[Perioperative management of transthoracic oesophagectomies : Fundamentals of interdisciplinary care and new approaches to accelerated recovery after surgery].

Locally advanced carcinomas of the oesophagus require multimodal treatment. The core element of curative therapy is transthoracic en bloc oesophagectomy, which is the standard procedure carried out in most specialized centres. Reconstruction of intestinal continuity is usually achieved with a gastric sleeve, which is anastomosed either intrathoracically or cervically to the remaining oesophagus. This thoraco-abdominal operation is associated with significant postoperative morbidity, not least because of a vast array of pre-existing illnesses in the surgical patient. For an optimal outcome, the

Heterophoria as a measure of subjects' alertness during recovery from general anesthesia.

The purpose of the study was to measure the influence of subjects' alertness on heterophoria. An experimental group of 43 patients was measured with the Maddox Wing test for heterophoria. Exophoria was greater 1 1/2, 2 1/2, and 3 1/2 hr. after the end of general anesthesia than at baseline level. Data for a control group of 25 did not vary on the Maddox Wing over four hours. The baseline level was not different between the two groups, so heterophoria as measured with the Maddox Wing can be used to evaluate subjects' alertness during recovery from general anesthesia.

Patient quality of recovery on the day of surgery after propofol total intravenous anesthesia for vitrectomy: A randomized controlled trial.

BACKGROUND: Vitrectomy under general anesthesia is considered as a candidate for ambulatory surgery. An anesthetic method with high quality of postoperative recovery should be selected for successful ambulatory surgery. We thus compared quality of postoperative recovery on the day of vitrectomy using the Quality of Recovery (QoR)-40 questionnaire between propofol total intravenous anesthesia (propofol group) and desflurane inhalation anesthesia (desflurane group) as the 2 representative anesthetic methods. METHODS: Eighty-four patients (20-80 years old) undergoing elective vitrectomy under ge

Use of therapeutic caffeine in acute care postoperative and critical care settings: a scoping review.

BACKGROUND: Caffeine is the most utilised psychoactive drug worldwide. However, caffeine withdrawal and the therapeutic use of caffeine in intensive care and in the perioperative period have not been well summarised. Our objective was to conduct a scoping review of caffeine withdrawal and use in the intensive care unit (ICU) and postoperative patients. METHODS: PubMed, Embase, CINAHL Complete, Scopus and Web of Science were systematically searched for studies investigating the effects of caffeine withdrawal or administration in ICU patients and in the perioperative period. Areas of recent sys

Routine flumazenil administration confounds emergence time comparisons between remimazolam and propofol: a methodological concern.

This letter to the editor identifies a critical methodological concern in a recent clinical trial by Qin et al. that compared the emergence profiles of remimazolam and propofol in pediatric anesthesia. We argue that the study's findings are confounded by a fundamental"asymmetry in interventions". The remimazolam group routinely received flumazenil for pharmacological reversal, while the propofol group emerged spontaneously via natural metabolism.

Comparison of a new EMG module, AF-201P, with acceleromyography using the post-tetanic counts during rocuronium-induced deep neuromuscular block: a prospective, multicenter study.

Recent advances in neuromuscular monitors have facilitated the development of a new electromyographic module, AF-201P™. The purpose of this study was to investigate the relationship between post-tetanic counts (PTCs) assessed using the AF-201P™ and the acceleromyographic TOF Watch SX™ during rocuronium-induced deep neuromuscular block. Forty adult patients consented to participate in this study. The integrated AF-201P™ stimulating and sensing electrode was placed over the ulnar nerve on the distal volar forearm and the belly of the abductor digiti minimi muscle of one arm. The TOF Watch SX™ wa

Sugammadex vs neostigmine in post-anesthesia recovery: A systematic review and meta-analysis.

Residual neuromuscular blockade (RNB) is linked to an increased risk of perioperative adverse events. This study systematically evaluates the impact of neuromuscular blockade antagonists on postoperative complications and quality of recovery in surgical patients. We conducted a systematic review and meta-analysis to compare the efficacy of sugammadex and neostigmine. Comprehensive searches were performed across medical databases, including Web of Science, PubMed, Embase, and the Cochrane Library, with a final search date of April 6, 2025. A total of thirty-five randomized controlled trials (RC

Systematic review with meta-analysis of relative risk of prolonged times to tracheal extubation with desflurane versus sevoflurane or isoflurane.

The objective of this systematic review was to estimate the relative risk of prolonged times to tracheal extubation with desflurane versus sevoflurane or isoflurane. Prolonged times are defined as ≥15 min from end of surgery (or anesthetic discontinuation) to extubation in the operating room. They are associated with reintubations, naloxone and flumazenil administration, longer times from procedure end to operating room exit, greater differences between actual and scheduled operating room times, longer times from operating room exit to next case start, longer durations of the workday, and more

Clinical Effects and Adverse Events Associated With Desflurane Use in Adult Patients Undergoing Supratentorial Craniotomy: A Systematic Review.

Desflurane is an inhalational anesthetic agent with an appealing recovery profile. The present systematic review investigates the clinical effects and adverse events associated with desflurane use during supratentorial craniotomy for brain tumor resection in adults in comparison with other inhalational and intravenous anesthetic agents. A literature search was conducted across the MEDLINE, Library of Congress and LISTA (EBSCO) databases from January 2001 to January 2021. Twelve studies published between 2003 and 2020 were included in this systematic review. Desflurane was compared with either

Adverse respiratory events with sevoflurane compared with desflurane in ambulatory surgery: A systematic review and meta-analysis.

BACKGROUND: An increasing number of studies have concluded that the number of adverse events in the upper airway caused by desflurane does not differ significantly from the number of adverse events caused by sevoflurane. The advantages of desflurane in ambulatory surgery should be reassessed. OBJECTIVES: The aim of this study was to compare adverse respiratory events and recovery outcomes in patients undergoing desflurane or sevoflurane-based anaesthesia in ambulatory surgery. DESIGN: A systematic review and meta-analysis of randomised controlled trials (RCTs). DATA SOURCES: A systematic se

INTRAMUSCULAR EPINEPHRINE RESULTS IN REDUCED ANESTHETIC RECOVERY TIME IN AMERICAN ALLIGATORS (ALLIGATOR MISSISSIPPIENSIS) UNDERGOING ISOFLURANE ANESTHESIA.

Inhalants are commonly used to anesthetize reptiles, but volatile anesthetics have been associated with prolonged recovery times. The objective of this study was to determine the effects of intramuscular (IM) epinephrine on anesthetic recovery times following isoflurane anesthesia in a population of subadult American alligators ( Alligator mississippiensis ). In this prospective randomized crossover study, five clinically healthy alligators were anesthetized for 90 min with the use of isoflurane. Alligators were randomly assigned into one of two treatment groups: Group E received IM epinephrin

Recovery characteristics of patients receiving either sugammadex or neostigmine and glycopyrrolate for reversal of neuromuscular block: a randomised controlled trial.

Sugammadex more rapidly and reliably reverses rocuronium-induced neuromuscular block compared with neostigmine, but it is not known if subsequent patient outcomes, including nausea, vomiting and other aspects of recovery are modified. In this study, we compared the recovery characteristics of sugammadex and neostigmine/glycopyrrolate following reversal of neuromuscular block. This was a single-centre, randomised, blinded, parallel-group clinical trial in women undergoing elective day-surgical laparoscopic gynaecological surgery, with a standardised general anaesthesia regimen that included roc

Effect of intraoperative analgesic therapy on end-expired concentrations of halothane associated with spontaneous eye opening in children.

We studied 94 healthy ASA physical status I or II children to determine the end-expired concentration of halothane associated with eye opening on emergence from anesthesia, and to determine if parenteral opioid therapy or regional analgesia significantly altered this concentration. In our study, anesthesia was maintained with halothane in an air-oxygen mixture. After the surgical procedure was completed, the inspired concentration of halothane was adjusted to zero and the end-expired concentrations were permitted to decrease spontaneously. The end-expired concentration at which the child spont
