Familial primary progressive aphasia.

Primary progressive aphasia (PPA) is a neurodegenerative disease presenting with isolated, progressive, language dysfunction. After at least 2 years, dementia may develop, but the aphasia predominates. Few families with hereditary PPA have been reported; some have autosomal dominance. A chromosome 17 mutation in tau exon 13 has been found in one family, and a few have linkage to chromosome 17. However, early appearance of prominent memory, behavior, and motor impairments differentiates these patients from typical PPA. The objective was to report clinical features, pathology, and genetic analys

Primary progressive semantic aphasia: a case study.

A longitudinal case study of a patient with a progressive loss of meaning of objects with preserved phonology and syntax is presented. Repeated measures of language, praxis, visual cognition, and semantic processing were carried out. The patient still has preserved conversational speech, social skills, and orientation in her 8th year of her illness, but shows severe anomia and comprehension deficit in all modalities of stimulus presentation. In addition to standardized tests of language, cognition, and memory, specific experiments of categorization, modalities of word access, item consistency,

An advancement of intraoperative language mapping in awake craniotomy.

Direct electrical stimulation (DES) during awake craniotomy is the gold-standard for language mapping; however, conventional tasks (e.g. counting, object naming, reading) provide limited sensitivity to grammatical and sentence-level processes. The present study introduces the first intraoperative application of the English Verb and Noun Test for Perioperative testing (VAN-POP), a sentence-based paradigm designed to probe object and action naming with finite verbs in past and present tense. VAN-POP was administered to four UK English-speaking patients undergoing awake craniotomy for WHO grade I

Akinetic mutism without a structural prefrontal lesion.

Akinetic mutism is characterized by profound apathy and a lack of verbal and motor output for action, despite preserved alertness. The condition usually follows bilateral damage to the medial frontal subcortical circuits. We report a 59-year-old right-handed woman who was admitted to the neurology ward with sudden-onset akinetic mutism. Her medical history included an ischemic stroke 3 years earlier, with residual anomia and mild agraphia but no motor dysfunction. On this admission, a cranial computed tomography scan disclosed an acute left superior cerebellar infarction embracing the vermis,

[A patient with amnesic syndrome with defective route finding due to left posterior cerebral artery territory infarction].

We report a right-handed 67-year-old woman with an infarction in the left posterior cerebral artery territory presenting amnesic syndrome, right homonymous hemianopsia, pure alexia, color anomia, and defective route finding. The patient often walked in wrong directions out of her hospital room as well as in her home. She was able to recognize her own house and nearby streets by looking at them. Prosopagnosia and constructional impairment was not observed. Wechsler memory scale revised (WMS-R) revealed that she had marked disturbance in both visual and verbal recent memory. Brain MRI revealed a

Dementia with leukoencephalopathy in systemic lupus erythematosus.

Neurologic manifestations, afflicting up to 70% of SLE patients, include psychosis, seizures, chorea, neuropathies, and stroke. MRI is useful in evaluation of lupus patients and several reports have documented cerebral atrophy or focal hyperintensities. We report an unusual MRI appearance in a 56-year-old woman with SLE, diagnosed on the basis of pleuritis, lymphopenia, anti-DNA antibodies, and neurologic involvement. She reported recent onset of Raynaud's phenomenon and generalized macular rash. She presented after two months of gradual deterioration with memory loss, flattened affect, dyspha

Posterior cortical dementia with alexia: neurobehavioural, MRI, and PET findings.

A progressive disorder of relatively focal but asymmetric biposterior dysfunction is described in a 54 year old right handed male. Initial clinical features included letter-by-letter alexia, visual anomia, acalculia, mild agraphia, constructional apraxia, and visuospatial compromise. Serial testing demonstrated relentless deterioration with additional development of transcortical sensory aphasia, Gerstmann's tetrad, and severe visuoperceptual impairment. Amnesia was not an early clinical feature. Judgment, personality, insight, and awareness remained preserved throughout most of the clinical c

Multimodal semantic battery to monitor progressive loss of concepts in the semantic variant of primary progressive aphasia (svPPA): an innovative proposal.

Semantic variant primary progressive aphasia (svPPA) is a rare neurodegenerative disease characterized by a progressive loss of semantic knowledge. Patients with svPPA show anomia, impaired word comprehension, poor object recognition, and difficulties in retrieving semantic information. svPPA is also a unique "natural" model that allows clinicians and cognitive neuroscientists to study the organization of semantic memory because only semantic knowledge is affected in the initial period of the disease, with relative sparing of other cognitive domains. In the clinical practice, semantic memory i

Evidence for a functional specialization of ventral anterior temporal lobe for language.

The controlled semantic cognition framework proposes that the ventral anterior temporal lobes (vATL) in the left and right hemisphere function as an integrated hub region supporting transmodal semantic representations. The clinical evidence for the transmodal function of vATL is largely based on studies of semantic dementia patients with severe anomia, who also show impaired performance on nonverbal tasks that involve the retrieval of knowledge about objects and their prototypical use, such as the production of tool use pantomimes. Yet, evidence from patients with apraxia and functional neuroi

e report the case of 48-year-old right-handed male patient with an infarction affecting most part of the body and the splenium of the left half of the corpus callosum. Neuropsychological examination revealed typical signs of callosal disconnection including left-sided apraxia, diagonistic dyspraxia, left-sided agraphia, left-hand tactile anomia, left hemialexia, and right-sided constructional disability. Moreover, he complained of impairment in activities involving the right hand disability and agraphia. He could not stop behaving with his right hand when he had a vague idea. For example, he i

Deterioration of visual-perceptual organization ability in Alzheimer's disease.

Although deterioration of higher-order visual information processing abilities occurs in Alzheimer's disease (AD), few cross-sectional or longitudinal studies have systematically examined this deficit. The performance of 135 patients with probable AD and 97 matched normal control (NC) participants were compared on a structured test of perceptual organization ability, the Hooper Visual Organization Test (VOT). Both the standard VOT score and a derived score that corrected for anomia were significantly lower for AD patients than for NC participants, but neither score was particularly effective a

Training-induced brain remapping in chronic aphasia: a pilot study.

BACKGROUND: The neural correlates of training-induced improvements of cognitive functions after brain damage remain still scarcely understood. In the specific case of aphasia, although several investigations have addressed the issue of the neural substrates of functional recovery, only a few studies have attempted to assess the impact of language training on the damaged brain. AIMS: The main goal of this study was to examine the neurobiological correlates of improved picture-naming performance in 2 aphasic patients who received intensive and specific training for a chronic and severe phonolog

Surgical approaches to tumours of the lateral ventricles in the dominant hemisphere.

BACKGROUND: Intraventricular neoplasms are rare tumours (1% of the tumours of the central nervous system). The surgical approach sometimes is quite difficult, particularly in the dominant hemisphere. The best choice of surgical approach is discussed. METHODS: The authors describe a series of 25 patients who underwent surgical treatment for tumours situated in the lateral ventricles of the dominant hemisphere. They discuss the most influential factors in the choice of surgical approach, which must allow maximum exposure of the lesion and vascular feeding branches without damaging highly functi

[Dysconnection syndrome in a boy with tumorous lesion of corpus callosum].

We report on a 12-years-old boy with glioma of the septum pellucidum which invaded and infiltrated the adjacent structures including neocortical commissures. Neuropsychological evaluation revealed the loss of interhemispheric integration and hemisphere dysconnection syndrome. Our patient demonstrated left-sided ideo-motor apraxia to verbal commands and left-sided agrafia, being unable to copy, write to dictation and formulate his own sentences with the left hand. He partly failed in attempts to name or describe objects out of sight in the left hand and showing an unilateral tactile anomia. The

Inside Semantic Feature Analysis: A Within-Trial Analysis of Feature Quantity and Quality.

PURPOSE: Aphasia rehabilitation increasingly emphasizes the importance of understanding the mechanisms and ingredients underlying behavioral interventions. Semantic feature analysis (SFA) is a commonly used intervention for anomia in aphasia, but there is little evidence directly examining its active ingredients. Using within-trial responses during SFA, we sought to explore how generating semantic features might facilitate naming improvements. METHOD: A retrospective analysis evaluated data collected from a clinical trial focused on intensive SFA treatment for individuals with chronic aphasia

A culturally grounded Arabic naming task differentiates Saudi Arabic-speaking adults with post-stroke aphasia from neurotypical adults.

BACKGROUND/OBJECTIVES: Anomia is a common linguistic consequence of post-stroke aphasia. Widely used confrontation-naming measures were developed primarily in English-speaking populations, and item familiarity may not transfer to Arabic speakers. We developed the Arabic Naming Test (ANT) as a culturally grounded picture-naming task and examined its preliminary clinical feasibility in Saudi Arabic-speaking adults. METHODS: In this cross-sectional convenience-sample study, 75 neurotypical native Arabic-speaking adults and 30 adults with post-stroke aphasia completed a 90-item picture-confrontat

Semantic memory: Which side are you on?

We present two patients in whom the combination of lesion site and cognitive performance was uniquely informative about the organisation and functional anatomy of semantic memory. One had had a single lobar stroke with an unusual distribution, largely destroying the whole of the left temporal lobe ventral to the superior temporal sulcus. The other patient had had herpes simplex encephalitis with destruction that was confined to the left cerebral hemisphere. The lesion again mainly encompassed the left temporal lobe, but also extended to the left inferior frontal gyrus. Cognitive outcomes in th

Neurodevelopmental characteristics of children with learning impairments classified according to the double-deficit hypothesis.

The double-deficit model has been examined primarily in relation to reading. We investigated whether children classified according to the double-deficit model would exhibit differences in other neuropsychological domains. Children referred for learning problems (N = 188), ages 7 to 11, were classified by double-deficit subtype. Only three of the four groups predicted by the model could be identified. There were no group differences in IQ or attention problems. The three groups showed different neuropsychological profiles, involving functional domains other than reading and language. Difference

Pure alexia and hemianopia related to dissection of the internal carotid artery.

This review summarises the present knowledge of infliximab therapy in children with inflammatory bowel disease (IBD) based on the available published literature. Infliximab, the chimeric monoclonal IgG(1) antibody to tumour necrosis factor-alpha, is indicated for medically refractory luminal and fistulising paediatric Crohn's disease. Recently, ulcerative colitis case series in children and adolescents suggested that infliximab might also be effective for treatment of ulcerative colitis resistant to standard medical therapy. Induction therapy with infliximab 5 mg/kg at weeks 0, 2 and 6 is rout

Optic aphasia and the right hemisphere: a replication and extension.

Optic aphasia is a rare, visual modality-specific naming disorder. We describe a patient who developed this disorder subsequent to a left occipital lobe infarction. Like another case that we described previously, this patient performed normally on a variety of tasks assessing the recognition of objects he could not name. Additionally, although he never read aloud a single word, his performance on lexical decision and word comprehension tasks was far better than chance. We suggest that his performance was mediated by a right hemisphere semantic system.
