Early Response to an Integrated Family and Adolescent-Focussed Treatment for Anorexia Nervosa and Atypical Anorexia Nervosa.

OBJECTIVE: Recovery rates for adolescent anorexia nervosa (AN) remain poor, requiring creativity within the field to explore alternative treatment options. One novel, evidence-informed approach integrates both family/parent and individual adolescent-focussed treatment components, through an individualised developmental lens. This study aimed to offer preliminary insights on early response to Adolescent and Parent Treatment (APT) in terms of 8-week weight trajectory. METHOD: A naturalistic, repeated measures design was employed. Twenty-four adolescents with AN or atypical AN receiving APT as s

Moving Beyond BMI: The Need for Biomarkers of Physiological Recovery in Anorexia Nervosa and Atypical Anorexia Nervosa.

OBJECTIVE: In the treatment of individuals with atypical anorexia nervosa (atypical AN), it is unclear how to determine when sufficient renourishment has been achieved. Reliance on body mass index (BMI) to define acute treatment goals in AN has led to gaps in understanding of the physiology of renourishment. Biological markers that reflect restoration of physical health during acute nutritional rehabilitation are needed to move beyond weight-based metrics. METHOD: This Spotlight briefly describes the physiology of starvation in patients with AN and atypical AN and existing strategies for acut

Association of Emotion Regulation Trajectories in Childhood With Anorexia Nervosa and Atypical Anorexia Nervosa in Early Adolescence.

IMPORTANCE: People with anorexia nervosa often experience difficulties regulating their emotions. There is no longitudinal evidence as to whether these differences are already present in childhood or when they begin to emerge. OBJECTIVE: To investigate the association between emotion regulation trajectories from 3 to 7 years of age and symptoms of anorexia nervosa and atypical anorexia nervosa in adolescence. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included all children with complete exposure data in the Millennium Cohort Study, a UK general population birth cohort. Data were ac

Anorexia Nervosa and a Lost Emotional Self: A Psychological Formulation of the Development, Maintenance, and Treatment of Anorexia Nervosa.

In this paper, we argue that Anorexia Nervosa (AN) can be explained as arising from a 'lost sense of emotional self.' We begin by briefly reviewing evidence accumulated to date supporting the consensus that a complex range of genetic, biological, psychological, and socio-environmental risk and maintenance factors contribute to the development and maintenance of AN. We consider how current interventions seek to tackle these factors in psychotherapy and potential limitations. We then propose our theory that many risk and maintenance factors may be unified by an underpinning explanation of emotio

What Amount of Weight Loss Can Entail Anorexia Nervosa or Atypical Anorexia Nervosa After Bariatric Surgery?

OBJECTIVE: Post-operative development of restrictive eating disorders can occur in patients after bariatric surgery. In children and adolescents with anorexia nervosa (AN) or atypical AN, premorbid body mass index (BMI) has recently been shown to predict total weight loss. We hypothesized that pre-operative BMI similarly predicts weight loss and the development of a restrictive eating disorder in adult bariatric patients. METHOD: A PubMed search identified case studies/series of 29 adult females who developed AN or atypical AN/eating disorder not otherwise specified following bariatric surger

[The spiritual dimensions of anorexia nervosa].

The historical roots of anorexia nervosa are closely linked to religious ascetic traditions. During the Middle Ages, numerous "fasting saints," including Catherine of Siena and Saint Margaret of Hungary, regarded prolonged fasting as a path to spiritual purification, self-mastery, and union with God. The denial of bodily needs symbolized holiness, moral purity, and transcendence. In modern secular societies, however, these meanings have not disappeared but have been transformed. Several scholars argue that the contemporary cult of thinness functions as a quasi-religious ideology in which weigh

Endocrinology of Underweight and Anorexia Nervosa.

More than any other mental illness, the course, prognosis, and therapy of anorexia nervosa are shaped by the physical changes associated with being underweight. This article provides an overview of the endocrine changes associated with malnutrition and underweight. This overview serves as a basis for understanding the other articles in this special issue, which deal with the health risks associated with being underweight. In this context, the differences between underweight in anorexia nervosa and in constitutional thinness are of particular importance in assessing the impact of intentional we

[Anorexia nervosa and the perinatal period].

As a complex pathology, anorexia nervosa interacts with the psychological challenges of pregnancy, and raises questions about how to support the women concerned. As part of a multidisciplinary approach, support groups are particularly important, offering both individual and social support. Child-parent drop-in centers can also provide a supportive space for the psychological work of parenting.

Anorexia nervosa and the gastrointestinal tract.

Anorexia nervosa (AN) is a complex eating disorder associated with a high morbidity and mortality, however, there is a lack of dedicated training for healthcare professionals outside of mental health specialities. There has been a reported increase in acute admissions of patients with AN, which may have been precipitated by the isolation and loss of support networks created by the COVID-19 pandemic. The purpose of this review is to highlight that AN can present with a wide variety of signs and symptoms relating to both the hollow and solid organs of the gastrointestinal (GI) tract some of whic

Stigmatization and knowledge of anorexia nervosa.

PURPOSE: To further understand the stigma, attitudes towards, and recognition of Anorexia Nervosa (AN), the current study utilized both stigma and mental health literacy methodology. This study also included another psychological disorder, major depressive disorder (MDD) and a normal, lonely female (C) as comparison conditions. METHODS: Male (291) and female (576) undergraduate students read a vignette describing one of the conditions. Participants then rated the vignettes on general personality characteristics, condition recognition, perceived seriousness and need for treatment. RESULTS: Ma

Managing anorexia nervosa in the community.

This article describes the development and evaluation of a new specialist intensive community-based service for adults (those aged 18 years and above) with severe and enduring eating disorders (SEEDs). The service was developed by the Yorkshire Centre for Eating Disorders based in Leeds. We developed and evaluated a nurse-delivered community-based service that aimed to manage the complex needs of people with SEEDs without hospital admission. The service was shown to improve quality of life, reduce the number of hospital admissions and improve communication with other health professionals invol

The significance of bradycardia in anorexia nervosa.

Anorexia nervosa (AN) is a life-threatening condition, with a significant risk for death, due to cardiovascular complications. It is characterized by abnormal eating behavior and has the highest mortality rate of all psychiatric disorders. It has been associated with bradycardia (a heart rate [HR] of less than 60 beats per minute) (up to 95%), hypotension, mitral valve prolapse, and heart failure. The diagnosis of AN can be elusive, and more than half of all cases are undetected. The purpose of this study was to raise and improve awareness to the possible diagnosis of AN in adolescent and youn

[Epidemiology of anorexia nervosa: a review].

UNLABELLED: A critical review of the literature was conducted to provide answers to three questions: firstly, are we facing a national and/or international epidemic of anorexia nervosa (AN) as claimed in the media by certain professionals? Secondly, is AN simply an ordinary crisis of adolescence? Thirdly, is it a pathology that solely affects the higher socio-professional categories? METHOD: A Medline search was conducted and backed up by a manual search to find all the studies or literature reviews published on prevalence, incidence rates, outcomes, morbidity and mortality in AN and its link

The clinical biochemistry of anorexia nervosa.

In anorexia nervosa, under-nutrition and weight regulatory behaviours such as vomiting and laxative abuse can lead to a range of biochemical problems. Hypokalaemia is the most common electrolyte abnormality. Metabolic alkalosis occurs in patients who vomit or abuse diuretics and acidosis in those misusing laxatives. Hyponatraemia is often due to excessive water ingestion, but may also occur in chronic energy deprivation or diuretic misuse. Urea and creatinine are generally low and normal concentrations may mask dehydration or renal dysfunction. Abnormalities of liver enzymes are predominantly

Neurobiology of anorexia and bulimia nervosa.

Anorexia nervosa (AN) and bulimia nervosa (BN) are related disorders of unknown etiology that most commonly begin during adolescence in women. AN and BN have unique and puzzling symptoms, such as restricted eating or binge-purge behaviors, body image distortions, denial of emaciation, and resistance to treatment. These are often chronic and relapsing disorders, and AN has the highest death rate of any psychiatric disorder. The lack of understanding of the pathogenesis of this illness has hindered the development of effective interventions, particularly for AN. Individuals with AN and BN are co
