standard care (n=58) or standard care alone (n=56) for 4 months. Primary outcomes were changes in estimated glomerular filtration rate (eGFR) and urine albumin-to-creatinine ratio (UACR), analyzed using ANCOVA adjusted for baseline values.

Albuminuria reflects systemic endothelial dysfunction, but its relationships with subclinical brain abnormalities have not been comprehensively catalogued. The Cardiovascular Health Study recruited older adults from four US communities, beginning in 1989-1990. Systematic measurements of albuminuria were performed in 1996-1997; two brain MRIs, in 1992-1994 and 1997-1999; and serum neurofilament light chain (NfL) measurements from 1996-1997 stored samples. We examined the associations of albuminuria with longitudinal progression of white matter hyperintensities (WMH) and ventricular size, incide

BACKGROUND: Guidelines currently recommend annual screening for albuminuria only among persons with diabetes mellitus (DM). There is no guidance about albuminuria screening in those with other important risk factors for chronic kidney disease (CKD), such as hypertension and/or family history of kidney disease. We sought to create a risk score that predicts the likelihood of albuminuria in adults without diabetes to prompt earlier detection and management of CKD.
METHODS: Data from 44,322 participants without diabetes, aged 18 + years from the National Health and Nutrition Examination Surveys

INTRODUCTION: Kidney disease is common after pediatric heart transplantation. Serum creatinine-based glomerular filtration rate is the most frequently reported measure of kidney function. Albuminuria is an additional marker of kidney dysfunction and is not well described in this population. In this study, we evaluate the prevalence and degree of albuminuria and describe clinical factors associated with albuminuria in a cohort of pediatric heart transplant recipients.
METHODS: This was a cross-sectional study of pediatric heart transplant recipients. Albuminuria was assessed using spot urine a

Environmental lead exposure has been linked with reduced kidney function. However, evidence about its role in diabetic kidney damage, especially when considering the nutritional status of vitamin D, is sparse. In this observational study, we investigated the association between low-level lead exposure and urinary albumin-to-creatinine ratio (UACR) and assessed potential impact of vitamin D among 4033 diabetic patients in Shanghai, China. Whole blood lead was measured by graphite furnace atomic absorption spectrometry. Serum 25-hydroxyvitamin D [25(OH)D] was tested using a chemiluminescence imm
